    Please ans my following queries.
    1.What do you mean by a non training job for those who have done Mcrp 1-2 and Paces ?
    2.How to get in ST for IM as well as Surgery?
    3.Is plab 2 needed ? Can we directly sit for MRCP/MRCS to get into speciality training?
    4.How much time is required to become for example a cardiologist or a cardiothoracic surgeon after MRCP MCRS or after PLAB ?
    5.What are the differences with government and private hospitals in UK ?
    6.How is this different from pg in India ?
    7.The expenses involved and the pay for a cardiologist or a cardiothoracic surgeon ?
    8. Competitiveness in speciality like surgery and IM ?

      Hi, thank you.
      1. Non-training jobs are trust grade/clinical fellow jobs. Training jobs will lead you towards obtaining a CCT (Certificate of Completion of Training) in that specialty.
      2. IM?- Do you mean internal medicine. Once you have obtained your GMC licence, apply for a trust grade SHO job in UK first. This will give you experiance working in the UK. While you are on the trust grade job, ensure that you obtain the Foundation Achievement of Competency Document (FACD). You will need the FACD to apply for specialty training. If you wish to do one of the medical specialties- apply for internal medicine training IMT (3 years), then apply for specialty training after that (4 years). Then you will achive a CCT in that specialty. You will need to complete MRCP during the 3 years of IMT.
      If you wish to do one of the sugical specialties, after obtaining FACD- apply for core surgical training (2 years), then apply for the sugical specialty (ortho, ent, vascular, urology etc) you are interested in (4-5 years). Then you will achive a CCT in that surgical specialty. You will need to sit and pass the MRCS during the 2 years of core surgical training.
      3. To work in the UK, you will need to complete all the parts of PLAB or MRCP or MRCS.
      You only need the MRCP and MRCS to enter into the higher specialty training as I mentioned above. Apart from the MRCP/MRCS- you will need to have experiance and complete the competencies equaivalent to the training programme before you apply to the next stage (higher specialty).
      4. If you have not done any specialty training preciously, it will take 7 -8 years to become a cardiologist/ cardiothoracic surgeon.
      Cardiologist- 3 years (IMT), 4 years (cardiology specialty training.
      Cardiothoracic surgery (2 years ) (core surgical training (4-5 years cardiothoracic surgery). OR (7-8 years cardiothoracic surgery specialty training).
      5. NHS hospitals are funded by the goverment. Private hospitals are not. All training jobs and most trust grade/clinical fellow jobs are in NHS hospitals.
      6. Unfortunately, I can't help you with this question. I am not familiar with the PG process in India.
      7. All specialty training in UK is free. You will be working for the trust/hospital while you train. You will earn a monthly salary as well. Specialty training in UK is not a masters/pHD where you are linked to a university. It is conducted by health education england.

      Consultants earn around 100k/year.
      8. IMT and CST is not competitive for IMG as long as you have completed the FACD and have some experience working in the UK. The higher specialty training - cardiology/ cardiothoracic can be competitive, so you need to have a good portfolio/cv before applying.

      Best to obtain GMC registration by taking the UK specialty exam like MRCS/FRCS.
      After that, You can find jobs on NHS jobs www.jobs.nhs.uk or use a recruitment agency. bdiresourcing.com .You can normally secure a job faster using a recruitment agency.

      Hi well done for completing the MRCP. SHOs are senior house officers in the UK. You must be able to do simple procedures- venesection, cannulation, ECGs, peak flows, cathetherisation, blood cultures, ABGs etc. Nothing to complicated. You will have adequate support from a senior doctor (registrar or consultant) if you need supervision or help.
      Yes, you will be able to go for Geriatrics. Geriatric medicine is not very competitive. You can ask for a geriatric/care of the elderly rotation while working in the UK.

    I have a query with regard to mrcp.
    Suppose I clear mrcp while working as a junior doctor in my country, should I still do CMT from uk? Or can I enter the speciality training.

      Hi! Thank you. I am glad that the video was useful. Apart from the MRCP, to enter into the speciality training programme you will need to complete the competencies required and have a portfolio similar to a CMT trainee. You are also encouraged to have experiance working in the UK to be registered by the GMC.

      Hi the best way to ensure you have a good portfolio is to look at the job/training you are applying. You need to show interest and commitment to the specialty. For example, try to do quality improvement projects, clinical audits, evidence of courses attended, teaching evidence, research and posters. These are just examples. You should at least have a few not all of the above.

    HI Lavyna! Thank you for your very helpful videos for us international doctors wishing to work in the UK. Found scarce videos on this especially for internal medicine doctors. I am from Malaysia, and have completed two years of Foundation year equivalent training here and am 1.5 years into my CMT equivalent training here. I have passed the MRCP part 1 and 2 this year and am contemplating to pursue my internal medicine training further in the UK. Would appreciate if you might be able to share your insights and advice on these issues:
    1. I plan to hopefully proceed with PACES in Malaysia prior to heading over to UK in view that I'll be exempt from PLAB should I have MRCP UK in hand.
    2. I understand that even with MRCP, international graduates will most likely be given a non training post which might be not of my interested specialty to begin with. And Ive heard of many being stuck in such CF posts for years without being considered into ST3.
    3. Do I still need to apply for CT1 posts if I do pass the MRCP ?
    4. Ill only qualify for Round 2 for ST3 posts as an international graduate until I obtain a PR Status? Chances sound dismal for any hopes of entering competitive fields e.g. cardiology/ gastro for an international doctor as myself
    Sorry for the lengthy post. Would very much appreciate your insights and opinion on this matter Lavnya. Thanks !

      Hi, i am malaysian as well. Nice to hear from you. Well done so far. I will try my best to answer your questions
      1. Yes, if you have completed all parts for the MRCP you do not need PLAB to work here in the UK.
      2. Yes, this is true. You are more likely to be offered a non-training post eg trust grade/clinical fellow job. But because you have completed the MRCP- you will be offered a medical registrar (ST3) job/role which is at a higher grade/pay than a SHO. You can apply for any speciality registrar trust grade job- eg gastro trust grade registrar.
      3. With regards to specialty training, you have a few options. You can apply for IMT (new CMT), or get all the competencies equivalent to IMT training while doing your trust grade job. As long as you have met all the requirements- you can apply for specialty training -gastro, cardio, resp.
      4. Yes, you will be offered a training post in clearing or once all the UK and EU candidates have been offered a post. It is still possible. Just be open, you might get a training post in a different location to where you wanted. eg nottingham instead of london. Make sure while you are in the trust grade job, you really concentrate on making a strong cv. Things like clinical audits, posters, quality improvement projects are what they focus on here.
      Your educational supervisor will be able to guide you to do this.
      5. Just thought i mention this, if you have worked in UK as a trust grade doctor for 5 years, you will be able to apply for IRL and then british citizenship. Once you are on IRL or BC you can apply as a british candidate and have no visa restrictions.

      @@tskhooo Hi, glad you found the information useful. I have completed CMT. I am currently an ST4 in acute medicine specialty training. I was only offered a training post in clearing/round 2. It is possible, even for competitive specialties like cardio/derm/gastro as long as you have a strong portfolio/cv.

      Hi yes, to work as a medical registrar (ST3 and above), you need to have MRCP and also 2-3 years of internal medicine clinical experience.

      You will most likely be an Foundation Year 2 (FY2) grade doctor. It is also called- SHO/trust grade SHO.

      Depends on your work schedule. Normal days 9-5. Oncall long days 9-21.Nights 21-9.

    Hi, I am soon to finish my MD in radiation oncology from India.
    I have published international papers during the course and an observer ship at Manchester in head and neck cancers.
    I plan to work in the UK please guide me which route is more appropriate between PLAB or FRCR and the prospects to work in clinical oncology with initial salary.

      @@deepanjalipatel3959 if you plan to settle, MTI route will not be a good option. MTI is for doctors who want to return to their home country. If you want to pursue clinical oncology, you should consider PLAB/MRCP. Most oncologist here hold MRCP.

      1.since housemanship training in Malaysia needs two years to complete,Can I take PLAB 1 after finishing only the first year of housemanship training?
      2.How long does it usually takes to get a job in uk after gmc registration and all the job interviews based on most of the IMG so far that you know?
      3.Is it a good choice of going back to homecountry after gmc registration done and doing skype job interview in home country instead of face to face interview in UK?Because,as we all know,staying in UK for a long time needs a lot of money to spend before having job

      2. Most people secure a job between 3-4 months.
      3. Yes, you can return to your home country to do skype interview.

    1. Can you tell me the differences between PLAB and MRCS? What are the advantages of each over the other?
    2. After completing PLAB or MRCS, how can one do post graduate, specialization and super-specialization on any subject or course in UK?
    3. How would you guide me if I wanna be a Surgical Oncologist from UK? Can I expect a good scope of learning and practising in surgical oncology in UK?
    4. Does foreign post graduate degrees prove any helpful for getting training or practising or to any other value in UK?

      Hi, thank you.
      1. PLAB is the medical licensing exam to register with the GMC and work as a doctor in the UK. MRCS is a surgical specialty exam. Doctors normally do the MRCS while doing core surgical training CST (2 years). After CST (2 years) and passing the MRCS, doctors can apply for higher surgical specialty training like vascular, general surgery, ENT, T&O, maxfax, plastics, etc.
      If you have completed any of the UK PG exams like MRCP/MRCS/MRCOG etc, you do not need to do the PLAB exam. You can register with the GMC if you have completed MRCS/MRCP/MRCPCH/MRCOG etc.
      2. After PLAB and internship/FY2, doctors apply for core surgical training (CST) (2 years). After CST, apply for higher surgical specialty training in your desired surgical specialty eg vascular, general surgery, plastics etc.
      3. You could consider a clinical fellow post in surgical oncology if you already have surgical experience and just want to gain UK experience. I am not a surgical trainee, so I can't really comment on the quality of training in UK vs other countries.
      4. Yes, it will be considered but it will depend more on your clinical skills and experience.

    Please help in clearing few of my doubts.
    1. I have gone through the competition ratio for different specialty. So, not all get selected for their desired branch. Is there a second chance to apply, or that is the end of the road if you are not selected.
    2. Selection depends on what?
    3. I could see in the link, there is a selection process in each year of ST. So, how actually it works? I am interested in Paediatrics.
    4. Can you please give me the total road map to becoming a consultant, for an international medical student, starting from PLAB.

      If you don't get into your desired specialty, you can apply again in the next intake. The selection into specialty training will depend on you CV/portfolio and interview performance.
      After PLAB and GMC registration, apply for FY2 jobs in the UK. After FY2, you can apply for specialty training in your desired specialty ie paediatrics specialty training (8 years). After that, you will become a paediatric consultant in UK.

      Hi, if your primary medical degree is not from UK or EU, you need to sit for PLAB or one of the UK specialty exams like MRCP/MRCS/MRCOG/MRCPCH etc to register with the GMC and work here.

    Hi Lavnya, may I know would the salary be different between training & non-training job? Besides, would they give you a formal certificate after completing GMC registration as an IMG?

      There is no difference between the salary. The salary is based on your grade not on whether it is a training post or a non-training post.
      I don't think there is a certificate. You are just registered with the GMC. You can check any doctors registration on the GMC website. It is public.

      Hi, well done. Start by doing PLAB. After obtaining PLAB and GMC registration, you can apply for trust grade FY2 jobs here. After FY2, you can apply for specialty training in your desired specialty. There is no fees for specailty training in UK. You will be working for the hospital while you train. You will receive a monthly salary.

      @@dr.buddhiofficial3912 The pay scales for junior doctors depends on your grade. Starts at 30K/year (basic). As you progress your pay increases. Consultants earn up to 100K/year.The cost of living, it depends on where you live and your social life.

    Is there a post duty off for mrcp training doctors after doing night duty and what will be the working hours in the UK?

      Hi Abishek. You normally do 3-4 nights in a row. Yes you normally have 2-3 days off after working nights. The hours for nights are maximum 12 hours. (You must have minimum 12 hours break/off time between each night shift). For normal day shift it is 9-5. Oncalls/long days are 9-9. Hope this helps.

      @@anuragdas9318 You do not need to re-train again if you have already completed your specialty training in another country. So , you do not need to join ST3 (ST is a training pathway). You can apply for a trust grade/clinical fellow job and then apply for CESR.

      After MBBS, internship and GMC registration, you can start as an FY2. FY2 is normally for one year. These are some of the competencies -
      1. Venepuncture
      2. IV cannulation
      3. Prepare and administer IV medications and injections
      4. Arterial puncture in an adult
      5. Blood culture from peripheral sites
      6. Intravenous infusion including the prescription of fluids
      7. Intravenous infusion of blood and blood products
      8. Injection of local anaesthetic to skin
      9. Injection - subcutaneous (e.g. insulin or LMW heparin)
      10. Injection - intramuscular
      11. Perform and interpret an ECG
      12. Perform and interpret peak flow
      13. Urethral catheterisation (male)
      14.Urethral catheterisation (female)
      15. Airway care including simple adjuncts (e.g. Guedal airway or laryngeal masks)

      Hi Nikhil. Yes, once you have done PLAB you are able to work here in the UK as a doctor. Once you have decided what specialty you want to do, you can apply for the specialty training programme. There are seperate exams for the different specialties.
      If you have done MRCP- you can apply for the medical specialties- gastro, resp, cardio etc. If you want to enter into GP or surgical specialties- you need different exams for the respected specialties.
      GP training is 3 years in total after you have done FY1 and FY2.

      It depends on your clincal experience. If you have 2-3 years of internal medicine experience and complete MRCP, you can apply for a registrar level post.

I think you need to do something similar to that in your home country or be able to show that you have gained all the competencies/experience of an IMT doctor.

      Hi, yes you can do either exams. It will depend on what specialty you want to do and your experience. If you are interested in the medical specialties (derm, resp, cardio, gastro etc) and have experience in internal medicine you can do the MRCP. If you are a newly qualified doctor or want to do other specialties like surgery, paeds, obgyn etc then go for the PLAB exam. PLAB is a lot easier than MRCP. Most doctors only attempt the MRCP 2-3 years after medical school. PLAB you can do directly after medical school.

      Yes, you can use FRCR to obtain GMC registration and apply for radiology jobs here. If you are already a radiology consultant in another country, you don't need to repeat the training programme here. However, it will depend on your CV/portfolio and experience if you need extra training.

      Hi, to apply for higher medical specialty training, you need to have completed MRCP and internal medicine training (IMT) (3 years). If you want to skip IMT, you need to be able to demonstrate that you have achieved the competencies and experience of an IMT trainee doctor.       Most IMGs do a trust grade job for 6-12 months here before applying for specialty training. This will give you NHS experience and help you prepare for the specialty training applications.

      @@lavnya70 i finished my internship in 2016 then had some gap .then i got training in critical care medicine for 15 months after that i shifted to internal medicine have already completed a year and abiut to start second year now.
      So m not sure that expereince of critical care they will consider in domain of int medixine or not.

      @@raisamuhammad6148 Hi, yes, your experience in critical care is part of internal medicine. Most doctors here do intensive care as part of internal medicine training (IMT).

      @@raisamuhammad6148 Hi, try and keep a log book with all the procedures/ cases you have done. Your clinical supervisor/ senior doctor should be able to sign it off. It would be good to have a recommendation letter noting that you have gained all the necessary competencies as well.

    Hi... After completing my PLAB 2 ... What is the procedure to pursue post graduation in dermatology.. Which exam is to be given.. What is the exact process? How much time it will take to finish dermatology? What is the passing percentage of that post graduation exam? Is it easy to get dermatology?

      Hi, once you have done PLAB- apply for a trust grade job here in the UK (for maybe 6-12 months). This will let you gain experiance working in the UK and gain the competencies you need to apply for Internal medicine training (IMT) (3 years). Once you have completed IMT- apply for specialty training in dermatology (4 years). Then you are done. Dermatology is one of the competitive medical specialties. But it is still achievable if you have a good portfolio. During your IMT, make sure you do audits/quality improvement projects/posters related to dermatology to show your comittment to the specialty.
      During IMT, you will need to complete the MRCP. And during dermatology specialty training, you will need to sit for the dermatology SCE. There are no other exams. Specialty training in UK is not exam focus. Most candidates do pass the specialty exam.
      Btw, this route is for junior doctors (1-2 years after medical school). If you are already a dermatology specialist in another country, you can apply via a different route.

      @@faizanalikhan6764 MRCP is a postgraduate exam. If you prepare well, you should be able to pass. You don't need MRCP to apply for IMT. IMT is not competitive, so as long as you have met all the requirements, you should be able to get a place.

    hello there, could you shed some light on the following questions

      Hi thank you for watching the video.
      1. You can still sit for the PLAB exam even if you have not practised. As long as you put in the required work for the exam you should be able to pass. I would suggest applying for a trust grade job, if you are thinking of working in UK. Some trust grade jobs are rotational so you do 4-6 months in different speciality as a junior doctor. This will give you a chance to see how the system works and which speciality you are interested in.
      2. If you are thinking about dermatology, once you have completed your PLAB and have have obtained all you Foundation Year 2 competencies ( you will be able to do this while working as a trust grade) apply for internal medicine training (IMT). IMT is for 3 years.
      3. The comepetition ratio varies for different speciality. Have a look at the competition ratio for 2018 (linked in the description box of this video). If you are not a resident in UK, your application will be subjected to labour market testing. Having said that, if you are not to fussed about where you train, you should be able to get your preferred speciality. Do make sure that you have a good portfolio and it is relevant to the speciality.
      4. You only need complete MRCP during your IMT. So you still have some time.
      5. It really depends on how much work you put in and if you have other commitments. Have a look at the sample questions for PLAB . This will help you gauge how hard the test is and how much work you need to put in.

    Hello doctor, I have one question to ask. I am an Indian medical student and pursuing my medical studies from ukraine. I read in the GMC guideline that one needs to complete an year of internship or complete FY1 & FY2 to be elegible to sit in the Plab exam. So my question is that as a IMG can I enter in the UK medical foundation program or is it available only for the UK medical grads ? If its possible then how hard or easy it is for an IMG to enter in the foundation program in the UK? Is there a tution fee that one has to pay while in their FYs ?       Hi, yes you can apply to enter the foundation programme in UK. It is comepetitive for IMG as the places are limited. You should be able to find all the information regarding the application process, timeline, PLAB on here www.foundationprogramme.nhs.uk.
      Alternatively, if you do not manage to get a place, try applying for a trust grade FY1 or clinical fellow FY1 (foundation fellowship programme). For this, you apply to the hospital/trust directly. www.jobs.nhs.uk
      Search for foundation doctor jobs.
      Both routes are free and there is no tuition fee. You will need to apply for a visa and there will be a charge for that.

      Hi, once you complete PLAB 2 you should be able to register with the GMC.
      What do you mean by skipping PG internship?
      Have you done internship already?
      IMG can apply for the foundation programme but it is via the 'eligibility route' . It is more competitive but still possible.

    After MBBS in India, what exam would you take if you wanted to get into a cardiac surgery residency? PLAB or MCERP? I’m confused about the difference between them. Also, would I have to take any extra courses or clinical rotations after MBBS before getting into residency? And if after completing my residency, I wanted to shift to the US to do my fellowship, would that be possible?

      I am not sure if the specialty training in UK is recognised in US and if it is possible to switch.
      This is the pathway to become a cardiothoracic surgeon in the UK
      After MBBS, internship and PLAB, apply for FY2 jobs in the UK. (Route 1) After FY2(1 year), apply for cardiothoracic training (8 years).
      Alternatively (route 2), after FY2, you can apply for core surgical training CST (2 years), then higher surgical training in cardiothoracics (6 years).
      Both routes, you need to take MRCS and FRCS during the training period.

      GMC registration here. Once you have obtained GMC registration, you can apply for a job here. You should get a job almost immediately as there are so many doctor jobs (trust grade) available. Once you have worked here for 6-12 months, and have obtained all the competencies, you can apply for speciality training. Ideally you should have some experiance working in the NHS before applying for speciality training. General practice (family medicine) is a different speciality and has its own training pathway. What speciality would you like to do?

If you have not done the internship, you can apply for a Foundation Year 1 grade job in UK- this is equivalent to an internship.     Hi doctor.... i completed my mbbs this year in March with 1 year of internship from India and took a complete year of gap ... planning to give my plab exams next year ..... any advice for me.... like what course to apply to after my plabs??? Also should i take a job here in India next year while my plab exams are going on..ll it help or is it unnecessary??
    Also for a speciality like paediatrics or radiology do we need mrcp after core training?

      Hi, it is best to keep working as the GMC likes doctors to be 'in clinical practice' before applying. After PLAB, you can register with the GMC and work in the UK. Apply for a trust grade FY2/ SHO post. Once you have done FY2 here, you can apply for specialty training. No, MRCP is not needed if you want to pursue paediatrics/radiology. MRCP is only needed for the medical specialties like resp, cardio, derm, rheum, gastro etc. Paeds and radiology has its own PG exams.

      Maam does gap year affects the probility to get matched .... how much of the gap period is allowed?

      ​@@arnabroychowdhury9366 No, it shouldn't. There is no fixed gap period. It is up to you how long you take. You must account for your gap year, ie reasons for taking a career break. When you return to clinical practice, you might need a phased return.

    Hii dear,which visa is required during foundation years?if it's tier 2,is it easy to get it,I mean will hospitals sponsor img's easily?and if I get a FY2 post,can I sponsor my husband as a dependant?sorry for the long post,hoping for a reply soon🙂

      As long as you have secured a job- you can easily apply for the visa. Yes, the hospital will be your sponsor for the tier 2 general visa. Yes, once you have obtained a tier 2 visa, you can apply for a dependent visa for your husband.
