- Peripheral Vascular Examination OSCE - Clinical Skills - Dr Gill
In the cardiovascular examination, particularly in the case of an OSCE station, we conclude the examination often by stating that the examiner would want to perform:
- An ECG
- Check full blood count
- and "do a peripheral vascular examination
In this video, we demonstrate that oft-talked about, but comparatively less common examination.
Starting off, with the examination of the hands, the radial, brachial and carotid pulses. before moving down to assess for a AAA, checking the femoral and popliteal pulses, before wrapping up around the ankle with the posterior tibial and dorsalis pedis pulses
For completeness, the cardiovascular examination is demonstrated here

What a great April Fool's video that would have been.
🤣🤣🤣
never seen that comment before....
Sometimes I imagine Dr. Gill just doing his regular exams , but instead of giving his usual positive comments he just goes :
"It's all wrong in here"
"Terrible"
"Nothing is alright"
All in his usual calming voice
Omg that would be hilarious
Your voice itself is a medicine.
Well there were a lot of people who bullied me at school for my voice would strongly disagree lol!
Not everyone got a good taste though. 😂
@@DrJamesGill it's amazing how we all shine in the right light! Thank you for your work.
@@DrJamesGill they were just jealous they don't have such dulcet tones
@@DrJamesGill I would kill for your voice ngl
Love how Oliver folded his shirt! 👌🏻👏🏻
My mum trained me well
@@OllieBurtonMed We’re all proud of you
@Ollie Burton You've met the legend. I envy you, sir.
@@OllieBurtonMed yes she did 👏🏻
🤣
I'm waiting for the day when someone does have questions for you
Not in the script 😉
@@DrJamesGill time for some improv then.
The legend hits again.
Well, not sure about hits, more carefully examines
@@DrJamesGill lmao
@@DrJamesGill The legend palpates
I feel like at this point the med-students started fighting over who gets to be the patient in the video haha what a privilege
🤣🤣 I wish. It’s often really difficult to get volunteers, and don’t want to over burden those who do offer help
@@DrJamesGill I would volunteer. Problem is that I'm on the other side of the North Sea...
@@DrJamesGill shame that many don’t want to be part of your videos,it would help bring more in to learning perhaps?
@@Tommovdv only a little swim
@@DrJamesGill I volunteer to drive down from Newcastle to help!
There’s a lot of hard work put into these videos and it’s appreciated, Dr. Gill! Even for those, like myself, who don’t actually study medicine.
Thanks. But it is enjoyable hard work seeing people benefiting 😊
I am a simple man: I see a Doctor James Gill video, I click on like
👍
Wow, that guy is the first ever to actually fold his shirt neatly and not toss it on the floor like everyone else. Completely difference in hygiene 👍🏼
But then he put his feet all over it when lying down :D
I am enamoured of the way he folds his shirt. so neat 😊
This man’s chest hair is top notch.
My thoughts exactly 💯.
Why not examine the pubic area too, 8t is a part of our anatomy?
As if the blood is going to the feet, it’s showing there is no issue higher up
I think many ppl that enjoy these amazing videos for asmr purposes enjoy these because there’s something utterly relaxing on imagining being a patient that’s not actually ill and just let go and be examined. Giving up control for a while..
Why is Dr James Gill the Bob Ross of medicine? So calming and soothing ...
If the doctor thing doesn't work out James, you are perfectly suited for narrating good British documentaries.
Nature documentaries too haha.
Imagine him saying in his calm voice “The lioness is victorious in her hunt, now watch as the pride devours their prey”
@@etchieSketchie
Hahahahahahahahaha
He could definitely tell us about the great Emperor Penguin males protecting their single egg during the brutal Antarctica winter while the female is off in the ocean feeding.
The call of cthulhu😂
I swear it! He is the Bob Ross of medical exams.
I love that you hold your breath with the patient. Sometimes students focus on listening so closely they forget to say 'and breathe again', poor patient starts to turn purple, lol. Looks like the new editing program is working out? Hope you're getting the hang of it now 😊
I find it a very useful trick for the students. Plus it improves rapport
I am awaiting this most peripherally
🤣🤣 so at distance?
Keeping 2 metres from my phone until your video premieres.
😂
@@markmccallum475 so, typing on a Bluetooth keyboard 3 times the size of your phone.
Your examination videos makes us feel better from pandemic stress. Thank You Dr.Gill :)
I’d be curious to see a video where something is abnormal or there is a sort of bruising to see exactly how different it would be versus a non-bruised/non-abnormal check up
It's such a pleasure to watch any of your videos
Thanks. Hope they are useful
@@DrJamesGill For medical purposes, maybe. For asmr, hell yea
@@DrJamesGill they're fantastic! While I am not a direct medical professional, I am in a human services field serving a lot of medically fragile folks. They're super helpful for me both due to the ASMR but also for the medical knowledge - I equally love the ones that are role-play style and the A-Z ones you've done that are more technical. Your voice is easy to listen to, your mannerism is very confident but not haughty like many doctors are (at least the ones I'm used to dealing with). While I definitely don't feel like I could take over for a nurse or anything crazy, I do feel a lot more confident about my monitoring of symptoms and having the language to tell medical staff whats wrong instead of "they aren't themselves"
Very nice! I liked how you show correct hands' placement in each situation under an angle which provides a convenient view. Editing and shot change is also very good to my mind :)
This is why we started doing the videos on the first place, so students at the back of the room could see a “students eye” perspective on where the placement was
I'm using this video asmr... I m korean but this channel is so good. Thanks dr.gill
Same here
I've always wondered if anyone has said no to you mr gill on an examination if they have just...
Why how could you refuse this legend
Just amazing! I love the way you don't go super fast! Way easier to see what's going on what to remember!
The medicine i need to get me through the pandemic = your voice , thanks for putting me to sleep doc
I was never very good at anaesthetics!

ah Dr Gill...another video to look forward too..you do know how to make a midweek feel good
My daily night routine is watching your videos before sleep, I was very excited when I saw this video on the Recommended Videos.
Ollie & The Gill...love this crossover event!
It looks like the roll of Aiden Cross has been recast.
Aidan can never be duplicated!
I didn't recognize him until he removed his shirt:D
i wish nothing but success for you and your channel Dr. Gill 👍🏿 good health to you
If you check my feet temperature, diagnostic would be "Yes, dead".
Thanks for another one, Doc. Hugs from Brazil, Kelly. (Already missing all that hair!)
This one was extra relaxing! Thanks Dr G
I'd like to see Dr. Gill as a field medic in war games
Oliver is SPLENDID♡
Oh man I love this kind of videos, thanks sir !
Thank you for the knowledge. I didnt even know this was something medical staff look for. 👍
It’s important in cardiovascular patients and in patients with diabetes as two simple examples
Noticed the Armani Jean's near the start, our Dr Gills got style!
Another great video from Dr Chill
so funny at the end of the video when he was checking on what you were doing to his legs hahaha
You can actually tell that the video has been edited differently, I'm glad You've got to grips with premiere (or at least it looks like You have) and I'm looking forward to the quality of the videos improving as You use the software more and as Your channel continues to grow!
Thanks. I could be wrong, and my previous self will hate me for this - the work flow BASICS are simpler with Adobe. But FCP does have features I miss
Oh and Thank You so much for the work You've been doing during the pandemic like don't get me wrong I loved the NHS before but I've found a new level of love for the NHS since the pandemic
@@DrJamesGill Yeah it was pretty easy to get to grips with adobe when I first tried it, however, I've not worked with Adobe in too much detail so I wouldn't know but I assume there would be a way to get the features from FCP in Adobe or some sort of bodge which works the same either way You could tell there was an improvement especially with the intro.
The intro was a new idea, I think it needs more polish though
I wish I had a doctor like you, btw loved the Armani pants
Great video! Thank you
He’s back at it again for another round POG
Dr.Gill, with that voice, I'll leave everything to you
I really enjoy Dr. Gill's videos but does anyone else prefer the wide shots were you can see him looking at each specific thing hes doing instead of an big close up with just his hands?
Another quality video!
Love my A & P
Great video as always.
The work with the new video editing software is nice! Not sure if this makes sense, but the videos feel more crisp
They are so relaxing!
I think ima watch this man til im 110 yrs old!!!!👌💪💯🙏
This is crazy I've watched your videos for a while, then started watching the island series on Netflix and i was like hey! I know this guy? I know its years ago but new to me and you done a great job on the island 🖤🙌
Thanks. It feels so long ago!
(I’d really like to go back given we now know a lot more about how to survive there!)
wait was it a show??
why is your voice so relaxing and beautiful 😍😍🤔greetings from Argentina👋🏻🇦🇷
Thanks for important video if possible please put more medical examination video
Abbie looks different...
Abbie. Ollie. They are basically twins 👯♀️
Something I have noticed in real life, whenever I have found myself being examined, is that there is often a somewhat frantic quality to the procedure with the doctor racing through the examination and even getting a little breathless with the "script"; I can understand that the time constraints in clinical practise might be responsible for this but, in the context of both an instructional video, and as ASMR entertainment, I think your videos could benefit from slowing the pace a little to make learning easier, and watching (from an ASMR perspective) more enjoyable.
Dr. Gill: "Hello! My name is Dr. Gill. I've been asked to do..."
Me: "Z z z z z z...."
Awesome gill
is there any chance you’d be able to explain why you’re doing the actions whilst you’re doing them? like why you have to push onto the stomach and what you’re looking for etc
love your videos btw!
The “Medical deep dive” videos where we do that are coming, but they take longer to film.
There are several in the deep dive play list 😊
‘....I can’t see anything obviously worrying....’ Reassuring 😂
I have been watching your videos for a good bit now and I think I have got the gist of this whole medical thing.
I have some room in my garage to set up a Doctor shop.
Any tips?
I give him a 4 out of 10 for the taking off of his shirt; his form could use some practice but a nice touch at the end in folding it; adding 2 points. That brings this overall score to 6 out of 10.
He is so polite about it!
Petition to make Dr. Gill the next Doctor.
THE FOLDING OF THE T-SHIRT!!!
It would be interesting if you had someone who wasn’t perfectly healthy just to see what happens next sort of thing
Useful, but the point of osce’s is to learn the exam. The pathology comes later.
@@BayernLean thank you, sometimes you don’t know if you don’t ask thanks for educating me :)
No problem. Cheers.
It probably has some problems with privacy too. He cant just ask his actual patients to get on camera probably. But maybe some day a person with a medical issue will volunteer
@@markvandenberg8548 very true I didn’t think of that either!
All be it, never actually turned up to one of these. But I'm looking forward to it nonetheless.
The quality somehow gets better and better 📈
Love his voice. One of the best European voices ever. Most sound stuffy, whiny or like they are bullfrogs. His is perfect
Excuse me what lol
Nice 👍🏼
His voice is better than a anxiolytic medication
Something I always wonder about is the camera angles. How good is his setup to get those close up shots from multiple angles
It’s one camera and a LOT of refilling
Another great video. I have to ask... are those armani trousers?
I'm just fascinated by the way the hair on his knees looks a lot lighter than the rest of his legs. Like, I know there's less hair, but it looks different. I think it's cool.
Is my man Gill rockin Armani jeans?
Is Oliver an understudy for the role of an Austin Powers sequel with that chest hair. Yeaahhh Baby!!
This is perfect ASMR
Super!
yees a new Video!!
👍
“Hello, my names Dr Gill and I was wondering if you wanted to be in one of my videos?”
“Uhhh 100% mate!”
“Fantastic, thank you!”
Much harder to get students to volunteer than you might think!
Interesting. Thanks 🏅👍🙂
Thanks for watching!
aw Oliver we're birthday twins! Happy belated birthday :)
Happy birthday!
Some love for the shirt for a change!
Im rarely this early, i like
I like the new intro you made. I think it could be improved if you filled the black space in the top left and bottom right with more clips as you pan to the actual video
lovely
Right where do I sign up to be a test patient, it would be an honour
*signs up as test patient because U.S. healthcare is too expensive* 😅
can you upload a video where you find something irregular on the examination?
What's your take on digital vs manual sphygmomanometers?
Could you do a video about Raynaud’s Phenomenon?
Why take the blood pressure without and then with the stethoscope? What different information do you get with that as opposed to just taking the pressure with the stethoscope?
Very nice
You practice medicine for how long now , nice videos very detailed
I've never had a check like that before.. but the whole knee thing.. pushing up under the cap had me cringe.. under my knees like that hurts when touched..good video though. Keep up the good job.
How would being right-handed or left-handed affect your blood pressure? And why take it twice?
Great video - overall, I found it very informative. However, I recommend (also would appreciate) you provide a little bit more narrative, particularly on the inspection section because, as students, that is the expectation from us on OSCE stations. For instance, on hand inspection, you said '"everything looks good there", rather than telling us what you're looking for or what you mean by 'good'. Also, you haven't summarised your findings either.
There is a specific reason for that. These have been designed for use in our clinical skills sessions, so we can pause the video and ask the students what is being looked for
The deep dive videos - on the medicine explained playlist are where I’ll do all the detailed bits for YT going through exactly what you just asked
Is that a digital stethoscope? Do you think you could put in the audios that you hear as well?
This one doesn’t record, only amplifies - BUT it will be something I’m looking into for the deep dives - I will have to buy the sound licenses, but I think it will be worth it 😊
I think it would be cool to hear that.
im just curious, what would be different (if anything) during the examination if the patient were to have ibs or other issues where pressing on the stomach would, in fact, hurt
im not interested in a medical career but i do have myself some medical issues and always find it calming to watch these and i usually think about how it would be different
It may cause discomfort, but we’d put it in context with the history
