Yes, some agencies offer PTO. And yes you can take off as much time as you like. I usually take off 2 weeks in between assignments and contract extensions

It depends on the contracts available. There will always be contracts in other states, sometimes there are some within your city/state

You can be unlicensed or have your LMSW. Just depends on the state and hospital requirements
