  • This video is about Eliquis, a commonly used anticoagulant medicine. The generic name for this is apixaban. Most people with atrial fibrillation have a high chance for stroke, and Eliquis decreases the chance of that happening by a lot. In this video I talk about how I use Eliquis in my Cardiology Practice. Visit my website CardioGauge.com for a personalized assessment of your heart health.

    As a younger nurse, I really appreciate it when people recall previous mainstays and exactly what benefits the newer drug was able to bring to the table. My knowledge on how we managed things 10-20 years ago vs how we manage them today is pretty limmitted, so thanks for adding that to the awesome summary.

Been a little confused about this medication especially ially worried about the side effect of bleeding more easily your video made it perfectly easy to understand and at least made me feel a little bit less worried too thank you. My sister died 4 months ago because she bled out while on Eliquis. Their website says to check with your health care team to see which hospitals stock Andexxa. If you are in a serious car crash, and life threatening bleeding is happening, you will be taken to the nearest hospital. You may not be in any shape to "request" to go to a hospital that stocks Andexxa. My sister was taken to a large San Antonio hospital, that did not have Andexxa. She died from Hematomesis and hypovolemic shock.

I had two stints put in a month ago and my cardiologist has me on 75 mg of Plavix to prevent the stent from clotting. Also, I am taking 40 mg of Lipitor. Of course, I am unhappy about taking this although I have not had any noticeable side effects. I am a 54-year old otherwise healthy man who has been a vegan for 6 years (Dean Ornish diet). I have a genetic variant that causes Lp(a) to be HIGH no matter what I eat. I assume that this is why I had these blockages in my right coronary artery as it couldn't possibly be diet-related. • @CardioGauge

      Hi John, The main side effect of Eliquis is bleeding of any sort. Nosebleeds, blood in the urine, blood in the stool, and bruising (which can be massive), or bleeding into the brain. While all that stuff sounds absolutely terrible, in my experience major bleeding is rare. I have about 200 patients in my practice on Eliquis. People anywhere from 50 yrs old to 95 yrs old. Major bleeding happens somewhere around 1-2% per year. Rash happens in about 2%. I have had a couple patients with stomach upset. I have never seen it cause kidney or liver damage, or nerve damage.
      When dealing with major blood clots, the best treatment is anticoagulation with Eliquis or similar medication. How long you take it relates to the specifics of your case, but is generally from 3 months to indefinite. Hope this helps, and best wishes.

He goes to the doctors regularly and always comes back with a healthy outcome. A month ago he was diagnosed with blood clots in his leg and rushed into surgery. He almost lost his leg but caught it in time. My question is why wasn't this caught by his doctor a long time ago? He just had knee surgery in 10/2019.

    • @CardioGauge
      @CardioGauge  4 года назад +6

      Hi Rochelle. Sorry to hear about the experience your husband just went through.
      It sounds like he had blood clot in the artery of his leg, rather than in the vein, since you mention that he could have lost his leg. This distinction is important because clot in the artery happens for different reasons than clot in the vein, And also these two situations are treated differently.
      When a person develops sudden severe symptoms in the leg due to a blood clot in the artery, this is generally not something that can be predicted by the doctors. Because your husband has a history of coronary artery stent, he almost certainly has cholesterol plug up in other arteries, for example the arteries of the legs. This is treated with the same medications used to treat coronary artery problems, so he likely was already medically treated for cholesterol buildup in the arteries of the legs.
      Sudden clot in an artery of the leg can happen if a blood clot developsDirectly in the artery. It can also happen if a blood clot forms inside the heart due to atrial fibrillation, and then pops loose and goes to his leg. I presume he does not have a history of atrial fibrillation. Sometimes in my patients who developed sudden clots in the leg, I will check them for atrial fibrillation with a heart monitor.
      However, to address your direct question, why was the clot not caught sooner, it is because we generally do not do any studies of the arteries of the legs, unless there are major symptoms. If he had been doing well without significant symptoms in his legs, the doctors would rightfully have not been checking for clots or cholesterol plug up.
      Best wishes for you and your husband!

      Hi Swompy, I cant directly tell you what to do because this is RUclips, but I can tell you what I typically do for my patients in this Situation. Some patients who have coronary artery bypass grafting have left atrial appendage excision at the time of surgery (which makes Eliquis or similar medication optional). However, this is mostly done in people who have already had an episode of atrial fibrillation. Since it sounds like you only had atrial fibrillation after the operation, they probably did not do the left atrial appendage excision. Anyway, it's standard to give you Eliquis for the episode of atrial fibrillation. After ulcer, We usually wait for the G.I. doctor to clear the patient, as has been done with you. It is reasonable to Go on Eliquis, I have numerous patients for whom I have done this, after the occurrence Of ulcer. For my patients with coronary artery disease and atrial fibrillation, I generally use Eliquis alone, rather than Eliquis with aspirin. This is especially appropriate in cases where people are on Eliquis and aspirin, and then develop an ulcer. If you were on Eliquis and aspirin when you got your ulcer, Being on Eliquis alone could be a good option. For people with atrial fibrillation who have a second ulcer with Eliquis or other anticoagulant, I consider the watchman device. Best Wishes

      the limited research on combining SSRI (sertraline is an SSRI) and medicines like Eliquis is reassuring.
      There are case reports of hemorrhage around heart in patients on Eliquis and SSRI, however, I have never seen this, and the patients in the case report had kidney trouble, which would more likely be a causative factor than the SSRI.

      Gloria Mary Haywood hi Gloria, I have many patients on Xarelto. It is an excellent medication for preventing stroke. I have a slight preference for Eliquis but this is not based on specific trouble with Xarelto but rather on some research that is not very direct or conclusive. In New York where I practice, some insurance companies give a good deal on one or the other medication, so we use whichever one (Xarelto or Eliquis) is less expensive. I have almost no patients on Pradaxa, it does not appear to work as well at decreasing stroke risk. So even though there is an antidote, what is the point if it doesn’t work as well.
      If I had a patient who was tolerating Xarelto, I would not switch him or her to Eliquis. Hope this helps!

    Do you recommend this for an 83 year old man, as my dad has been on Warfarin for years and his doctor seems to be determined to put him on these next week. Im a bit worried as Warfarin your checked but this your not, after reading all the side affects im wondering is it more dangerous to change at his time of life and take a gamble that he wont suffer any of the side affects. As there is an antidote for warfarin but not for this and he weighs about 56 kilos. His also on Tamsulosin finasteride metro-pol 1/2 a day and now Vitamin D as his not been out for a year due to covid his isolated, which also means no real exercise he eats well but his weight stays the same, he had a fall in a restaurant 3 years ago and his now half the weight he was but the weight stays the same. Because your not paid for this i was hoping you could give an honest reply, as Ive tried asking all these questions to his doctor but he fobs me off, I just dont want my dad to die unnecessarily just because they want to change his drugs, just to save costs.

    • @CardioGauge
      @CardioGauge  3 года назад +1

      Hi Julie, sorry for delay in reply. the bottom line is: For my elderly patients who have been stable on warfarin, who have not had stroke or major bleeding episode, i generally leave them on warfarin rather than switching them. I consider it an "if it aint broke dont fix it" situation.
      However:
      If my patients are having trouble with getting the blood tests done, or their INRs are all over the place, or there is some other substantial issue that comes up, then I will readily switch them over to eliquis, which I use frequently.
      For most of my patients who are new to atrial fibrillation (which I presume your father has), I will start them on Eliquis, because, in general, it is easier to take and slightly more effective (for the average person) than warfarin.
      With your father's very low weight, I do wonder about what the level of Eliquis will be in his system. The proper dose for him would be 2.5 mg twice a day, that would be a textbook dose.
      I think his doctor's plan to switch is reasonable. I think many of my peers would agree with it. In terms of costs, the decision probably does not relate to that. The eliquis costs a lot, typically. For most people the warfarin costs way less, less for the patient, less for the insurance company. So the doctor is not recommending a switch to save money.
      Lastly, there indeed is antidote for eliquis now, so I need to update the video!
      Either course of action is reasonable.
      Best wishes for you and your father!

    Thanks a lot for your VDO's, very educational. I am very interesting in your opinion about the Keto-diet being beneficial for preventing or lowering the chance of getting an Heart event. I believe the theory behind it is as follows: By lowering your carbs intake significantly (< 50 grams per day) you decrease your insulin-level, which then reduces the level of inflammation, which then reduces damages to your arteries and thus reduces plaque development in your arteries and so decreases the chance of an heart event. Do you agree with this theory? Is there any evidence of this theory? Would you recommend your patients to go on a Ket-diet? Thanks a lot!

    • @CardioGauge
      @CardioGauge  4 года назад +1

      Hi Erik, this is an important question that comes up frequently, and I don't have a clear answer for you. Weight loss by basically any means increases a person's sensitivity to insulin, decreases insulin level. The improved sensitivity to insulin decreases inflammation by multiple mechanisms (www.ncbi.nlm.nih.gov/pubmed/18407031). There is good scientific logic that Keto should decrease chance of heart event. However, this idea is not conclusively proven. It can only be studied observationally. In the Nurses' Health Study and Health Professionals Study, low carbohydrate diets' effects on mortality was studied. Taking all low carbohydrate diets together, there was a slight increase in mortality (Dr Atkins had a lot of heart disease). Low carbohydrate diets that emphasized plant foods, however, had lower all cause mortality (20% relative decrease) compared to the average diet of the people in the study (www.ncbi.nlm.nih.gov/pubmed/20820038). What I take away from this is that if a person is going for a low carb diet, plant foods should be chosen as much as possible. While a vegan keto diet is possible, it is not for the faint of heart, I don't think there is any way I could do it and maintain my level of function (www.healthline.com/nutrition/vegan-keto-diet). Throw in some fish and chicken here and there, and it might be more doable.
      I think a plant focused keto approach would be superior to a meat based, Atkins type low carb diet.
      I have many patients who have had success losing weight on keto, and I believe that it helps them reduce their risk, however I do not know this for certain. My go-to diet for people who have had a heart attack or coronary stent is the whole food plant based diet promoted by Caldwell Esselstyn (Prevent and Reverse Heart Disease), Dean Ornish, and Colin Campbell (The China Study).
      I personally avoid foods with added sugar (this includes honey and natural syrups), so no ice cream / pastries / donuts / juices / soda / etc etc. I eat a lot of fruits and vegetables and nuts. This helps me keep a healthy weight and I really could care less about the sugar. I don't avoid animal foods entirely by any stretch but I will eat lacto vegan 2 - 3 days a week.
      I hope this answer helps you some, best wishes!

So I believe I am very insulin sensitive. I more or less eat in the same way as you personally do, maybe I eat a bit more chicken, fish and fruits to be able to come to 4000 kcal. Thanks again!!

    Why not Garlic Oil? It is the best blood thinner you can take and very safe..

    • @CardioGauge
      @CardioGauge  4 года назад +3

      Hi Lucius, Eliquis is mostly commonly used in a specific medical condition called atrial fibrillation. The top chambers of the heart (atria) are not squeezing properly, instead, they are wiggling. because of this, blood moves sluggishly through the heart and when blood slows down, it can form a blood clot, which is a glom of solid blood. The clot can shoot into the brain and block an artery (blood vessel) and cause a stroke which results in permanent brain damage. To prevent blood clots like this, a strong blood thinner is needed. Garlic is a very weak blood thinner, and not strong enough for the job! So while garlic tastes great, and in general is good for you, and is a weak blood thinner, it is definitely not sufficient to take the place of a medicine like Eliquis. Thanks for your question!

    • @luciustate4386
