    I am down 50 pounds on semiglutide over 200 days. Great info here . Just want to add you need to know your basal metabolic rate and then determine a new daily rate for weight loss. I also journal my foods including drinks everyday. It was an eye opening exercise in where the weight came from and how to not fall into that trap again. I am 20 pounds away from original goal and hope to be there by years end. Dont give up even when struggling with side effects, some are more sensitive than others to dose related issues.

    I hit my goal and my 7.5 I use for maintenance is not as effective and the food noise is coming back. So at this point I guess the best thing to do is double down on my lifestyle changes instead of going up on dosage? ☹️ I already know the answer to that. I just hate the food noise and the fear of gaining it back is real!!! I have a whole closet of new clothes! 😮

    • @GoodDrG
      @GoodDrG  3 месяца назад +32

      You Absolutely do know the answer. The food noise is telling you something. What is it? Only you know. The one thing it you know it is can solve is hunger. Everything else is lies in the form of boredom, depression, sadness, anxiety, fatigue etc...... fix that with the appropriate corrective action. Not with food. Good Luck to you!

      @@GoodDrGI am a nurse myself. I have never had any issues with nausea, but I am taking it for insulin resistance as a symptom of Lipedema. My A1C is great however so I have been tracking my blood glucose sporatically and was noticing the trend of it rising more on the last 3 days especially once we went up to 50mg. We also were worried about the Lipedema fat not allowing even or entire well absorption. I am doing well on 25mg so my doc and I discussed spacing it. Even if my main focus in not nessisary large weightloss as Lipedema fat doesn't seem very responsive, the MASSIVE HELP for generalize and localized inflammation in my tissues has made me a fan for life. I see it being used in small or micro doses for more inflammation diseases like PCOS etc.

    Different strokes for different folks. I have a WICKED CASE of Type 2 Diabetes, and, am on 20 units of Basal insulin (I’ve been able to reduce my dose, gradually, from 26, and now, have successfully eliminated the need for Bolus injections!)
    My endocrinologist put me on Ozempic to try to mitigate the weight gain (and, difficulty *losing* weight) associated with exogenous insulin injections.
    My condition is so extreme, even the lactose in 1/2 cup of 2% milk makes me spike, so, eating the fruits you mentioned would send my blood sugar sky high.
    I follow Dr.Jason Fung (a nephrologist from Toronto), and, Dr.Richard K Bernstein.
    I find intermittent fasting, and, selecting extremely low carb vegetables, -some of which, you have listed, along with moderate protein, and, fats, are helping me (in conjunction with the Ozempic).
    I have about 35 lbs to lose…which will likely fall off gradually, as I attempt to wean off insulin.
    Thank you so much for your videos. Although my case and requirements may be a bit out of the norm, I am finding a lot of helpful information in your content.

You need to see a hormone specialist and get on bioidentical hormones. I personally take 120mg of thyroid, 60mg progesterone and use a testosterone cream and vit d supplement. I sleep better, have more energy and my metabolism works now. Best of luck!

    While I like reducing the ups and downs concerning the effect mounjaro has during one week, a am a bit afraid that I might get used to a certain dose quicker if I split the Dose into two doeses per week. Do you have any experience with your patients as to whether splitting the dose leads to having to go up with the dosage in a shorter period of time due to lower up and downs?
    And thank you so much for all the Information you are giving us here- I‘m German and unfortunately it‘s way more difficult to get lots of information from German doctors as you have way more experience.

    • @GoodDrG
      @GoodDrG  3 месяца назад +6

      Hey! I have only needed to split doses as a last resort when I have patients running into issues with tolerability / side effects and aren’t seeing adequate weight loss/still having food noise. I haven’t seen any issues with leading to having to go up in dose. We use use the smaller doses as a way for the patients body to adjust to the medication and hopefully the issues with tolerability / effects subside over a couple of weeks. The goal is always to get back to weekly full dosing as soon as possible.

    Just got my Wegovy yesterday. BMI around 40 and have pre-diabetes and congestive heart failure. My cardiologist suggested Wegovy but I'm very apprehensive of taking it because of the side effects. I absolutely will run down the highway in my underwear to avoid throwing up. I'm a single earner so cant afford much down time. Im traveling in two weeks and am considering holding off on starting it until I get back, as I dont want to have to be worried about what to eat when Im on vacay. I've been following a Mediterranean diet the last few months and have lost a little weight, but its slow and I have some days where I slip up.

    @Florida Concierge Medicine and Wellness. I'm thankful I live in the Northeast. We have a place in South Florida, but it seems like alot of doctors are refusing insurance for legimate reasons and I get it. But I have wonderful insurance and I'm thankful to God and my medical provider that I receive my Mounjaro and visit almost fully paid for by my insurance. What's with Florida? Why so many concierge services? As an RN this is one of the main reasons why we hesitate to relocate there permanently. It reminds me of the haves and have nots. So many cannot pay out of pocket. Anyway thanks for the content.

    • @GoodDrG
      @GoodDrG  2 месяца назад +3

      Hey there! Wow! What a great comment with so much to unpack! In 2021 I opened Florida Concierge Medicine & Wellness as a direct primary care practice. I put “concierge” in the name because we truly try to make every patient feel like VIP. Direct primary care is membership based care. Basically all your primary services are paid for through the premium paid directly to the doctor/office. You get timely visits, comprehensive visits, calls, televisits, urgent care, after hours care, etc. no insurance is billed to our DPC patients. If you look at it it’s actually more affordable in many cases than the insurance based care with a lot less hoops. Insurance based visits have to have element completed in order to meet metrics that frankly may not align with the desires of you and your doctor. It’s like have a third wheel in a marriage who is never there but has the last word every time! Anyway we now are hybrid meaning we also take major insurances in my office with a small cohort of DPC patients. Why? Because teaching the population about DPC is not easy.
      Most experts agree it started in the northwest around 1977 and then eventually became extremely popular in your neck of the woods before the rest of the country caught on. I invite you to read more about it. My practice now does accept insurance and “concierge” doctors are the exception in Florida. You may have a home here where the folks have a high net worth and feel there are more concierge doctors here than insurance based but that is definitely not the case. The real question we should be asking is, “Is DPC/Concierge medicine the problem or is the issue our cumbersome insurance based model of healthcare?” I’m happy you happen to have great insurance but what about all the folks who are uninsured or have terrible insurance that lives to deny services? They pay premiums every month for poor access or not at all. I am confident DPC/Concierge medicine is not why folks across the country are not getting GLP1 agonist meds or any other medication for that matter. It’s the insurance companies that hold the keys to the medicine cabinet. They are the ones who make your formulary and decide what is “covered” and for what reasons even though you pay your premiums to them every single month. This is a discussion we need to have more often because so many of us do not truly understand the American healthcare system as it exists today and we simply accept it as the ONLY way. Thank you so much for this comment and welcome to the channel!

      @@GoodDrG My husband worked with the criminally insane which was an EXTREMELY dangerous job for the state of CT for over 40 years, yes he more than anyone is deserving of great insurance. You are correct it's not concierge service that's causing the shortage of people's access to GLP medicines. I have no problem with access to my physicians here in CT so I'm thankful I don't need concierge service. We certainly don't have a high net worth, nor do we wish to pretend to be around that. We are regular middle class people. We inherited our place in South Florida. I enjoy your channel. You provide wonderful information. Thank you so much😊🙏

      I appreciate this discussion because although I don't agree with socialized Healthcare, the insurance industry and pharmaceutical industries have an unholy pact to increase net profit at any cost. This means keeping people sick. The DPC model is great because it is not subject to this corrupt influence. Unfortunately, I fired my DPC doctor a couple of years ago when she started pushing the covid Vax despite the literature I sent her on dangerous side effects, etc. I now have insurance through work but see a hormone specialist for most things because my regular doctor doesn't prescribe buoidentical hormones and insurance won't cover it as it actually deals with root causes and fixes the root problem. Anyway, I think the current insurance model should be eliminated and replaced with DPC. Not sure how to fix all the problems cause by the pharmaceutical companies though, I guess just rarely use anything they produce. Glp 1s seem to be promising though....

      Hey there! I only go to “Stacking” doses when I come across a patient who has issues with tolerability of a full weekly dose. Like nausea at the beginning of the week that makes it really hard to tolerate food at all or fatigue or constipation that doesn’t improve much after trying all the other things I’ve talked about in my videos etc… sounds like you can go to 1.25-1.5mg with no problems. All depends on your personal preference on how many grams you want to lose a week. 100-200 g /week is well below the “safe zone”.
