Where I live in Colombia, sildenafil 50 mg is available over the counter in any pharmacy for less than 50 cents USD per tablet. This is the promised land.

Sildenafil is awful for me, it gives me terrible headaches at the minimum effective dose. On the other hand, Tadalafil is awesome, 5mg last for the whole weekend without side effects, other than minor nasal congestion
I was having diminishing returns from Sildenafil at max dose and Tadalafil at 10 mg works so much better and requires less planning than Sildenafil because on Sildenafil you have to have not eaten for 2 hours so it might be 3 hours before I was ready with Sildenafil.
Never needed any of these pills for the last 20 years. What I need is a pill that creates a willing partner :) Never be ashamed or upset you need to take a pill, at least you have need of them!
How does this explanation fit with those of us that have had Prostate Cancer and were treated with radiation? Sildenafil has a tendency to create severe cramps in my thighs if used with a dose more than 75mg and that is a stressor for me. Thanks for the excellent video, Dr. Malik...
I was taking daily Tadalafil 5mg for awhile and noticed that its effectiveness decreased a little over time. So I have 10mg Tadalafil to take on days with the lady. Another thing I’ve done, which I wouldn’t recommend, unless you check with your doctor first, is supplement the daily Tadalafil with L-Citrulline or beet root capsules. When I do that I don’t need the 10mg at all because the nitric oxide boost turns me into a superpower. I’ve never had any negative side effects doing this. The only thing that gets annoying with nitric oxide boosters is they make you urinate A LOT. But it’s worth putting up with for the slamming erections. I don’t need a lot of the nitric oxide booster either because I’m already taking Tadalafil. 1500-2000mg of L-Citrulline or beet root capsules will fix you right up. CONSULT WITH YOUR DOCTOR FIRST if you have low blood pressure issues.

When I lived in Oman my PSA spiked again at 7.2. I have enlarged prostate (77ml now), prostatitis/BPH and Gleason 3+3 PC on right mid apex (have had 4 biopsies and 5 MRI). Saw Urologist in Dubai and he performed multiple MRI. Prescribed Tadalafil saying it might help with urine flow. I did not know it was used for ED as well. I started to notice a ringing in my ears caused by it after a couple weeks. And now, I have tinnitis since I stopped taking it after only 2-3 weeks in 2016. Fine print on adverse reactions on these meds even says can cause deafness.
I had to stop taking Tadalafil because it seemed to be causing tinnitus and deafness, which lessened in severity after I stopped taking Tadalafil, but some still lingers.

I'm 76. Three years ago I was in very poor general health. I was considering taking the ED pills. Instead, I abandoned the Standard American Diet and switched to eating the Carnivore diet way. Meat, eggs, butter, salt and water. That's it. Now my health is excellent and I have no need for any meds whatsoever. I'm good in the bedroom now and have no performance worries or fears. I'll never go back to eating those nasty, inflammatory sugars, carbs, or any other "food" from plants. We don't have medicine deficiency, we are malnourished. Eat the Proper Human Diet and enjoy a life filled with pleasure.
From experience - these pills do not work if excitement is replaced by anxiety (you start thinking about something unpleasant that can happen) or your little friend gets a rubber cap (which acts as a tourniquet)

Unlike most people, I suffer from low blood pressure. I was told I have the BP of a pregnant woman. Though I'm 75, I exercise vigorously daily. I was taking nitric oxide for a couple of years until I change to L-Citrulline 3 times a day. It's not like being 20 but I wake up most mornings with a strong tent pole. Don't have a reason to employ a tent pole so not sure if the need arose if I could complete the task. I would like to find out. LOL. I don't think I'd have any ED with Dr. Malik.
60+ guy here - works best for me if I take 1/3rd dose 3 hours before 'event', 1/3rd 2 hours and 1/3rd 1 hour before. I accidentally found this out when I took a half dose because we might have to postpone, which we did, and then I took the other half and we waited an hour - and it worked great. Trial and error - ended up best result with 1/3rd an hour apart and sort of 'build it up'. But avoid any 'stimulation' during the 3 hours - that weakens the affect.
Interesting but a bit more work than wanted. You didn't mention the dosage or which med but couldn't you do, for example, a 5 mg cialis pill at 3 hours before, then another 5 mg 2 hours before, then 1 last 5 mg one hour before?
For me, Sildenafil was not usable because of the terrible headaches - migraine level - that it caused. My doctor put me on a daily maintenance dose of Tadalafil and BOOM. I even wake up with erections like I used to when I was 10 years younger. And the added benefit of daily dose it that I never need to worry about timing. I’m always ready. Apparently it stays in your system up to 36 hours.
I get Tadalafil by mail. The product is called Blue Chew, and it's a blue pill that you chew up. I'm ready for action in half an hour. I also smoke and drink beer every day. I think chewing it up helps it get there faster. But I wouldn't do that with a normal Cialis or Viagra. Blue Chew is formulated like a candy, it's sweet and tastes great when chewed. Not a sponsor.

Sildenafil is also used in both men and women to treat the symptoms of pulmonary arterial hypertension. This is a type of high blood pressure that occurs between the heart and the lungs. When hypertension occurs in the lungs, the heart must work harder to pump enough blood through the lungs.
After a few years of no sex life due to my wife's illness (from which she died), when I remarried I found that the old organ wasn't grinding any longer. Since I was 64 at the time, I thought that age was the problem and that I'd need a blue pill to overcome the problem. But I found that I had to take a higher dose to get things working. Fortunately, after a period of more regular relations I found that I didn't need the pill any longer. I'm not sure if a long period of abstinence was the cause of the ED, though. Might have been something else, but I have no clue what it might have been. On a somewhat related note, I later had a DNA test done for genealogical purposes, but amongst the analysis of my chromosomes it turned out that one of my characteristics was a genetic resistance to sildenafil! This explained the need for a larger dose.
What if I've been on 20mg daily of Escitalopram for years? What effect might this have on the efficacy of Sildenafil? I'm one of those people who tried Sildenafil once and it didn't work and I gave up. I'm highly encouraged by the data you present in this video so I now plan to try a few more times. But I've always felt that the Escitalopram, which has already kills my libido, was at least partly responsible for my ED. If you take your sildenafil with a glass of pure grapefruit juice 50 mg will act like 100 mg. Grapefruit juice is a strong inhibitor of the cytochrome P450 hepatic metabolic pathway which is how PDE5 inhibitors are metabolized.
I took viagra till it no longer worked , then switched to cialis , until that no longer worked . Been injecting with trimix for 6 years now . I'll be 50 next year , so if that stops working , I'm officially retired from sex . The injection is a cruel monster. On one hand you get a boner like you had when you were 18 and you can pound for hours . On the other hand , you don't nut . It's like running a marathon but stopping short of the finish line every time.

I take 5mg of Cialis and it works great. I find if I “ load in” it works even better. Take it during the afternoon and then take another 5mg about an hour before bedtime. Works like a charm. Then I’ll take another 5mg 24 hours later depending on when/often I’m seeing my fiancee. If we’re going to be together a whole weekend or week away, I’ll take one every day or even every other day. Cialis has great residual effects too, even if I don’t take another one I’m still seeing benefits 2-3 days later. I tried Viagra and it was not good for me.
ED meds are being marketed to younger and healther men so inhave a question. Can your body build up a resistance to ED medication? Or in other words, assuming a man is considered generally healthy, is there a downside or risk to taking ED medication when it comes to erectile health?

Some say that ED medications stop working or they have to switch to something else later but my question is are there other outside factors such as poor health that also contribute to a particular medication no longer working or does the medication actually stop working.

Two things you didn't point out: 1.) After 100 mg, there isn't any increased effectiveness. In fact, taking 200 or 300 mg could cause dangerously low blood pressure and a stroke 2.) These medications lose their effectiveness over time usage.
I think some people's body developes a tolerance for Viagra or Cialis. I started out using Viagra many years. It worked fine at the start and gradually became ineffective. I then asked my doctor if I could switch to Cialis and viola it worked like a charm for 3 to 4 days. Now many years later I notice that Cialis is taking longer to work and instead of 3 to 4 days effect, its down to 1 to 2 days effective. And I am an occasional user, a 4 pill packet will last me 1 month.
I take this once in a while. I can stand up tall and hard but keeping it up is a little more difficult then it used to be. I have HPB so I use the little blue pill sparingly and I take things to offset the side effects. I have taken Cialis but the insurance pays for the other one. I prefer Cialis as the side effects are not so harsh. Taking the blue pill messes with my BP and my joints ache for a day or two after I take it.
Now there's an idea, make sure to try and match the wood color to your skin tone, assuming your uncircumcised as I can't recall ever seeing PINK wood!!!
Micro dose 5mg of Cilias daily, and then take full does when you know sex is in the near future, and unless you have serious ED you'll enjoy the benefits, and it will also help with BPH issues if you're dealing with that, getting up to pee 6 times a night. Micro dosing is the way to go. And yes, get your butt in the gym, and clean up your nutrition.
I've heard alot of positives about 5mg per day. I do 5mg daily but I dont really notice any difference. With or without I can perform but its about 65-70% of what it was (hardness) when young. Can still penetrate and have intercourse but when done, it goes flaccid very quickly. I'm 62 and really dont notice the difference being on 5 mg per day. I only write this because nobody else seems to say this but I suspect there are others.
@usaman7358 it doesn't work for everyone. In addition to the micro dosing, try a full dose on the day you know you're going to have sex. I think a full dose is 20mg. Maybe work up to that 5mg at a time, see what works.
A urologist recently prescribed me 5mg Tadalafil for bph symptoms ( feeling of not completely emptying bladder n frequency ) it did help but on the third day i developed really bad lower back pain.
So, to be clear, in some cases, part of the lack of result with the medication has the same root causes than the issue itself and people should start working on these causes BEFORE trying these medication ?

A medical doctor is only part of the solution. They can help with the mechanics of the issue, but the emotional or psychological aspects need a specialist. A percentage of men with ED do have mechanical issues that a pill may help with. But today in what we call "first world countries" the social issues are far more of an issue than pretty much any MD knows. As only the words were used in this video about arousal, the state of being aroused in a man is becoming more and more difficult with the societal and political issues as well as financial and other stresses that are being over looked when diagnosing ED. For those of you that watch a lot of youtube, I encourage you to find the old Johnny Carson interview with an old farmer from Illinois. Merritt Heaton on Johnny Carson. It's a good laugh, but it touches on the difference of arousal then vs. now.
Funny, i think “the mind” plays a bigger role than the pills. I only have erectile dysfunction with women I’m not attracted to. And the pills don’t work. I don’t have any problems when I’m with women whom I’m attracted to. So, I don’t need pills. So, maybe I don’t need drugs, I just need to set my bar higher.
Any E.D. meds I've gotten have a 1 year limit printed on the label. Like most prescriptions though, they'll still work but at a lesser degree. That's been my experience.
What about the pain I get during the night trying to sleep. When I get up I cannot straighten up because of the pain in my lower back. I also have pain in the front just at the lower stomach between my legs. No doctor has explained that the pain. Of course, I have all the symptoms of an enlarged prostate. I’m also a LADA 1.5 diabetic caused by an autoimmune started when I was 57. Although the pain subsides during the day, it’s there but not as intense when I wake up in the morning.
My problem is not that it doesn’t work but the after effects it has on my body. Headaches and joint pain occurs for days after taking these medications. These after effects are not worth the short happy endings.

I found I need to I take Sildenafil on an empty stomach for it to have any effect, so I do not eat anything at least 4-5 hours before taking it.

Certain foods especially saturated fats interfere with nitric oxide production. Other foods such as beets and arugula increase nitric oxide production

I developed a hearing problem. I learned that hearing problems can be caused by sildenafil. Since then I stopped sildenafil.
My PCP is an NP it is tough to bring these kind of issues up. You helped me breach this issues thank you.

After a few years of no sex life due to my wife's illness (from which she died), when I remarried I found that the old organ wasn't grinding any longer. Since I was 64 at the time, I thought that age was the problem and that I'd need a blue pill to overcome the problem. But I found that I had to take a higher dose to get things working. Fortunately, after a period of more regular relations I found that I didn't need the pill any longer. I'm not sure if a long period of abstinence was the cause of the ED, though. Might have been something else, but I have no clue what it might have been.
On a somewhat related note, I later had a DNA test done for genealogical purposes, but amongst the analysis of my chromosomes it turned out that one of my characteristics was a genetic resistance to sildenafil! This explained the need for a larger dose.

What if I've been on 20mg daily of Escitalopram for years? What effect might this have on the efficacy of Sildenafil? I'm one of those people who tried Sildenafil once and it didn't work and I gave up. I'm highly encouraged by the data you present in this video so I now plan to try a few more times. But I've always felt that the Escitalopram, which has already kills my libido, was at least partly responsible for my ED. If you take your sildenafil with a glass of pure grapefruit juice 50 mg will act like 100 mg. Grapefruit juice is a strong inhibitor of the cytochrome P450 hepatic metabolic pathway which is how PDE5 inhibitors are metabolized.

I took viagra till it no longer worked , then switched to cialis , until that no longer worked . Been injecting with trimix for 6 years now . I'll be 50 next year , so if that stops working , I'm officially retired from sex . The injection is a cruel monster. On one hand you get a boner like you had when you were 18 and you can pound for hours . On the other hand , you don't nut . It's like running a marathon but stopping short of the finish line every time.

I take 5mg of Cialis and it works great. I find if I “ load in” it works even better. Take it during the afternoon and then take another 5mg about an hour before bedtime. Works like a charm. Then I’ll take another 5mg 24 hours later depending on when/often I’m seeing my fiancee. If we’re going to be together a whole weekend or week away, I’ll take one every day or even every other day. Cialis has great residual effects too, even if I don’t take another one I’m still seeing benefits 2-3 days later. I tried Viagra and it was not good for me.

Some say that ED medications stop working or they have to switch to something else later but my question is are there other outside factors such as poor health that also contribute to a particular medication no longer working or does the medication actually stop working.
I noticed my body develope a tolerance over the many years of occasional use. I got use to Viagra within 3 yrs, then I switched to Cialis. Now Cialis is taking long to work and the duration of effectiveness is shorter. From 3 to 4 days in the past and now its 1 to 2 days effective.

I'm one of the lucky ones. I take 60mg of sildenafil and it's working in 20 minutes, sometimes even sooner.
It stays working for 12 hours easily.
Two things you didn't point out: 1.) After 100 mg, there isn't any increased effectiveness. In fact, taking 200 or 300 mg could cause dangerously low blood pressure and a stroke 2.) These medications lose their effectiveness over time usage.
My urologist said they don't lose effectiveness but i keep seeing people say they do

I think some people's body developes a tolerance for Viagra or Cialis. I started out using Viagra many years. It worked fine at the start and gradually became ineffective. I then asked my doctor if I could switch to Cialis and viola it worked like a charm for 3 to 4 days. Now many years later I notice that Cialis is taking longer to work and instead of 3 to 4 days effect, its down to 1 to 2 days effective. And I am an occasional user, a 4 pill packet will last me 1 month.
They definitely lose effectiveness. I was on viagra , till it stopped working. Then on to cialis , a few years later , that stopped working as well.
They work, I just have to rotate from one to another from time to time for whatever reason.
I take this once in a while. I can stand up tall and hard but keeping it up is a little more difficult then it used to be. I have HPB so I use the little blue pill sparingly and I take things to offset the side effects. I have taken Cialis but the insurance pays for the other one. I prefer Cialis as the side effects are not so harsh. Taking the blue pill messes with my BP and my joints ache for a day or two after I take it.
In other words, "Just DO it". And keep doing it.

Micro dose 5mg of Cilias daily, and then take full does when you know sex is in the near future, and unless you have serious ED you'll enjoy the benefits, and it will also help with BPH issues if you're dealing with that, getting up to pee 6 times a night. Micro dosing is the way to go. And yes, get your butt in the gym, and clean up your nutrition.

I've heard alot of positives about 5mg per day. I do 5mg daily but I dont really notice any difference. With or without I can perform but its about 65-70% of what it was (hardness) when young. Can still penetrate and have intercourse but when done, it goes flaccid very quickly. I'm 62 and really dont notice the difference being on 5 mg per day. I only write this because nobody else seems to say this but I suspect there are others.
@usaman7358 it doesn't work for everyone. In addition to the micro dosing, try a full dose on the day you know you're going to have sex. I think a full dose is 20mg. Maybe work up to that 5mg at a time, see what works.
A urologist recently prescribed me 5mg Tadalafil for bph symptoms ( feeling of not completely emptying bladder n frequency ) it did help but on the third day i developed really bad lower back pain.
Under the tongue. Good to go in 5 mins.. also lowers the blood pressure.
With me it’s mostly in my head. If it’s a new girl I will normally pop a pill for the first time or two after that I’m good.

No. Only misogynistic idiots would systematically blame their partners for a problem of their own cause by their inability to reflect on the changes required in their way to deal with the problem.
A medical doctor is only part of the solution. They can help with the mechanics of the issue, but the emotional or psychological aspects need a specialist.
A percentage of men with ED do have mechanical issues that a pill may help with. But today in what we call "first world countries" the social issues are far more of an issue than pretty much any MD knows. As only the words were used in this video about arousal, the state of being aroused in a man is becoming more and more difficult with the societal and political issues as well as financial and other stresses that are being over looked when diagnosing ED.
For those of you that watch a lot of youtube, I encourage you to find the old Johnny Carson interview with an old farmer from Illinois. Merritt Heaton on Johnny Carson. It's a good laugh, but it touches on the difference of arousal then vs. now.

Any E.D. meds I've gotten have a 1 year limit printed on the label. Like most prescriptions though, they'll still work but at a lesser degree. That's been my experience.
So, what is the recommended dosage for Tadalafil, mine is 10 mg.
What about the pain I get during the night trying to sleep. When I get up I cannot straighten up because of the pain in my lower back. I also have pain in the front just at the lower stomach between my legs. No doctor has explained that the pain. Of course, I have all the symptoms of an enlarged prostate. I’m also a LADA 1.5 diabetic caused by an autoimmune started when I was 57. Although the pain subsides during the day, it’s there but not as intense when I wake up in the morning.

My problem is not that it doesn’t work but the after effects it has on my body. Headaches and joint pain occurs for days after taking these medications. These after effects are not worth the short happy endings.
Tried once. Didnt work, but gave me horrendous hangover the next day.
