  • Lower Limb Neurological Examination - Power, Reflexes, Sensation, and Coordination - Dr Gill
often neurological issues will affect the limbs, so the "complete" neuro examination can be considered to be split into three sections:
    - The Cranial Nerve Examination

Today we follow the Appearance, Tone, Power, Reflexes, Sensation and Coordination approach to the neuro exam for the lower limbs
    #NeurologicalExam #DrGill #ClinicalExamination

Neurology as it pertains to the lower limbs is something that has always piqued my interest because of the condition I have suffered from all my life.
As far as I know, given that I live in the United States of America in a lower-middle class house, I've had a condition that doctors were convinced was a self-inflicted issue. When I learned to walk, I had extreme difficulty putting my heels to the ground. I could only walk on my tip toes, and the most I received was occupational therapy to try and get me to stretch the Achilles tendon. eventually, I stopped getting that therapy through the school system and I was left with my parents attempting to discipline me for not overcoming my condition. I was plagued with lots of allergant related issues as a child and a relatively weak auto-immune system when I was a child, which lead to frequent cold and flu, which even then my mother was reluctant to take me to see a doctor because of the expenses incurred.
I would like to see medical professionals for a number of reasons right now; I clearly suffer from extreme bouts of depression and anxiety. Walking has also become a far more difficult task as I have matured. I'd... like to not think it's my fault that I never learned to walk correctly. that's the place I've been put in my entire life and I really tried to reverse that throughout my childhood, but...
I guess the thing I want to ask is if my problem could possibly be an issue with a neuron lesion or such. I do exhibit tremor in my limbs when I'm not particularly focused, although if I pay attention I can get it under control, and I have full sensation in the limbs, even if I struggle with coordination in some ways. I suppose that's what the examination is for. that's why I listen to videos of doctors going through these routine exams to try and lull myself to sleep at night, to try and associate it with a sense of security and comfort rather than anxiety over financial devastation.

With respect to gait and mobility, is there an appropriate test to ask for if I was to see a GP about an ankle concern? I dislocated my ankle a few years back and unfortunately even after reduction and a surgical procedure to plate my snapped fibula, it's never quite recovered properly, particularly in dorsiflexion, and seems malformed. Physio never seemed to fully address it and I'm worried that it'd need some kind of further surgical procedure to resolve!

Do you know where the studies are on the efficacy of the 3 word recall test or how many words that start with F? There are 3 little articles I’ve found, but not any decent studies to justify giving that test to the elderly. I know it's a standard test and I’m betting someone came up with it decades ago, and it has just been passed on down without any clear research. I’m writing a graphic memoir of how I used my health and fitness knowledge to stop my mother with mild dementia from forgetting me, and it worked. She always knew me. I’ve been flummoxed on the actual studies for the 3 word recall test and the cube drawing test. I want to put that in my book.

Do medical students get introduced to examples of abnormal conditions while still in a classroom setting or do they interact with them for the first time during rotations? Clinicals? (I'm not sure of the term I'm sorry)
I have quite the fun range of medical conditions at a relatively young age (@21 I have/have had an experimental typanoplasty, ehlers-danlos syndrome-hypermobile, Mixed Connective Tissue Disease, POTS, and an unknown heart condition we're calling Inappropriate Sinus Tachycardia for the time being along with a few others) so I usually have medical students sitting in on my appointments, ultrasounds, and procedures to a range of reactions and confidence levels. I was just wondering if that was likely the first time they had seen the conditions or if they've seen it before in a classroom setting.
And thank you for these videos btw!

Parts of the exam will happen without you realising, eg I go and collect my patient from the waiting room- thus seeing them get out of the chair and walking into the clinic room
If there is something there, then I’ll look more closely.

You really are a good Dr that really does do his job. I walk to the room myself and wait 1-8 minutes for Dr to show up then I am basically rushed out. I have never had a full exam for anything done ever. Any tests I've ever had done results come back fine but I am not fine so??? Clinic Dr's same I am rushed out. I hardly ever go to the Dr cause I sadly feel its a waste of time and never get any resolve.
