    Hi.
    I have a Toyota Avensis 2012.(Norway)
    I did all the steps you mentioned but the code(P2195) in addition to code (c1201) still shows up after one-two days,
    things to know;
    1- the car had en faulty head gasket, and the faulty code where there.
    2- I changed the whole engine including the EGR system.
    3- after changing the engine I changed the MAF sensor and the Lambda sensor(O2 sensor ) from another working car so I'm 100% sure that those sensors work.
    4- I checked the exhaust system, and there is no leak or damage.
    5- all values are ok, (I'm using Autel scanner) the o2 sensor gets volts and there is a current goes in the circuit.
    My doubt now is about the A/F relay.
    I hope you can help me to find out the issue.

    I got this code after I changed the oxygen sensors on my 05 Ford explorer 4.0L. I also changed my throttle body, egr valve, fuel pump and filter and PVC. My truck still has a rough idle after I replaced all of those parts.
