SOLUTION: A retainer clip at the top of the brake pedal assembly breaks, causing a loose bolt to squeak. Your local toyota dealership WILL replace the assembly under warranty. This is the easiest and most consistent solution. The problem is known by Toyota, so there shouldn't be any issues. @@jasonrobert112 there is a fifth screw near the top of the brake pedal assembly that loosens, but i recommend if your car is on warranty to have the dealer take care of it. send the service advisor this video and a video of the noise in your car

nice figured out where my noise is coming from and i think i know how to fix it
same but someone said could be a pinched weld that noise driving me crazy
