Card or terminal? What happens when terminal forces transaction for online authentication? And how is it forced?

In case of Contactless transactions, the card is held by the customer in hand and so long as the card/device is present RF zone (terminal RF zone), the script processing can be performed successfully. If the card is removed from the RF zone, then the script processing can NOT be executed. Refer Section 3.11 - Issuer Script Processing of “Book C-5” of “EMV Contactless Specifications for Payment Systems” for further details. Guessing that you are asking if POS/ATM generates the 8583 or Acquiring switch generates the 8583 message?

Yep, typically after all the emv validation happens. PoS builds the message and passes it to the acquiring switch.
