This is based on the type of model you have. What relationships are being moderated. I recommend searching Models in Hayes Process Macro, and it will provide you with a pdf for all the different types of models.
Hello Sir, my moderator was a categorical variable (gender, where 1=female, and 2=male). For the conditional indirect effects of X on Y, the indirect effect is significant for females but not significant for males. My index of moderated mediation is significant as well. Does this mean that gender moderates the indirect effects of X on Y? And does it also mean that with the index of moderated mediation significant, that the difference between genders is significant even though it was significant for females but not for males?

The moderator doesn't have to be significant.
Sir, how to continue if all paths significant except X-Y path?
Report the results as they are. You will have to present possible reasons for insignificance and significance in the discussion section.

Yes you can, but this kills the actual analysis and the results are no longer reliable.

You cannot use Model 4 or SPSS because you have multiple IV and DVs.
Sample size over 150 is sizeable for pls
