If that happens,then maybe your needle is too big for the fabric which causes it to make a big hole in the fabric I recommend u to buy a bigger hole fabric or a smaller needle Hope it helps!!

check if your fabric is TIGHT. like, stretch it as much as you can while securing it. you have to also unravel your yarn while punching and only pull out as much yarn as you need.
