I think the reason why this movie is so mercilessly criticized is, despite all the talent involved,it was dumb.It has a first rate cast,high production values,excellent cinematography,and a beautiful and haunting musical score.All this was offset by a ridiculous script,and laughable dialogue("The good locust","The wings have brushed me").Plus it just wasn't scary.I still kind of enjoy this film,warts and all.
The movie was doomed from the start. The exorcist effects was passing and executives wanted more. They pressed for a second movie and gave resources but they forgot Blatty. That mistake is what makes this movie a monumental fail. They thought they could create a movie with special effects and mediocre to bad story telling. Using the face of Linda Blair and some good actors the result was a nightmare, but not a quite scary one.
This is one of the only sequels that really tried to be different from the original. It earns a special place in my heart for that alone, no matter how ridiculous it is.

I've always loved this movie, because you really have the feeling you're entering a supernatural world full of mysteries. The first movie has absolutely no atmosphere.
I personally always thought that the first Exorcist movie‘s charm *is* its “lack” of atmosphere. (Actually, I wouldn’t say it completely lacks atmosphere, as a few of the scenes are atmospheric, but for the most part it doesn’t have a strong one.) And I think that’s part of what makes the movie so interesting and so chilling for some people. Atmosphere is cool, but the first Exorcist film is supposed to feel eerily realistic-hence why it barely even uses background music. It gives you a sickening sense that this is real, or that it could really happen. Sure, you miss out on the cool things like a creepy atmosphere (background music,“setting the mood” with eerie visual effects, etc), but for some people (like me), that’s the magic of the first movie. Of course, some moviegoers don’t appreciate that sober, realistic form of filmmaking, and prefer a more visually and sonically immersive experience. It’s all taste.
There were some nice touches (locusts, hypnosis, the brilliant music score, the exotic locations) but the final result is beyond ridiculous. Even the name Pazuzu is ridiculous (it was never mentioned in the first movie).
