Just a small note: Ovarian torsion begins with compression of the ovarian veins and lymphatics, which later on causes compression of the artery due to edema of the fallopian tube.

was diagnosed with 1.8" ovarian cysts and was taking birth control pills per my doctor`s suggestions. We are not worried about pelvic inflammatory disease because she hasn’t had an STD? What if she had one but didn’t know it
I think it’s still possible for it to be PID
Don't mumble words here please... The case is not a typical presentation of a PID... PIDs generally have a stronger pain to them, there is dyspareunia associated with them, the pain peaks few days following a period and lasts for 3-4 days after it and later on fade out... On physical examination you will also find adnexal tenderness or uterine tenderness, additionally usually on physical examination we can note a foul-smelling cervical discharge, this lady obviously doesn't have a typical clinical presentation of PID and the palpable mass on the left makes the image less suggestive of a PID... To be able to fully diagnose this lady we will have to start by a B-hCG, which if it turns to be negative we will do a pelvic ultrasound which would me suggestive of what she could possibly have... So no need to sit and nag on what the mentioned regarding this lady's history on STIs... The Dr. meant that her overall picture is not suggestive of PID...
