    2 minutes is my new goal. I am an overthinker and worry about liability. More tips or an actual example would be great!

      Yeah, I’m not even close either. I’m still really new, but 2 min doesn’t seem real. But if she can, good for her!!!

      When she said two minutes I jutted forward, mouth agape. I was taking an hour under ideal conditions…now I’m at twenty or a little less when I don’t strain very hard to be concise… so I feel you…

      I take between 5-7 minutes. Dang! I wish it would only take me 2 minutes to write a note.

    2 minutes per note (BIRP format) or any format is highly unbelievable! 15-20 minutes on a medicated day for myself lol

      ADHD? I have ADHD predominantly inattentive and I heard her say 2 minutes and literally laughed out loud

    Definitely 10 to 20 minutes per note… Partially because I’m a shitty typist and my hand arthritis is making it so much harder and dictation software never seems to work for me. But yes over thinking due to fear of subpoenas. How can anybody do an out in two minutes???

    Yes, it is possible. However when you have supervisors saying "I know this client and he or she won't said that" especially when I was the one who conducted the interview. As a Patient Navigator for Ryan White Program I've learned a lot from pt.'s in the past 6 years. I like the trainings on how to compose the notes what you should place on the progress notes and what you should not.

    I am glad you are taking about this subject. As a patient, I was upset after reviewing my therapist notes on my session with him. I wonder why he didn't record the session to be more accurate. Because what I found was irrelevant information, he talked about what I was wearing. There were session that he fail to record entirely. These notes would have been evidence.. Which would have been a liability issue. I understand he may have many patients and not enough time to document the session. But that's too important not to prioritize.
