That's a good question, and not one that is directly answered by the game as far as I can tell. I have a theory. Perhaps he came this way before the gith arrived (they are a VERY recent development) and thus the bridge directly to Baldur's Gate was not yet destroyed. I imagine he knows that there are ways to get to the Underdark nearby (through the Selûnite temple or even through the well that's right there if he has some feather fall magic, or perhaps a secret way that only he knows as a native of the area who may have come up through the Underdark a long time ago) and was headed in that direction through the Blighted Village when he came across the goblins, which themselves only recently became more organized. Later... [spoiler space] . . . . . . . . . . ... we'll find him in Grymforge, which suggests he does indeed find his way down to the Underdark only to be captured by the duergar.

Halsin knowing this can think of only two other means to get to Baldur's Gate and apparently Barcus didn't use the Underdark yet. if he had he would have encountered his kin earlier. the other option, travelling over land, was strongly discouraged by Halsin and I doubt Barcus used that route to get where he is now.
I looked up both of those words. Pustulant would have been a medication that causes "pustulation", whereas pustulent means filled with pus or a sore. He's being insulting, he's not literally saying they're filled with or oozing pus.
