  • Exercises for chondromalacia patella are often recommended to try in an effort to help heal the cartilage on the back side of the knee cap.
    But so many exercises that are commonly recommended by healthcare professionals ARE TERRIBLE for this knee problem because they add more pressure to the knee cap.
    Over time it hurts the patellar cartilage and sets up worse knee problems.
    Most healthcare professionals are not specialists in helping people heal naturally. Instead they specialize in helping people by using medications, injections, and surgery.
    Even physical therapists, the "medical exercise specialists" of the healthcare industry tend to focus on helping people who have just had surgery... making them great at getting someone up and moving AFTER surgery.

    So should you never work your quads for rehab?

    So in summary........you can't do ANYTHING? Except for some very low level leg swinging; or clamshell type exercises.
    Should you let your quads atrophy to assist recovery.......but won't weak quads create problems in other areas?

    thank you for your advice! I've got a bit of a tear in both meniscus and I've got condromalacia(on both knees, 4 on ythe left and 3 on the right one) and the cartilage in my left knee is at 4, basically they said that I had no cartilage between my bones (it is bone on bone) and they advise to get surgery replacement for my left knee. I said no! and got physiotherapy, that help me a lot , but I coud feel the tension on my knees caps with several of the exercise You advice against: knee extension with weight on the ankle, step onto a "step" one leg, alternating the leg and the one where you squat resting against the wall. I told them I could feel the tension and preasure on my knee cap and I thought that was not good and they asked me to hold a boll betweemy thighs , that helped to aliviate the preasure! Most People there had had surgery, ..I think you are right! I will stop doing those exercises in my rutine.

      Thank you I have the same thing as 4 on my left & 3 on my right and Dr keeps pushing me to do knee replacement!! I avoid food that would trigger the arthritis and stop doing those exercises too!! Now I feel better!!

    But in the physiology of vmo it pulls the patella to the medial side of the knee,annd strengthing the glutes is only going to take the pressure on the knee but not gonna fic the patella tracking laterally,strengthing the glutes will decrease pain but not fix the problem ,and there is some exercises that will target vmo ,and yes it will target the other quads but in a lesser extinct so thats fine,hope you answere me(im a radiology technician and a registered nurse)

      His advices were the only thing that worked for me. These are all the things other PTs asked me to do and I ended up miserable with more pain. Only thing he suggests to begin with is releasing quad muscles tightness through deep tissue massage and strengthening glute muscles. I can't thank him enough.

    Waiting for full kneereplacement ( yes or no) with special hospital surgeon. ....long long waiting list ....cant effort private 🏥 ....

    I like idea of importance of gluts muscles . But I think it is not only mechanical aspect , this muscles as big group , does feeds of cartilage of knee, How do you think, doctor David?

    To summarize, no sitting, no walking... Is flying ok? I've had severe knee issues for most of my life and I actually find walking very beneficial. Not power walking anymore, but just regular walking is ok. Know your body and recognize what works and what doesn't work for you. There is no One Size Fits All when it comes to knee issues.

    Is it okay to chair cardio ( e.g. Silver Sneakers) to stay fit? I am 69 with severe RA in all my joints, especially in the knee. I guess from now on I will just focus on hips, core and upper body. It is so confusing because so many other PT s are asking you to do exact opposite.

    Is cycling a quad dominant exercise? If so, is glute strengthening the fix for the quads working too much?

    I wish I could go to u, but I can’t for now
    I am in miami and after listening to u, I see my trainer gave me bad exercises for my body … I always had larger quads and the last time I saw her she told me to march on the concrete garage floor…
    I need someone to guide me

    Hi this is interesting but I do knee extensions because doing them cures my CMP completely! If i stop doing them it comes back and I have not found anything else that works

    Dear dr David,
    Could you share exercises for someone with joint laxity and hypermobility? I am a 32-year-old woman and have been experiencing intermittent pain in both knees for the past two years, making it difficult to hike, run, and squat due to the discomfort. An MRI showed suspected tears in both meniscus, but the doctor said I don't need surgery yet. I hope to strengthen my knees so I can be active comfortably again. Thank you very much in advance.❤

    In my case, walking is the best exercise to manage my pain. I need to keep my legs moving, 10000 steps is a good start

    But the leg press machine strengthens the glutes, (that's where I feel it) which is what he said was good for knee.

    I've been doing the bridge but you said not to feel it in the hamstrings, I spread my feet apart but I still get some stretch in the hamstrings, what am I doing wrong

      Hi Deb,
      You'll feel this in your hamstrings, but the main goal is to feel it in your glutes. When you raise up, press your feet evenly into the ground or table and really squeeze those glutes. If you can, try to engage your lower abs a bit too-like you’re holding in your pee (it’s the easiest way to explain it!). If that’s too much to think about, just focus on the glute squeeze when you come up.

    Is swimming and walking in the pool useful?

      Hey Negar! Swimming will be helpful if you can get your glutes to work rather than your quad muscles during the exercise. If you feel you are using your quads more than your glutes while swimming, then you will likely make your knee problem worse. Walking can help, but much like swimming if done incorrectly it can make your knee pain worse.

    It's sad to see a pt say quad ext are bad.... However, I only train this machine in a hold at light weight to strengthen the the vmo....you fail to talk about the importance of the troc groove... Ultimately, you doing a disservice to the health community, sorry to say... I've never seen a pt say so much conflicting confusing information.... 😓😓😓 And you're also scaring ppl... Sadly it's crazy how you have this many subs.... Scare tatcitcs are excatly that... Scary..

      As far as biking goes... As a pt, you'd think you'd be able to train ppl to use their glutes aka post chain during biking.... Man.... Are you saying you can't even help ppl to use their body correctly!?! Walking is bad... Man... Dood.. Ppl need to unsub... You're scaring ppl man... I'm sorry
