How to Treat Melasma

  • A question that often comes up is how to treat melasma. Melasma and hyperpigmentation can often be difficult to treat and maintain. Watch this video to better understand the different types of treatments available and why melanoma can be difficult to control.
    I earned my Medical Degree from UC Irvine and completed my residency in Dermatology at UCLA. Upon completing my residency, I joined the full-time faculty at UCLA as a Clinical Instructor and Professor in Dermatology and Dermatologic Surgery. I then went on to complete advanced fellowship training in Dermatologic Surgery at UC Davis and Cosmetic Dermatology at the Laser and Skin Surgery Center of Northern California with world-renowned expert, Dr. Suzanne Kilmer. After fellowship, I was appointed the Director of Dermatologic Surgery at the Laser & Skin Surgery Center of Northern California where I remained active in clinical trials and have been on the forefront of the latest advances in aesthetics and dermatology. As an Orange County native, I love the SoCal community and currently enjoy being a Dermatologist in Newport Beach, CA.
    My areas of expertise include cosmetic dermatology with a focus on injectable neuromodulators (Botox, Dysport, Xeomin), as well as dermal fillers (Juvederm, Voluma, Volbella, Vollure, Sculptra, Restylane, Belotero, and Radiesse). In addition, I travel nationally and internationally where I teach other physicians injection techniques and about the latest advances in dermatology. I am highly-trained in the use of lasers and energy-based devices to treat all aspects of photoaging, including fine lines, wrinkles, skin laxity, brown spots and facial vessels. I offer a comprehensive approach to body contouring with the use of minimally-invasive procedures. In addition, I specialize in Dermatologic Surgery including Mohs micrographic surgery, blepharoplasty, skin cancer excisions, sclerotherapy, surgical earlobe repair and cosmetic tattoo excisions.

    Dr. Kappel, I CAN'T begin to thank you enough for sharing your knowledge and love for skincare. It's incredibly difficult to navigate all the information that exists in this day and time when it comes to skincare and skincare products. It's so easy to get misdirected when it comes to skincare and what to do or not to do, especially when you have hyperpigmentation issues.     Would you please clarify the heliocare sunscreen and also the "heliocare" enzyme that is an internal supplement thats an antioxidant? I have listened to that portion of this video so many times and am missing something.

    Thank you so much for this info! I see Heliocare has a regular, ultra new advanced formulation. My initial thought is to go for the advanced version but I also know more isn’t always better. Your recommendation would be appreciated :-) ty

      The Advanced has niacinamide in it and the Ultra has pomegranate extracts and may be a little more pricey. The regular Heliocare is a little less expensive and has the main ingredient that you need which is the polopodium leukotomas ;)

      I have tried so many products and don't see any result so it is getting very frustrating. Thank you.

    Could you please address keratosis pilaris on the face? I have red bumps on the cheeks. How do I get rid of it? Moisturize, or lazer or v-beam?

      I had just gotten a facial 2 hours before posting that video 🙈 I was still a little pink from dermaplanning and a hydrafacial.       Hi and thank you!! Over 60 and only one product, I’d go with the Hydrator (packed with skin tightening anti-aging peptides) or the eye serum (helps with under eye wrinkles and puffiness) 😉

    Dr Kappel, first of all thank you so much, your videos are impressive - I recently discovered you on YT, you were highly recommended by a friend of mine who is an MD .I can see why she thinks so highly of you! I have a question with regard Clear and Brilliant . I have done it only once. While I wait for my 2nd and 3rd treatment, is it worth to " pretreat" the skin as you suggest for other lasers ? I am 55 and I have some sun spots but also some post inflammatory hyperpigmentation ( from filler needle injections believe it or not...),which I would love to remove, but also skin laxity, fine wrinkles. I have been using Adapalene and Defenage as my main skin care routine. Your suggestions to maximize results are greatly appreciated!

      Hi and thanks so much for following!! Yes I always tell my patients that what you do at home is just as important as treatment you have done in the office. Its important to use good products at home between laser treatments. If it is pigmentation you are trying to correct, I recommend the ZO Skin Health pigment control kit. This contains a 4% HQ and is not available without a prescription. If you are looking for a non-HQ I would recommend the SkinBetter Science EvenTone or my Vit C serum (MDAiRE). These will all stabilize the melanocytes pre and post treatment. Alastin also has a regenerating nectar that is great to prep your skin before and after laser. Hope this helps!!!

      @@DrStefaniKappel Thank you so much for your suggestions.I do have a questions with regard how do product work (perhaps this would be good topic for a video and if you have already addressed it then let me know) .If the epidermis is such a great barrier, how do products, cream, serum, etc actually penetrate this barrier? Is there only a small percentage that is absorbed? What should we, average consumers, be looking for not to fall for hypes? As I mentioned above, I use Defenage. I used in the past Obagi for a few years, loved the result with the HQ system but my skin was quite dry, not to talk about the sulfur stink of the product . So I switched to Defenage for a few years, with either Adapalene or I used one tube of 0.025% RA. However, all I can find about Defenage is that they have done their own studies and biopsies. I like the way the products feel on my skin, but can I say my skin has changed in such a way that I can actually see? I am not that confident. Sorry if this may be a repetitive theme. Perhaps there is a rate limiting factor at one point in age where no matter what you do you cannot stimulate collagen, elastin production?

    Is there a way we can find a laser surgeon that’s highly rated in our area?

      Usually physicians list their credentials on their website. Where they went to medical school, where they completed their internship and residency, etc. Look for providers who completed a laser fellowship which will be listed as an “ACGME procedural fellowship”. You can also use the ASDS website to search for provides in your area. The American Society for Dermatologic Surgery is a great reference too. If you have trouble finding one, let me know your area and I may be able to recommend someone near you. ❤️

      @@samr5816 yes and her credentials are very impressive. You want a doctor who talks about his/her credentials. Not how many offices and businesses they have. Just my opinion. This physician is highly impressive to me as a colleague and I’d recommend her without even knowing her personally.

      I owe it all to you. You started me on my journey to great skin. However, I am a African American and would like to find a laser specialist who s experience in treating this type of skin. I live in South Carolina. Any recommendations? I will travel a little for the best

      @@katrinaconner6482 I’m so happy to hear. African American skin can be safely treated with lasers when done correctly. For instance, appropriate settings with a clear + brilliant laser can give amazing results. I cannot give medical advice over RUclips of course, however I can recommend someone near you.     What are your thoughts on using IPL and pretreating patients?

      IPL isn’t truly as laser and doesn’t work as well as lasers in my opinion. Pretreating and post treating skin is a good way to manage and prevent hyperpigmentation in all skin types.
