Anterior shoulder dislocation Animation : Orthopedics Lecture
Anterior shoulder dislocation, the most common type of shoulder dislocation, typically occurs when the arm is forcefully abducted and externally rotated, often seen in sports injuries or falls. The humeral head dislocates from the glenoid fossa, moving anteriorly and inferiorly. This dislocation can damage the anterior glenohumeral ligaments, the joint capsule, and sometimes the rotator cuff muscles, particularly in older patients. Clinically, patients present with pain and a squared-off shoulder due to the dislocation of the humeral head; the arm is often held in slight abduction and external rotation. The axillary nerve can be at risk, so sensory function in the regimental badge area should be assessed. Diagnosis is typically confirmed with an anteroposterior X-ray, which shows the humeral head out of the glenoid fossa. Treatment involves reduction of the dislocation, often with sedation and specific maneuvers like the Kocher or Hippocratic method, followed by immobilization and physiotherapy. Recurrent dislocations can occur, especially in younger patients, and may require surgical intervention. Understanding the mechanisms, clinical presentation, management, and potential complications of anterior shoulder dislocation is crucial for effective treatment and patient care in clinical practice.
