    As someone who recently went through anal fistula surgery your information and friendliness really helped me feel comfortable and calm about my surgery and recovery and was super helpful.

    I had massive circumferential grade 4 hemorrhoids that had thrombosed, it was the worst thing that's happened to my life. I had to to an open Hemmorhoidectomy they removed two of the Three hemorrhoids and since they were massive so I had the most terrible post OP journey. This is my third week, still have pain on pooping, and the hemorrhoid thats left behind is still bleeding but we can't intervene for now.
    I can Vow that the past 1 month has been the worst month of my life, due to the sheer amount of pain that I've been through. I was on multiple cocktails of pain injection, tramadol, Diclofenac, pethidine, Paracetamol. At some point I even took morphine just be be able to defecate. Please guys, eat well, take fibers, drink more water

      How are you now ? Why didn't you go for, for example, banding earlier when the hemorrhoids were, for example, grade 2 - 3?

      This is me right now cause I’m at a point where I have been in pains for a month now and I have outer thromboses and it’s extremely painful after every bowel movement

      What do you mean by “pain injection” ? Is tramadol the most powerful pain Med they gave you? Cant imagine that helped much at all, I wouldn’t do this without morphine at least , on a daily basis.
      What would you say led to this? Like was there lifestyle changes you should’ve made but didn’t? Or supplements you should’ve taken but didn’t?

    I’m real close to cutting into my own clotted hemorrhoid. I hate getting the run around with docs wanting to turn to creams instead of GETTING THESE CLOTS OUT OF ME

    Dear friendly proctologist, I discovered you through a video you posted previously about anal itching. You recommended pranicura and being the desperate, tried everything under the sun sufferer I am, haunted by this problem, I bought the cream and I need to thank you so much. I've been using for a week and so far, I'll report, it's so nice to live my day without the itch. Hopefully my sores are healing in the meantime but either way it's such a relief to know I won't have to itch and suffer all day, wishing I was just a person who could gets to live without this embarrassing problem.     I live in Colorado. Had upper endoscopy and colonoscopy for terrible chronic symptoms. Got a letter informing me of internal hemorrhoids. FOLLOW UP WITH PRIMARY. No instructions, no explanations, NOTHING. Thank you for being a real doctor. THANK YOU.

      In my colonoscopy the nurse practitioner (men) met me to say everything was fine and the doctor didnt find anything. Five years in excruciating pain I had another colonoscopy. The doctor (woman) told me having babies took its toll! No polyps JUST MANY HEMROHIDS!! They are very high up (rectal not anal) and she is addressing them one by one. So painful

    I just found your channel! So happy I did. I was searching for help for burning in my bottom. I recently had a total colectomy with IRA and seems everything is angry down there. Who knew your bottom could burn and hurt so bad?! Thank you for your information! 😊😊

    I've had 2 thrombosed internal hemorrhoid flare ups 20 years apart. Absolute agony. In both cases they didn't start to get better until they eventually burst and the blood and blood clots could get out. Very messy for a couple of days though. I recommend ibuprofen for the pain, don't take if bleeding though. Regular baths and sitz baths with epsom salts. Flushable wet wipes with aloe Vera. And some small gel ice packs. Most doctors have a "just ride it out" mentality so don't expect much help from them. That said, if they do burst and the blood loss is excessive, get help. Good luck 👍

      Exactly what you said. I’ve had 2 myself over the past 15 years. Worst pain and agony I’ve experienced.

      ​@bigcod50 I've had minor roids over the years, but once I ditched dry toilet paper, that seemed to fix the issue. But last year I had to attend a funeral, and it was a 15 hour drive, which means I sat on my butt pretty much for that long. When we arrived at our Airbnb I noticed a small roid. It got bigger and bigger (and of course more painful) over the course of the trip, but now it was time to drive back home. That's another 15 hour drive, but this time sitting on a roid. It was agony. When I got home and took a warm shower to try to relieve some of the pain, I couldn't believe what I felt back there. There were now 3 huge ones, and they all hurt to the heavens. The next 4 days was the most pain I have ever felt. And the worse part was I had jury duty starting in 1 week, and what do you do at jury duty? Sit all day long lol Luckily about 2 days before that the pain subsided enough that I could actually sit somewhat comfortably, and a week after that they were gone. That was last april, but 2 weeks ago they came back, and they are still here. Help. lol

      @@letitiaandreeafekete7048 Afraid so. It was thrombosed. Saw a specialist who said no need to remove it, that it would shrink naturally - & it did. But very painful / uncomfortable for 2-3 weeks. If in doubt, see a doctor!!

      @@Markus-_- Kept clean, ate plenty of roughage & let nature take its course. A colo-rectal Consultant advised no surgery, just be patient & the body will probably heal itself - which it did.

    I hate the things....hate even more the awful docs who gaslight and dismiss the intractable pain. I have grade 4 and now will have the lasix surgery next wk....cost 6500.00 ....pension money but desperate times calls for desperate measures!!!!

    Finally getting the most painful thing I've ever experienced under control entirely through self-care. I've learned that the "level 20 pain" is caused not by the external hemorrhoids, but by an anal fissure (split in one of the many creases surrounding the anus). That's my own diagnosis from what I can feel when inserting a gloved finger coated in antibiotic+pain-relief cream and lidocaine cream to treat it.
    I have to disagree with the Dr on one issue as well... Leaving residue on that fissure in particular will prolong the intense pain. I do my best to wipe it off very gently, sometimes with Tucks(Witchhazel pads), before applying the creams.
    I also learned from another Dr that antiperspirant (aluminum chlorhydrate), with no fragrances or anything, will shrink the external hemorrhoids but must be applied every time you poop.

    Sid’s baths are key. It will bring it to a head and then drain on its own. I was able to do this twice. No blood but pus when I put pressure on it

    Doc can you aspirate the blood in the thrombus hemorrhoid instead of cutting it? The wound will take time to heal & if there’s a wound the pain is so sharp & excruciating.

    Since poop is mostly bacteria, how do you prevent infection in the open wound created by cutting the hemroid?

      It is odd but don’t seem to get major infection. Blood and poop, you’d think they’d be loads of infection.

    i already have hemorrhoid 18 years ago after giving birth via cesarean section my hemorrhoid is like a round small greenbeans but few days ago i decede to get lose some weight and i eat more fiber and i go 4x a day toilet for 7 days and i stop because my hemorrhoid become swollen and painful i try to use cream but now it become harder and bigger.before i can push it inside but now cannot go back already im very sad and scared about it

      Too much fibre can make things a bit hard if there isn’t enough fluid for the fibre to soak up.

      i've read ppl dip a cotton ball with witch hazel add the sugar and place it on your anus. I guess the sugar helps with the swelling. The sugar reduces the edema by osmotically drawing out the fluid. It's often used for prolapse.

      I get it sometimes when I push up my haemorrhoids or sometimes if I push up a medication thing. I wonder if it shrinks the haemorrhoids and perhaps part of my gut?

    I'm in this situation right now. I had a flare up start last week from pushing during a bowel movement. They popped back out and haven't been able to put it back in. I've seen several doctors and surgeons know one will help. I've been in extreme pain for a week. Only to be prescribed pain medicine to do everything you just said and still waiting for the swelling to go down. This has been a life altering experience I wouldn't wish on anybody. 😢😂

      I finally was referred by my primary physician a bariatric surgeon that saw me the next day and agreed that hemmroidectomy surgery was my only option. Scheduled me the next morning in a hospital. I'm now 9 days post operation and as difficult as this recovery has been I'm glad I'm on the other side and they're gone. As hard as recovery has been I know eventually I will heal completely and my quality of life will increase. Thanks for asking for an update.

    I see its recommended that people should get 25-30 g fibre a day. You mentioned too much fibre can sometimes be a problem with making big bowel movements. Do you mean higher than 30 g or should we aim for less when our hemorrhoids flare?

    "Haemorrhoids are considered among the most frequent proctologi-cal condition at a general practitioner (GP) practice. Acute prolapse of internal haemorrhoids is presented with oedema, inflammation and acute pain. The application of granulated sugar on swollen hae-morrhoids leads to an immediate reduction of their edema and to the patient's relief. After the topical application of sugar, haemorrhoids begin to shrink immediately and edema is drastically reduced, while haemorrhoidal tissue can easily retracted back into the anal canal. The method is a cheap, quick and painless way to control the worsening symptoms such as swelling, bleeding and irritation. In addition, this method can easily be applied in the GP practice without the necessity of any form of anaesthesia."

      Put it directly on your bum for 15-20 min. Lay on stomach. As much as many times as you can. Rinse with warm water. Lay down as much as you can.

    I believe in have internal hemorrhoid thrombosis. I'm on vacation and in Dubai and came down with hemorrhoids. This video is the only one that actually describes my problem to a T. I do have one question what do you do with sugar and what kind of sugar? brown/white?

      Its just a shallow bath for your bottom with warm water… maybe epsom salts. You can use a bathtub …. It is to relax and clean and help ya

      I use Epsom salts in bath or u could put some in a large wide open plastic bowl from dollar store to sit your butt in for 20 min with Epsom salt) . Ive also ordered a donut hole medical store for hemorrhoids pain so not sitting on sore butt all the time.

    I had a blood test and colonoscopy in May 2022 after painless bleeding before pooping and got the all clear, no cancer. They found Piles and said it was that causing the bleeding.
    Now is late Nov 2023 and i still suffer from recurring piles (internal) painless bleeding before and after pooping with bright red blood all in the water, paper and on the stool. The actual Stool colour is normal but im now paranoid its cancerous Polyups etc... The internal piles go back in after a bowel movement but come out when pooping. Its the before and after blood im worried about!

      My toilet bowl/water was bright red/pink with blood for over a day, i went like 5 times in that time, and there was bright red/pink blood in water each time, went to doctor and he said was hemorrhoids, but how can it make the toilet bowl/water red/pink for over a day and each time i go to toilet ? cant just be hemorrhoids surely ?

    My surgeon did not attempt to push my hemorrhoids back in after being lanced. Is this normal? Since you said you try and push them back in. Thank you

    Life sucks when you get those painful ones. There is no escaping the pain and you just suffer. Motrin 800 does jack, hot baths, do zip, you just SUFFER.
    You're a living walking hemorrhoidal pain.

      Yeah it really ruins your life. The pain is insane. Not as bad as kidney stones but definitely worse than lumbar herniated discs. It’s a different kind of pain. Add the bleeding, it’s like something out of a horror film.

    When is it time to see a doctor? I've had persistent pain for months but nothing too severe until tonight. This was pain on the level it made me cry out and just did bath after bath. I'm chugging fiber just so I can avoid that level again but wonder when is it time to get pro help

    Dr chung I have fissures that have caused anal spasm for the past 7 weeks and life has been hell. I now also have a trombosed hemorrhoid, but the cream I would use for that is the opposite of diltiazem because it would restrict the bloodflow whereas with the spasm and fissure you want to increase bloodflow… I am at my wits end and my doctor doesn’t know what to do anymore. Would you recommend I keep using diltiazem or start with hemorrhoid cream for the trombosed one? Thank you for all the videos and the community, makes me feel less alone.

    I got a little relief by dipping a witch hazel wipe in sugar and pressing it against the hemorrhoids for about 10 seconds, repeating every 45 min- 1hr until they go back inside. My problem is that I also have pelvic floor dysfunction so my rectum clinches up and doesn’t release. This is the worst pain I have ever experienced when both happen at the same time. Not sure how to proceed.

    I had minor issues with hemorrhoids because of my opiate addiction. Been clean for over three years now and ive had the worst case ive ever had. It looks like cauliflower ear around my anus. Cant even see the hole anymore! Ive had cancer 4 times now,the most recent being just about a year ago. This has been some of the worst crippling pain ive ever had. Idk what to do! Creams,wipes etc arent doing much at all.

    dr chung... i have hemorrhoids (small ones) that are soft, painless and come out when i take a sitz bath or poop and go right back in when i stand up or maybe pull it back without using my hand
    HOW DO I MAKE SURE I DONT WORSEN THESE THINGS UP??
    (i am taking polyethylene glycol 3350, miralax everyday and 5-6 days a week, my poop is soft)
