Mounjaro (tirzepatide): The newest diabetes and obesity medication (explanation and overview)

  • Tirzepatide (brand name: Mounjaro) is the newest injectable medication for the treatment of type II diabetes and obesity. Mounjaro was recently approved for diabetes and is awaiting FDA approval for the primary treatment of obesity. Based on the results of the SURMOUNT trial, tirzepatide will likely be the most effective obesity medication EVER. This once per week injection produced an average weight loss of nearly 21% of body weight at 72 weeks.

    I am 60 year old male who’s battled weight issues for the past two decades. Been on Mounjaro for seven weeks and have lost 29 lbs. Starting 7.5 mg on Saturday. I am exercising (cardio) 6 days a week and light weights twice a week. Eating about 1,200 calories daily. Only side effect I have had is minor constipation. Mounjaro has a great appetite suppressant. Excellent medication.

      So I’m curious if you ever tried keto and do you avoid sugar and rice and high carb foods?

      @@geauxp as a matter of fact, I have eliminated all added sugars and fast digesting carbs from my diet. I limit myself to 30 or less grams of sugar and 100 or less grams carbs daily. I am up to 51 lbs lost in the past 3 months.

      @@reyvaldes611 wow that's alot of carbs. When i started it was under 50 carbs (not net, just carbs of any kind) a day and then when i got my a1c under 5 i raised it to 100 carbs a day. A1c is now 4.7. Check out Ken Berry on youtube.

      @@geauxp I’ve tried keto & lose nothing unless fasting extensively. I just can’t do that much fasting any more. Keto doesn’t work for weight loss for everyone, but I do think it’s the best way of eating

    I’m taking 2.5mg and already lost 21lbs in 15 days.
    Started at 248 on 7/15/2022 and 8/1/2022 I’m 227.0
    I don’t feel hungry or get jittery for snacks. Can’t wait to see more progress :)

      @@lanceguerin4781 I stopped using door dash lol! I order my meals through PK prep kitchen and stay under 400 calories per meal. I eat 2 of the meals so roughly 800 calories a day. I also bike for 30 minutes every morning. I will incorporate slow jogging to build a better sweat than what biking provides

    I just started Trizeparide Mounjaro and it is a game changer. I have an A1C of 7.6 so I asked my doctor to put me on this drug. I am amazed. For years I could not control the food noise. On this medication I have total control of my eating and it kills your appetite. I have no side effects but I am still on only 2.5. I strongly think anyone that is obease or has diabetes should seek out this tool.

    I just took my first dose of Mounjaro - 2.5 mg. I'm not expecting great appetite suppression yet but I'm going to give it my all. I hope to work up to the max dose to really get my weight loss rolling - I have just over 100 lbs to lose. If all goes well my doctor said I can take the med the rest of my life (I'm 65). That's comforting as I really have issues with self control when food is around.

      The highest dose doesn’t produce more weight loss in fact less it’s strongest most effective dose for diabetes i dropped back down from it aft 6th injection to 12.5mg + it overactived my thyroid…(15mg)…

    Great video doctor. I was on monjarno for about 4 months. I just took my last injection of 7.5 mg. I did loose about 20 lbs and lowered my A1C from 6.2 to 5.0. Even with all of the benefits of the medication and taking anti nausea medicine, I can't take it anymore. The constant nausea is too much. I couldn't have a routine endoscope done because of old food was still sitting in my stomach 2 days prior!! Good luck to everyone else who can endure!

    Been looking into tirzepatide & found this video. I kept thinking this doctors face looks familiar… and then it clicked. Back in Nov 2019 Dr. McGowan perform on me the ESG procedure in Raleigh, NC. The procedure was successful in a way, it helped me with my A1C & I was able to maintain a normal range of BS. But I didn’t loose as much weight as I wanted. Still, I consider it as successful. My heaviest weight, prior to procedure was 316 lbs, currently weighting between 265-270. My BS is starting to change. I try my best to keep it under control, but it seems my body is not cooperating with me. My endo recommended Wegovy and I loved it, use it for a while, but my insurance won’t cover it. Soon I have an appointment back with my Endo, and I really want to see if I can try tirzepatide.

    Insurance companies have a shortsighted view of pretty much anything that’s good for patients. I think this is amazing and I hope that more people who need it can have access and take advantage of these medications.

      I agree with you that insurance companies have a very shortsighted view and they are really missing a lot and I don’t understand why there is no mention of eating a low-carb on all of these comments or at least I didn’t come upon them yet. I’m trying to read all the comments.

    This was extremely helpful and informative! During the pandemic I gained a significant amount if weight and has had trouble ever since losing it. I just started my mounjaro journey today and am really excited to get back to my old self. I had minor success with saxenda a few years back but that was for a trial study so I only used it for a limited time.

      I gained 40lbs during that pandemic after losing 20 just before. Ughhh I'm on Ozempic right now. I think the biggest issue for me is just getting control over my hunger signals because I still don't understand how some people just "forget" to eat. Usually 5-6 hours without food and my stomach is forming a conga line and it's worse if I do weight lifting which I have been for the past 4 months. So far it's working, but it's definitely a bit weird getting used to not having the cravings an hunger signals I'd normally get.

    About to start week 4 tomorrow. I've lost 21 pounds in roughly three weeks. I have PCOS with insulin resistance, which makes losing weight extremely difficult. The weight is falling off now! My cravings are basically gone and I'm trying my best to follow a low carb diet. It's a shame that insurance doesn't cover this medication. I do worry when the manufacturer's coupon expires next year. I'm hoping to get to my goal weight by then.

    Well, the Semaglutde study was 52 weeks with 15% loss. The Tirzepatide study was 72 weeks for about 20% at max dose. Doing the math it seems about even.

      There was a head-to-head study, SURPASS-2, comparing semaglutide 1mg QW vs tirzepatide 5mg/10mg/15mg QW in diabetes patients. For body weight loss, tirzepatide 5mg QW is superior than semaglutide 1mg QW at 40 weeks. (semaglutide 6.2 kg vs tirzepatide 7.8 kg/10.3 kg/12.4 kg for 5mg/10mg/15mg, respectively)
      Now the study was performed on diabetes patients, and they didn't give semaglutide at the maximum dose (3mg QW), so take that with some grain of salt.

    I was 360lbs when I started. It has been 3 months. I am now under 270lbs.
    Never has anyone lost weight as quickly as me. I didn't diet or exercise. The weight just fell off like a lead balloon.
    This medication is amazing to me personally
    However , I think it causes Cancer.

    I just took my first 2.5 dose two days ago and in the morning I had minor nausea and vomit which resolved as soon as the vomiting was completed almost immediately my appetite was suppressed now the challenge is to make sure I eat enough calories to maintain health and to drink a sufficient amount of water I'm looking forward to the results

    I am on my 4th month and I'm on the 7.5 now. At my heaviest, I was at 303 lbs. As of today I'm down to 272.2. I'm amazed that I can finally loose weight having multiple health issues. I was diagnosed with severe gastritis and ulcer prior to taking this. I do experience the tummy upset when I changed milligrams. Loose stool and nauseated. I also suffer from chronic migraines too so that can be normal for me. I have hashimoto's hypothyroidism so weight loss has always been a challenge for me.

      High most doctors put thyroid patients on just Synthroid what happens it causes the reverse T3 to go way too high. It is not the thyroid medication. Our bodies need in many many cases. Please get your labs, checked and check out stop the thyroid madness for the proper levels.

    I was currently on Ozempic 0.5ml and have been off Ozempic for about 4 weeks now. I just got switched to Mounjaro 5mg to start. Since I've been off Ozempic for 4 weeks now is 5mg Mounjaro ok to start with? I did well on Ozempic and hopefully I do well on Mounjaro.

    I have benign nodules on the left lobe of my thyroid and over the last ten years have been hyperthyroid requiring treatment with methimiazole. Considering the risks of thyroid cancer vs being 70 years old, diabetic on insulin and obese where do I stand on the risk/reward curve? Ball park estimates.

    I really wanted Mounjaro to work, but I'm in that group where the nausea symptoms were too severe to continue once I reached my 2nd week of 5mg. This is a really well balanced video though which I truly appreciate! Looks like I'll be trying Ozempic next.

    Instead of just blaming the insurance company, maybe we should blame the Pharma cos for charging insurance cos $1000+ a month for the meds in the first place. The insurance company passes that "bad news" to policy holders in the form of hoops and higher premiums....but the root cause is that medicine is so ungodly expensive because of what the Pharma cos charge.

    Does this med have rebound effect? Like if u take it for awhile then stop, do you gain all the weight back? Also does it cause b1 and b12 deficiency? Thank you

    I tried it.. sucks to have food stuck in your stomach so long.. may be good for you but i actually gained weight and was uncomfortable. I love love love my fast metabolism and slowing it down really bothered my sleep and comfort

    I've been on Metformin for diabetes, fenofibrate for hig tryglicerides, rouvastatin for high cholesteral. I changed PCP's as my old, burnt out doctor retired and the new Doc prescribed Mounjaro. I just finished my first week of 2.5mg. In the long run, will Mounjaro help me wean off the above mentioned medications? I read a review and it said that Mounjaro reduces A1c, as well as increases HDL and lowers LDL. and Triglycerides. That would be fantastic to lose weight and be only on Mounjaro and not the other medications....what are your thoughts?

    I have been on Mounjaro for 10 months was diagnosis pre-diabetic and my A1C was showing to be a diabetic. I was diagnosed with an auto immune thyroid disease. I tried going gluten free for almost a year with no results. I didnt want to be on medications for diabetes and thyoid issues that had more side effects than being overweight, I was already on high blood pressure meds. It doesnt matter what it is some are going to abuse it. People that need to lose 10 lbs I dont feel this is for them. Those that are insulin resistant /thyroid issues that have tried everything I feel taking these drugs to help take your health back is awesome. I have lost 70 lbs and do not have to take any meds any longer. So for those that might say non diabetics shouldnt be allowed to use this med is just not right. Some need to look at most of our medications that are prescribed different than what the label says its for. If it works and this does , yeah for us for taking our health back.

    Hi i have severe RA andI take Humira once a week, would this interact? Also I have Barrett’s esophagus would this affect it in anyway because I know it mentioned indigestion. I’m going through menopause so my orthopedic recommended this for weight loss for my osteoarthritis and bursitis in my hips and shoulders as well. I know I’m just falling apart.

    I have Hashimotos and PCOS with insulin resistance, am over 100 pounds over weight, have done restrictive calorie diets (under 1000 a day) with a nutritionist that didn't work. I don't qualify for bariatric because of Lupus. My insurance company (CVS) won't cover it until I'm T2. I'm frustrated that my health has to get EVEN WORSE than it is before trying something that could be life saving. I'm going to use the $25 coupon until mid next year and hope and pray something changes between now and then to make it 1) more affordable or 2) covered by insurance or 3) it Jumpstart my body in some way to suddenly make it work for me. I'm depressed and feeling pretty hopeless right now. I was never over weight until the PCOS, and always really fit. I don't even recognize myself anymore.

      I have hashimoto’s as well, and prediabetes. My insurance (Horizon BCBS) doesn’t cover Wegovy, but covers Ozempic and Mounjaro 100%. Hopefully your coverage will get one of those covered!

Eating sweets is not my favorite, but if i take a large portion of them, my blood sugar level rises high. I think it's the high concentration of the glucose flowing into the bloodstream all at once. This is a lesson worth taking note of. This is a great material to learn from.

      An A1c of 6.5 or higher is diagnostic of diabetes. Between 5.7 and 6.4 is pre-diabetes.

    This was very informative. I just started Ozempic last month, went in to see my MD today, and suggestions changing to this. They gave me my 1st month as a sample in office, and a coupon for the next rxs, if it works out. I'm really excited to try it. I used the wegovy, and did well, but couldn't afford it after the 6 month coupon ran out.

    I have taken the 2.5 for a month now. At first, it was just some acid and bloating. I took some Omeprazole and was fine. Just a couple days ago, I was diagnosed with Gastritis from taking the Mounjaro. I have stomach pain and constant diarrhea now. I even had a CAT scan to show it. Each person will have different reactions. Sometimes, they occur right away and some develop over time. I even changed my diet beforehand. I stayed away from the greasy and sugary foods and drinks. This is just something to watch out for. I have lost nine pounds from the Mounjaro though.

    We are so lucky in the UK we have a 4 dose in 1 pen kwick pen we have the National Health Service so its free for Mounjaro or any of the GLP 1 and GIP meds im type 2 and ive lost 1 stone 14lbs in the first month...

    Has anything been released about its effect on body composition? I keep looking for DEXA scans so we can see how much muscle loss there was vs fat loss.

      Yes, a subset of 160 patients in the SURMOUNT-1 study underwent DEXA at baseline and after 72 weeks. Patients on tirzepatide experienced a significant reduction in body fat mass (33.9%) compared to those on placebo (8.2%). Visceral fat mass also decreased more with tirzepatide (40.1%) than with placebo (7.3%). Lean mass loss was higher in the tirzepatide group (10.9%) compared to the placebo group (2.6%), but the overall body composition improved due to a greater reduction in fat mass. Fat mass as a proportion of body mass decreased more with tirzepatide, while lean mass as a proportion of body mass increased. The fat-to-lean mass ratio improved more with tirzepatide, particularly in patients who lost more weight.

    I am heading into 4th week of 2.5 mg. Going to 5 starting week 5. But, I don’t understand something. Do people lost more weight the higher the dose they go??? It would seem unsafe to eat less than 1,000 calories per day. Right now, I am eating 1200. If I continue to go up in dose, I am

    Thank you Doc for this info! Now, how can one get their insurance to pay for this? I am
    the perfect candidate as you mentioned in this video; obese and diabetic. My insurance refused to pay for Wegovy a few months back, they suggested taking metformin first. Is there a loop hope to get the insurance to pay?

      You can print out a coupon on their site, if you have a diagnosis of diabetes you can get this for $25 per month. Coupon only works if you have a diabetes diagnosis.

      There’s a little problem with the coupon. On their website, it’s says the medication has to be covered by your insurance in order for one to get the card. It’s not covered by mine. Had the same issue with my previous insurance and just switched to a new one, and am running into the same issue. I answered yes to the question to get the card, will come back and update you guys if it works.

      @@parris05 they have been having probs because too many people have been getting it w/o a diabetes diagnosis. But per my pharmacist friend, your ins has to refuse coverage and then the coupon will kick in and you have to have the diagnosis. Unbelievably my cheap ins actually covers a small portion, but every other insurance I heard of denies it.

      @@marymiles7327 my insurance covers it. Just picked up my first script @ walgreens. "United Healthcare Dual Complete" varies by state & must have both Medicare AND Medicaid to qualify for this plan IF it's even offered in your state. Great insurance tho, so might be worth looking into.

    I gained a lot of weight without increase in caloric intake and I can’t really reduce my intake to lose weight because I already eat less than 1,000 calories/day (I have no appetite and get full very quick) yet my endocrinologist prescribed this. Is it only affective due to appetite suppression or are there other ways that this can help? I didn’t know it was an appetite suppressant when prescribed and I’m wondering if there is any reason I should even try this.

    On 5 mg for 3 months now. Only lost 4 lbs and still have appetite . Im on insulin 2 times a day. Does this make it harder for me to lose weight?? Injecting insulin in the belly has increased immensely.

    I’m curious how to manage exercise with less intake of food. For example, if take it and eat less resulting in weight loss. I’m worried that eating less means less energy for exercise? Is that true. When I currently exercise on an empty stomach I get light headed

      Ideally, you should work with a dietitian to help guide your nutrition. But at a minimum, 60-90g of protein per day will help to support your exercise regimen. And try to eat smaller, more frequent meals if your capacity is limited.

    I recently was on Ozempic and was moved up to a 5mg dose, after 2 doses of the 5mg Ozempic insurance will not cover it. Doctor switched me to Mounjaro at 5mg. My last dose of Ozempic was 6 weeks ago. My question is it ok to go start to 5mg Mounjaro? or Should I go back down to 2.5 mg Mounjaro first? Feed back appreciated

      I was on Ozempic and was switched to Mounjaro I started on the samples at 2.5, went to 5mg & I start my first dose of 7.5mg today. I think you should be ok with starting at 5mg.

    I have been on Ozempic since May and now insurance won't cover it. I had no side effects with the medication injecting in the thigh. My insurance won't cover it now and my Doctor just started me on Mounjaro since this has a couple added benefits than what Ozempic has. Since I did so well on the Ozempic having no side effects, my question is with the side effects? will Mounjaro be just as good?

      It's never easy to entirely predict your response, but I'd expect it to work similarly for you, and have similar side effects.
