    7 months mounjaro were amazing! Lost 35 quick with no side effects. Just lost all interest food. Then the coupon ran out so switched to Wegovy which barely stopped the food noise. Didn't gain it back but didn't lose any either. I had to be careful. Now Im one month back on Zepbound and staring to lost again finally. This Zepbound is MAGIC. But Wegovy didn't do it for me.

    TLDR: Zepbound and Wegovy are both effective weight loss medications, but cost and insurance coverage are major factors to consider when choosing between the two.
    00:00 📈 Zepbound and Wegovy are both effective weight loss medications that work by boosting naturally occurring hormones in the body, specifically increasing glucagonlike peptide 1 levels.
    00:55 💡 Zepbound (tirzepatide) targets both GLP1 and GIP receptors, while Wegovy (semaglutide) only impacts GLP1, making Zepbound more effective in managing appetite, satiety, glucose levels, and weight.
    01:37 💉 Both Zepbound and Wegovy are weekly injections with a gradual dose increase over 4-5 months, but their milligram strength should not be directly compared, and they both impact PCT glp1.
    02:00 💊 Both Zepbound (tirzepatide) and Wegovy (semaglutide) have similar side effects, with rare potential risks including pancreatitis, gallbladder disease, and gallstones, as well as a blackbox warning for thyroid cancer.
    02:35 💡 Zepbound (tirzepatide) showed greater weight loss compared to Wegovy (semaglutide) in individual randomized trials.
    03:28 💡 Zepbound (tirzepatide) and Wegovy (semaglutide) are both effective for weight loss, but cost is a major factor due to limited insurance coverage.
    03:55 💰 Both Zepbound and Wegovy cost around $1,000 per month, making them unaffordable for most people without insurance.
    04:12 📈 Insurance coverage and prices for Zepbound and Wegovy may improve, but it's best to consult with an obesity medicine specialist for guidance on the best option, and adding support from a dietitian or health coach can maximize effectiveness in managing obesity.

    How do you compare the dose escalation done in clinical trials to the current trend of keeping people on a low dose for a longer period of time? Does it result in greater ultimate weight loss? The current chatter is to "stay low and keep it slow", but the Surmount studies would indicate that keeping the dose at 5 mg will lead to less weight loss.

    • @DrMcGowanMD
      @DrMcGowanMD  11 месяцев назад +9

      I recommend following the FDA-approved dose escalation schedule, whenever possible. Some patients may need to titrate at a slower pace due to side effects. Otherwise, we should follow the protocol that was studied in the clinical trials.

  • @MomOfSeven123
    @MomOfSeven123 11 месяцев назад +3

    In looking at the Surmount trial results, there appeared to be a cap of weight loss at 20%, resulting in a plateau. Are you seeing this in patients?

    • @DrMcGowanMD
      @DrMcGowanMD  11 месяцев назад +4

      Yes, all patients will reach a nadir weight. The weight loss is not continuous. Typically, this is reached between 1 and 1.5 years after starting the medication. Those weight loss percentages are averages, so keep in mind that an individual’s weight loss may vary from that number. Some patients may lose more than 20%. Some patients may lose less (up to 20% of patients will lose less than 10% of their weight).

My concern.....my body hates moving off Stasis. I never eat junk food, I live on a ranch in the Idaho Mountains and cook for my wife and I.. No deep frying, only bake, roast, boil and air fry. I also move around a lot, yet no weight loss yet.

    Here my experience and disclaimer - a not so positive one. I started with Zepbound because of high triglycerides in my blood. I was overweight but not severely (talking about 10 extra pounds). Usually patients increase the dose fast, it took me 4 mounted to get from 2.5mg to 5mg because my body couldn’t handle it. We talk about severe gallbladder pain, throwing up and a consistent acid reflux that made life miserable. Let’s also talk about insurance compatibility - neither Kaiser nor Multiplan PPO covered me and furthermore the providers savings card wasn’t accepted. Paid over $1000 myself every months being afraid to end up diabetic. Did I lose extra weight - yes sure but guess what happens if you can’t eat at all? You will feel weak. I used to hit the gym regularly. Right after my first injection I stopped, I couldn’t take physical exercise. I’m glad it helped a lot of people, for me it was horrific.

    I wish doctors were more proactive at stressing the need of an informed dietitian. It took quite a bit of research to understand how to eat to reduce or prevent side effects. I have had a very thorough understanding of nutrition and how to cook healthy meals. The food noise and the sweets sirens call is what i struggled with.
    I have seen criticism about these medications due to people losing muscle mass and other effects that are associated with poor nutrition. These individuals mostly didn’t understand the importance of prioritizing protein intake, and eating nutritious foods and avoiding high sugar, high fats and processed foods. I learned, painful, that my mild lactose intolerance was now a high intolerance. Also the broccoli I regularly ate was now something I needed to eat sparingly, it resulted in a ridiculous amount of gas. Also I needed to be selective about my fruit intake. I can only eat a little bit of apple due to its high sugar content.
    This has been a massive game changer for me. I started at 285 and I’m now 215. I have the fantastic issue of my pants falling off.
