Yes, ghouls are basically normal people who - for whatever reason - did not die upon exposure to learhal foses of radiation. Instead, they were imbued (cursed) with extended life and slowly decaying bodies. Eventually, that rot reaches their brain and they turn into violent, zombie-like ferals - as such, they are generally shunned by most normal folk ("smooth skins"), if youll remember the merchant in the 2nd episode telling The Ghoul that "your kind aren't welcome" in Filly. Some of the most popular (and tragic) characters in the FO games have been ghouls.
My guess is that Snip-Snip gave Lucy the finger because he was originally a medical robot and it was part of his core programming. It was hardwired in to do that. And the organ-harvesting program was added later, so it wouldn't be triggered automatically, someone would have to run the program ("Do her before the next shipment.") Yeh, Roger was a mercy killing. When Coop entered the Super Duper Mart, he discovered large quantities of drugs (I'm assuming the dopey guys were also drug dealers.). Ghoul metabolisms run differently from humans, so more chemicals are required to feel the effects - hence the reason he was gorging powders and liquids like a fat kid at a pie eating contest.
