Boomhauer is incredibly intelligent. He thinks way faster than he talks, a common trait for speach impediments and stuttering. So in his mind this is how he hears himself, and everyone else seems to be the ones that aren't speaking correctly.

I grew up with a Boomhauer. He’s a somewhat normal guy by the name Wyatt Justice. The transformation begins with the purchase of at least a 12 pack of a high end beer such as Old Milwaukee. After about the sixth beer you can hear the Boomhauer speech pattern start to appear. Full transition at around 12 beers. We always laughed about it but Wyatt would say his speech never changed. When my son became old enough I allowed him to witness this phenomenon. When I introduced them Wyatt was in full Boomhauer mode. My son was impressed and then Wyatt (Boomhauer) iced the cake. He said in full Boomhauer with a big grin “I bet you never met nobody like me before”. My son said later and still says to this day that he has not.

As a kid that grew up in Houston, I had no clue what Boomhauer was saying. As an adult in Iowa, I can have a conversation with him without saying "what". And that says alot.
It just dawned on me that both Boomhauer and Hank both say "I tell you what".
Maybe that's why Hank can understand him. Boomhauer has a 10/10 accent while Hank has the same accent but only 2/10.
