This was a life altering decision which carries a risk of death under anesthesia, especially in these underlicensed clinics abroad; it should take months to make the decision, so for her to say nothing, I assume she knew exactly how they would feel about it and disregarded her partner's thoughts and feelings on the subject. It's with that in mind I feel you're absolutely right in saying he(?) can't win no matter what. Professional intervention is likely necessary because she obviously doesn't trust her partner enough to have an open and honest conversation and now the writer is stuck looking at this drastically different face on the person they love.

When I was in my early 20s, my girlfriend dragged me along to her haircut appointment when we were out. I wasn't paying attention and was just reading.. When she was done, she had cut off all her beautiful hair and had this super short cut... I don't mean short for a woman, I mean almost a buzz cut. I didn't even have time to prepare/manage my surprise or anger because she's like "ready to go? What do you think?" and I must have made a disgusted face because she instantly freaks out. We broke up like a week later. It wasn't 100% because of the haircut, but honestly it played a part. Nose job is even crazier because that shit ain't growing back. Rough

As someone who lives in Thailand, it's very likely that the trip wasn't really for visiting a sick uncle. It was just used as an excuse for the trip to go back to Thailand to get cheap plastic surgery. You can be sure it wasn't just the nose either. She likely got lip filler etc as well.
