I think that was a non CA truck converted to CA emissions, if so it should have an a/f sensor up front and a o2 sensor after the second cat since you have 2 cats. Regardless you're right about the downstream being after the 2nd cat. You might have a vacuum leak couldn't tell from your revving part, also check the maf as I had a similar issue where I forgot to plug it in all the way and it would act the same way yours does where it wouldn't start unless I gave it some gas and it would die on me about a mile or less after.
Bro, I'm a toyota technician im telling you do NOT over think..do a compression test and it will tell u whats wrong with the car..your car is having a mechanical problem not electrical problem..those 3rz are notorious known for burnt valves..I guarantee you your going to need to do a cylinder head service. Just by you starting the engine I can tell the engine has low conpresssion..when the check engine light flashes the engine is having excessive misfire. Not good to drive it if u keep driving it your going to blow up the motor...
I know this is a year old post but my 95 Tacoma with the 3rz is flashing the cylinder 1 severe misfire. Had good compression 170. Replaced the distributor cap,rotor, wires, plugs, cleaned the MAF. Have checked the fuel injector. If that is okay, can my exhaust valve still be bad or leaking heading head gasket?
Good diagnosis. I just saw further down in the comments where he posted a couple months ago saying he had zero compression on cylinder 1 and it had burnt valves.
My 99 Tacoma 2.7 on 31’s no lift gets about 9-10 Mpg and gassing it when taking off almost has loss of power till I rev it or it will stall itself and I’ve been driving stick for years. It also misfires if I take off before it’s totally warmed up. Someone else suggested I check fuel injectors as well but I’ve been told maybe a burnt valve too. Not sure where to start

Hey brother my truck is doing the same thing right now I've done put new spark plugs new spark plug wires my engine is one year newer so I have the two coil packs I put on brand new TPS sensor the upstream O2 sensor fuel injectors I'm still having random misfire's and a big lag and loss of power and this truck doesn't have much power from the beginning so it's a little annoying

Did you clean the throttle body and IAC valve,? check the fuel pressure regulator.
It was burnt valves
Do you have to remove the throttle body to remove the fuel rail?

check out the engine when someone is pushing on the gas, it could be a worn mount.

Fuel injector maybe plugged. Check injection pump. All injectors wouldn't hurt to change.Blow all lines out right back to tank.

The pump might not be bad it might just be clogged if you have never cleaned the screen in the reservoir
