  • Saving patient's time and money.. if we try to extract the tooth we would be replacing all the crowns in his tooth.. if we try to make a new crown it would look different to his other crown no matter how accurate we get the color.. so this is the most efficient way our clinic can provide the patient
    Disclaimer: 5 to 10 years was a little bit too optimistic.. it might be lesser than that due to the compromised situation... but it was only to prolong the teeth until a longer and more permanent solution is available..

      They never had such problems. No refined carbs or sugar in their diets to cause and accelerate tooth and bone decay.

      Not true was watching a documentary on Neanderthals and saw that one of the skeletons cause of dead was and infection that had started from the teeth and moved up; they believe it was from too much honey.

    Very nice work!!! You can tell loves to do this work. Most dentists now days don’t care about the patients. I’ve been to some and mentioned things I wanted checked out and fixed and they avoid them and just make excuses.

      Yeah those are the dentists to avoid for sure. My wife and I have had that happen to us, then those things they say "aren't a big deal" end up being much worse 12 months down the road...

    Amazing work. My wife and I are 71 and 68, we have good teeth, but several teeth have had work done. These dental procedures, equipment, digital equipment for exray, photography is so far advanced in recent years. Our dentist are wonderful and talented. ( in St. Louis).

      bearable pain after local anesthesia wears out and had to drink 1 pill of ibuprofen and that's about it, no pain anymore or any issues so far

      Well you dont really save it just prolonging it enough before it fully dies once you get a root canal so its better to fix it before it hits the pulp which is the extreme pain you feel when it finally reaches it.

    This is only a temporary fix. The fracture is at the crown margin and will leak and fracture again quickly and crown will fall out again. If the root canal is viable then you need a stronger post like a cast metal post and new crown prepared 2 millimeter deeper into the gum line to create a ferrule effect. Ferrule effect is like this metal strip wrapped around a wooden barrel.

      I agree.. but we have modern studies and technologies nowadays.. and fiber post and self cure composite shows that it flexes together with the dentin which avoids leakage and fracture.. and creating a ferrule on the tooth would mean replacing everything..

      @@happyfacemeow Ortho extrusion maybe needed because we need to place new margins 2 millimeter apically. Without x-rays I don't know if we need to extrude the root. Certainly we do not want to "violate " biologic width .

    When i was comparing metal posts to fiber posts indications, i found out that you cannot use fiber posts when you have less than 2 mm of tooth structure above gingival level..
    Because the fiber posts are less rigid , and will come out if not supported laterally..
    For similar case , i would make a metal post, and even that wouldn't last long

      I think, you can't compare adhesive properties of fiber posts over metal posts, Also less rigid it's better for prevent rooth fractures. Personally i don't work with metal posts, a Long time ago.

    This is very heroic, but it’s very difficult to give any prognosis on that. It might not last 24 hours. Sometimes this is the best thing for patients who can’t afford anything else, but another option would have been an immediate implant with using the existing crown as an immediate temporary to mold the gingiva in preparation for a new crown on a custom abutment

      Don't forget new adhesive properties. You better try universal Bond II from tokuyama

      new adhesive techniques are stronger than before. I have cases that is still intact upto now that I did 3 to 4 years ago.. it might not last as long as an ideal case but it is still better than extracting the tooth.. and we can always do the usual treatment if ever it failed.. but if it lasted 3 or more years and I can already say that it already did its job..

    This case you're solving, it only solves temporarily, soon it will break, what is right is to make a metallic core that comes from the inside out that will last 10 to 15 years.My similar case is this patient of yours, but mine also underwent root canal treatment and the mold was removed to be able to place a metal core that has lasted 15 years to this day and where the false tooth is stuck in a metal core.

      Yes..that's also a good option..but the problem is..if you make a new core you have to make a new crown..and making a new crown will more or less not esthetically fit the other crowns.. so we also have to replace the old crowns..which is totally more expensive.. and in a metal core we need ferrule.. and the case has a very small ferrile..so we picked adhesive dentistry which is usually stronger nowadays..but i appreciate your insight..

      @@DrArkhedeLeon All it takes is a bite of apple or hard bread and that crown will fly out of his mouth
      Resin or glass are way too weak compared to a cast post. Ferrule is absolutely necessary and a new crown can be made with zirconium for a close shade match.....I've done it many times . Your temporary fix will last a few weeks.

    I think resin cement has less bonding property comparing to luting glass ionomers while fixing a PFM l crown

    This is fantastic. My dentist said my front tooth crown is wobbly and said I need to get it removed but he will not even try to give me root canal treatment. He said it’s better to have a screw in tooth or denture. Wish I could come to a dentist like yourself. You’d fill me with confidence

      He may be right. The decay and cracks etc may be to bad and it will be to weak a structure and only temporary. A dental implant will last a very long time but is expensive sadly.

      @@applejuice4065 I've had a missing canine since 21 so I feel your pain. Honestly didn't smile for years, so long that smiling feels unnatural like terminator. I just rock it though, the gap is kinda small as I had the adult tooth removed before it come through when I was just a kid. They left the baby tooth which lasted until 21 and chopped out the canine because it was growing on an angle.
      Get the denture and if you can ever afford a permanent implant in the future do that. Good luck, hope it all works out okay for you.

    Hi ! I’m a student in dentistry in France, appreciate your content, i wish you can answer my question.
    Here we learn that to make this procedure you need at least 2 walls with 2mm high and 2mm wide otherwise the fracture risks are high.
    Do you find this obsolete or just a rule that can be overstepped in few cases (like this one) ???

      Hey, dentist from Switzerland here. You're right, this would be perfect. But what are the alternatives here? Extraction an a Bridge or Ex and an Implant with a Crown. This is much more expensive, and sometimes patients don't have so much money. So we try to fix it as good as possible and as cheap as possible.

    I had 2 roots like that fractured tooth removed via surgery- both were my very back molars on top from breaking them on popcorn seeds I bit into. They came out in tiny pieces at a time until just the roots were left. I wish I knew this was possible and that my and my Dad's former dentist knew of this repair method!

      But it takes time, and most dentists are paid like rockstars for simple checkups. Look how resistive they are to do reasonably simple root canal treatments - just because it takes time and some effort. "oh, phew, phew, that will be £700.. " is common. Horrible, cash-driven dentist attitude at the same the time, when neurosurgeons are making magic and average orthopedic surgeon knows how to successfully perform at least 5 different methods of how to fix a broken neck of femur. In my opinion - current dentistry this is an 18 century science utilising 19 century technology. But are getting paid at 21 century rates. Fuck them all.

      It was not possible so there is nothing to wish for my friend, it’s highly unlikely this restoration will last longer than 3 months (might be under 24h) and you want to fix a molar with the same technique? Obviously you don’t know how ridiculous it sounds to a professional,in the future don’t bad mouth your doc if you don’t know what you’re talking about.
      Im not being mean just spitting facts.

    Same broken teeth of mine... And my doctor did bridge cap for that one teeth and trim my healthy tooth of 1 left and 1 right for putting bridge 😔😔😔. For placing 1 teeth my two healthy teeth got trimed and get small and thin😔😔😔. I hate those dentists who did mine... They have not even told me that this process works like this they just told me price and did it.

    My pain tolerance could never 😩 even if I got that thingy that relieves the pain I will still be screaming lol anyways this is really satisfying to watch ngl I love it! It inspires me to be a dentist but I think I'd rather be the job I like hehe 😆
