@@weegowee you could pull off of the office 365 credentials, and have the it person update the ticket and automatically capture their name or email from office 365 with user().email after implementing office 365 connector

how will you share the sharepoint list to the power automate
Just go into power automate and add the your sharepoint get item connector. This will give you access to Sharepoint data.
How bout this Form -->Automate-->Create Planner Task-->Populate Planner Task with Form Info-->set due date based off of SLA and submit date of form
I'm sure that could be configured.
