Sex is a complex, multi-faceted phenomenon, some aspects of which are indeed immutable (e.g. one's karyotype), while others are not, and some are even liable to change of their own accord (e.g. sex hormone chemistry). Thus, making a blanket statement that sex is immutable doesn't make any sense.

Please give an example of spontaneous biological phenomena that can literally result in a human changing gender. There are none.
If gender is not determined by body parts then why does gender reassignment surgery alter body parts?
Because the two are often correlated, & functioning in society as that gender is greatly hindered if you don't fit the mould, so to speak ☺️
