    Last 2 weeks I was diagnos with breast cancer and this week thursday the result from the core biopsy was her2 3+ stage 4 breast cancer and my doctor will give me herceptin+perjeta(phesgo) and docetaxel and will do this set of treatment 6 rounds and I hope after that 6 rounds we will get a good result... thank you so much for the information it helps a lot knowing about this disease and not to be scared from it😊 posting this comment today march 16, 2024

    Thank you for all of the time you gave to making these videos. I am about to start treatment for Inflammatory Brest Cancer, we are waiting on the PET Scan to find out if it's Stage 3C or 4. I am a little concerned about how long it is taking to get started on Chemo. I was diagnosed December 6. I won't start Chemo until the first of the year.

      I am so sorry that you are facing this. The 3 week delay, although not ideal, doesn't worry me much. It's important to fully stage your cancer as you know for so many reasons. Please keep us posted on your progress as we wish you so very well.

    Hi Dr! Thanks for this video. Is phesgo a safe option for the THP treatment protocol? Or do you use herceptin and perjeta separately? And if so then why? Thank you very much!

      A lot of people like to use it separately when given with the taxol and then switch to phesgo after the taxol part is complete. However, with that said I would have no issue if your physician or you wanted to use taxol + phesgo. The only thing about that is often times you want to be able to delineate which side effects are attributable to which drug. For example, say you had a marked rash. That could be taxol, herceptin, or pertuzumab. If you give the drugs separately you can tease that out a bit better than if you use them in combination. With that said, it's really splitting hairs and it would be fine if you wanted to use the phesgo with taxol in theory. It's just that in practice people like me like to wait until the taxol part is done and you're on "phesgo maintenance".

      I had one more question please- does being diagnosed as de novo stage 4 have an impact on treatment plans or prognosis? Thank you again for all that you do! God bless 🌸

      It doesn't have a big impact on treatment per se, but one would expect a de novo patient to potentially have a better response as the cancer has never seen treatment before, but take that statement with a grain of salt.

    Hello Dr, it’s me again with one more question pls. What happens to HER2 targeted treatment (phesgo) success if the PIK3CA gene is found as well? Does it render the phesgo useless? Or are there other drugs to deal with the PIK3CA that’ll let the phesgo do it’s job as well? Thank you again- your videos and words are all a blessing.

      It does not render phesgo useless. There are other drugs that target PIK3CA (alpelisib, copanlisib,etc.), but they are not FDA approved for HER2+ breast cancer.

      So you’d say that PIK3CA diagnosis is not the end of the road for her2 positive metastasic breast cancer? Targetted therapies can still work? 🙏🏻thank you again- your videos have given me so much information and hope.

      Generally it is not. Immunotherapy can make rheumatoid arthritis markedly flair.

    I appreciate you for saying this ❤❤❤❤ I really needed to hear this because most doctors don’t make you feel like this. when I was diagnosed 2 months ago after COVID-19 VIRUS 🦠. I have a swollen lymph node in left arm and from there that’s when all the problems started. I started immunotherapy today IV, and I hope 🤞🏾 it goes away immediately because I still can’t wrap my mind around this.
