    When I was 11 years old (almost 40 now) I barely survived appendicitis. My general practitioner said I just had a belly fever.
    After a few days I couldn't walk or breath properly because of horrible pain. My mom didn't trust it and brought me to hospital. One hour later I was on the operating table with a perforated appendix and peritonitis. After operation it took 3 different antibiotics to get the peritonitis going away. They were almost going to make a zipper in my belly and put me to sleep for 1 week and rinse my belly. Luckily it didn't go that far, because 3rd antibiotic started working. I was in hospital for 1 month. Doktors told me I would've died in a couple days if my mom didn't bring me there. So yeah I'm pretty lucky to be here today.

      when i had appendicitis i think my appendix must have ruptured because the surgeons had to use 8 bags of saline solution to fully get rid of the infection

      I thought mine was ulcer too until my aunt took me to a hospital where the doctor diagnosed it as appendix and recommended an emergency surgery. He said I would have been dead if I wasn't operated on for just a few more days. Currently in my recovery process. Ain't funny but I'm glad for life.

      @@Picozani I had appendix removed at first laparoscopic but then turned to open surgery.yes my appendix ruptured .I had this surgery done 3/31/24 and in recovery process .doctors say 6-7 weeks for full recovery..yes it is not easy but I'm recovering well... thank you lord (this is her husband using my wife laptop)

      You’re fine. The stitches dissolve when it’s been long enough for the scar to heal. There have been lots of research and there are no issues with internal stitches. That’s what you are asking, right?

      The sutures are absorbable so they dissolve over time. You'll be fine.

    As someone who had an appendectomy, RUclips apparently felt it necessary to recommend this video. While I want to learn what was done to me while I was asleep (thank God), I can't stop screaming every three seconds seeing how my body was opened up. Thank you to all the doctors and nurses out there for all you do!

    When I was 10 I had this surgery with the bikini incision. I am 58 now. For the last 20 years I have increasingly intense pain at the surgery scar site that now migrates into the hip area. Any investigation shows up nothing except some calcification of 2 cm in this area. That’s what it feels like. A sharp rock in there. Amy insights Surgeons?

    You said close External Oblique Aponeurosis with continuous vicryl, but both threads were cut. You also said close Scarpa's Fascia with interrupted sutures, but that was done continuously 🤷‍♀

    I wish he'd say a little bit about the instruments, variations in instrument names as well as why it's used a little more. (Kellies, Army-Navy's, Weitlaner's, etc.) Also, he could've differentiated between using a #15 blade to create an opening in the peritoneum in contrast to the #10 blade initially used for the first incision, which is a big no no in aseptic technique. I don't mean to state the obvious but it would be a great point to be made for those that wouldn't know that.

      @@ccoop3774 because the #10 was used on the skin.. even if you clean the skin, anything left on it like bacteria could be transported into the peritoneum if you use the same blade deeper in..

    Love it! Most of the actual videos of surgery-given the limitations of perspective and the resolution of the Camera-all look like a Latexed Finger poking and probing a bloody trench. Your plastic model was much easier to grasp-and I worked in slaughterhouses. I know what blood, bone and internal organs look like. If I can't make heads or tails of a video, then few can. I should have been a Doctor. I could groove on being a Surgeon! What was with those 900-someodd knob-gobblers who gave you a "Thumbs-Down"?

    Well it was interesting to see just what they did inside me when I was 9. Though they stapled my skin shut. They also placed a drain in it, as mine burst, to allow stuff to drain out. wonder if they have one of these for gallbladder removal too?

    I'm glad most appendectomies are done laprosopy now. I had my gallbladder removed laproscopy and I was in pain everytime I had to get up. Imagine had they removed it by opening me up. Same goes for the appendix. I still have mine.

    That's a very nice model. The cut edge of the appendiceal stump is contaminated unless treated with absolute alcohol or phenol. Therefore the forceps that touched it for the umbilication (an unnecessary and potentially problematic step) are also contaminated.

      Some surgeons do not even bury the stump anymore. They just treat with alcohol and leave it as it is.

    In the surgical approach of the abdomen(abdominal wall) and chest (chest wall) the surgeon can cut all the arteries and veins that he/she find in the way .In the surgical access to the abdomen the surgeon try do not section/injure the epigastric artery and in the chest the internal thoracic (mammary ) artery but if he /she cut or injure these arteries,normally nothing bad or adverse happen.Medium /large vessels(arteries or veins) when sectioned have to be ligated ,small vessels can be electrocoagulated with a special cautery scalpel device .Medium and large size arteries and veins have to be surgically repaired .Among the arteries, the aorta, pulmonary artery carotid,coronary, ileo-femoral ,visceral, braquial and other important arteries have to be repaired when sectioned and/or injured . The same policy of repair in the great veins as the vena cava ,pulmonary veins, iliac veins, other .

    Yesterday I had a dream that I was somewhere whit my friends with no medical care in our reach and one of them fell really ill and we somehow figured out its appendicitis. I told them I would do it as I remembered that you have to cut on the right very low abdomen, parallel to the muscle fibers. Then I started to look for some book, to have a clue at least. Then I woke up. It was crazy but I looked this video up anyways, you never know.. :D

      My mum's friend underwent this operation in the bush without anaesthesia in 1968 during the civil war in Nigeria. You never can tell where you need these skills.

    I has this same exact process done to me it started with random pain in the upper half of my stomach then it slowly travelled below my belly button, It was hard for me to sleep, some clinic practitioner gave me pain medicines but the pain returned in morning no injection worked too, he said my ultrasound is clear but the pain was so excruciating that I had to go again in evening when he told me to refer to a bigger hospital in the ct scans they quickly found my appendicitis only I know how I slept that night I was operated and my appendix was burst luckily no infections or anything and my operation wasn't horrible like some people here ( hope they're okay) but I was vomiting water out for a solid 4 days and sleeping at night was difficult I still have stitches there it has been 12 days since my operation I'll get then half cut today according to the doctor let's see but this was a reminded to my parents that something serious can be happening to you while you think it's gas pain

    Throwing stitches everywhere... a lot of other videos don't do a lot of them. In one video they said the purse string is optional. As well as the peretenium... they said you can stitch that if you want because it may help keep things inside while you close. Then they skip right to closing the dermal incision.
    Could be that the vicryl is made to biodegrade... so it's convenient to use more stitches where other videos may have used prolene so you wouldn't want to leave that in except maybe on the actual secum/appandectic ligation?
    Who knows? Not I said the duck.

    what suture do you use for the circundant tissue of the appendix ? chromic or vicryl ?

      Most surgeons use Vicryl 2-0 or 3-0. It all depends on their training. Different areas of the country have different methods.

    Most useful when straightforward, but no mention is made of excess subcutaneous FAT. In many of us today, the adipose layer can be inches thick. Proto-surgeons need to know it can manifestly alter the op technique and the outcome.
    With larger incisions, e.g. exploratory lap, one used to see, post-surgery, the occasional sudden 'burst abdomen' in elderly patients with poor tissue viability; that too has to be understood and assessed - esp. if the op has to be extended, e.g. if bile is found. Hence the benefit of procedures which are less traumatic, esp. keyhole, the minimal breech has reduced risk of post-op rupture. In hernia repair, incorprating synthetic mesh is a boon in wound strengthening, but I digress. Thankfully, some of those old post op issues I used to see rarely occur now. Florence Nightingale's rule ever applies: "The hospital should do no harm"

      You're absolutely right. Open abdominal surgery these days is substantially more difficult than it used to be simply because such a large part of the population is obese. People who think themselves "a little over-weight" aren't; they're OBESE and make the lives of even the most skilled surgeons very difficult.

    I remember when I performed my first appendectomy during my second year residency in General Surgery.My Attending physician timed the procedure.Open to close took 30 minutes.Not bad.

      How about the open one where it already burst. How long it takes to recover? Asking for an 8yr old child.
      Diagnosed very late even after rupture, almost a week after.

    That first vessel was quite thick and important, but they just cut it and left it like this. Wasn't it supplying blood to other organs and now they might be lack of it?

      Zilvinas, these are simulated vessels. They are supposed to be clamped on both side and cut in the middle and either tied off with a suture strand or cauterized. He does mention that these vessels should be ligated with a 3-0 Vicryl. He just didn't do it for times sake.
      This is a teaching video for surgeons, who are leaning this procedure. He is skipping through some of the very basic steps, only for the sake of time, but he does mention what each of the proper steps are. Perhaps you had the sound muted. He gives a great explanation for the entire procedure!

      In case you care a whole year later! There are different levels of importance to your blood vessels. One of the large, named, vessels that are known to supply an organ will not be cut to get it out of the way. Smaller vessels will supply a much smaller amount if tissue and tend to be part of a finer network so the body good at generating replacement blood supply (collateral supply). Think of it as the disruption to the number of cars affected and effect on traffic flow by shutting down a motorway vs a residential road.
      You still want to stop these vessels bleeding as it affects your ability to see what you're doing and no point in unnecessary blood loss.

      @@Comet1808 the comment didn't mean that. They just wanted to know why it was cut. Why it wasn't "reconnected". And the comment after you answered that. Other vessels will compensate. Major vessels are never cut.

    Doc, I have a question, I suffer from irritable bowl syndrome, i feel pressure under my navel all my life, that never goes away, when I do water enemas, I feel much better, usually after I am done, almost dry fecal matter comes out, size of a matchstick, looks old, like it has been somewhere in a pocket somewhere for long, thickness of an earthworm, and every time I do deep enemas, it comes out like it has been hiding somewhere in my large intestine, I am wondering it is what we usually find in the appendix? its has hard consistency, not soft, and long as a matchstick, and thick as an earthworm.

      I will try..I have stopped milk, and bread, I eat veg and rice, When I wear Indian cloths, or any light cloth I do improve...but the pressure is and has always been there.

      pressure comes from gases in your intestine, gases usually came from lactose and milk intolerance and not from irritable bowel syndrome... usually doctors do not tell the difference between irritable bowel and milk intolerance... drink a lot of water and eat apples and do what ive told you about, im sure ur condition will improve

      When I was 4 I drank from dads kerosine he used for cleaning his brushes...negligence I know...however mom says even when I was a baby I had always diarhea...grew up to become handicapped by this..I don't like buss trips...or plane trips because of this...I take long in toa...and this makes me hate flying long distance..i usually fast 24 hours I feel perfect in my tummy but tired overall....

      Milk intolerance, as you call it, is very vague. Lactose (milk sugar) intolerance is often partial as lactase (lactose splitting enzyme) diminishes with age, whereas it may be absent altogether (genetically) in e.g. Asians. Drinking milk in adulthood and being able to assimilate the sugar is an evolved adaptation, seen in people descended from Africans and Northern Europeans. Gas also comes from fermentive processes involving carbs (starches, sugars) and other components of greens and beans. Un-split lactose is broken down by lower gut bacteria. The breakdown products can lead to bloating, gas, diarrhoea, spasm (colic) and discomfort. Don't get your diagnosis from anyone here; see a proper doctor and get checked out if you have GI tract problems. Otherwise, eating fruit and drinking water are generally good tips for health. In the over 50s, IBS is often a misdiagnosis; colonoscopy may reveal diverticular disease. Many other types of colitis or even combos of more than one disease can occur - get proper management from a registered medical practitioner.

    So so so scary, while watching , i can't emagine, when i was in CS operation surgery of my 2 babies ...now my daughter is 19 y/o and my son is 17 y/o...i'm sure is so difficult and so scary when we are CS and also for those surgeon..amazing....All doctor's are instrument to save our lives.God bless to all our hero first of all to our almighty God..

    Les temps de l'appendicectomie sont respectés. Mais, le moignon appendiculaire me paraît trop long.
    Pour moi, le caecum aurait été extériorisé complètement et l'appendice aurait été ligaturé à sa base avant l'enfouissemrnt.. Merci. Pr. JAMES Komlavi Lomé (Togo).
