    I'm a dental hygienist...for 25 years and can no longer work. No one has ever mentioned scalenus syndrome to me! I've had Cortizone injections in my shoulder and elbow, and extensive physical therapy. I'm in pain every day. I am so relieved to have found your videos! Just doing some pressure on trigger points, and stretches have relieved more pain than my doctors have in the past nine months! Some days the numbness and tingling go all the way up into my jaw and face! I will work on triggerpoints and posture and stretching! So glad I found your channel!

      Terry, I am a retired dental hygienist; class of 1978. I have your exact same pain, tons of money spent on physical therapy, and one neck fusion of C-4/C-5. Chiropractic care has helped, retiring, physical strengthening, Dr. Mandel's scalene muscle stretches, and one other treatment done by my physical therapist. There is a treatment modality called dry needling. I hardly have pain anymore. I have to stop forward head tilt; posture is everything. Good luck. Lynne Holland

      HiTerry, I had a work injury 2 mos. ago which triggered all my back up to my neck pain to get worse. I had a massage which left me with numbness to my neck and tingling on my face, I really got scared. My PTherapist is really good, he is doing the same 2 last exercises on me. But I am just so happy to find this channel. It explains what the conventional doctor can't or won't because they only spend less than an hour with you :( BTW I found a good massage therapist after seeing 5, who works on my tight muscles.

    Thank you so much Dr. I have been plagued with left arm pain, shoulder pain, hand pain, neck pain, back pain and even shortness of breath. All the symptoms are also signs of heart disease. After having a panic attack (thinking heart attack) I went to the ER. They told me my heart was fine and sent me on my way. So for another year I suffered panic attacks thinking something was seriously wrong. I decided to investigate myself and ended up hearing about this muscle and watched your video. Oh my god! the first relief I have felt in years! After rubbing the muscle and stretching something just seemed to release and I was able to take a full deep breath and the pain almost completely subsided. Thank you from the bottom of my heart, I thought I was going to have to live with this my whole life. No doctors could tell me what was wrong until you, thank you so much!

      dude same.. ive had so many panic attacks over this thinking its something going on with the arteries in my neck bursting or something. been to my Primary care and he sent me to a cardiologist because i have constant anxiety with elevated blood pressure. im on beta blockers for it now for rapid heart rate from adrenaline rushes. im certain this is my problem too. hope youre still doing well

      This is exactly what I'm going through at the moment including wasting 4 thousand dollars at the ER because I thought I was dying with fast heat rate, pain and shortness of breath, shoulder and left arm pain/numbness.

    You touched on- the combination of confluences, which can create syndromes, which is most important point. In many instances muscles turn 'on' and stay on, and other muscles turn on to counteract the first set, and create, pain, fatigue, constriction, inflammation, etc... The joints are fighting for stability and guarding from injury. I see I have scalenus syndrome, TOS, with a winged scapula from a serious fall. The arm pain is the worst experience, though it's not injured. Just need everything to calm down then stretch. Hydration and Vitamin C is critical for connective tissue. Great video, well explained. Thank goodness for RUclips.

    Thanks Doc! I passed the first test, but failed the 2nd and 3rd miserably. Been struggling with this for 6 or more years now. Will continue the corrective stretches and of course as always, keep working on posture. Door Peck stretches, Chin tucks, and ect.

    I can’t believe I came across this video. I watch you all the time. I have thoracic outlet syndrome. I had part of my first rib removed because of subclavian compression. I feel my nerves are still compressed.

    I have had nothing but problems with my spine since 2006 when it became obvious an old work injury was more than i had thought before.
    I had several disc removed and then years after was diagnosed with cervical spondylosis with multi level problems.
    In all these years never has any Dr or physio mentioned any of this to me and i can say that doing much of what this guy has shown me through a few videos has helped me out some in just a few weeks. Just take it very easy folks.
    I once thought i would never be in the situation i am in now.

    I had a terrible time with this. I was misdiagnosed with bells palsy, possible MS, possible stroke, possible chest wall tumor , possible lyme disease.
    I had woken one day with numbness in my face , it moved up my head , down my arm and I ended up with TMD. After test after test I ended up at my dentist because of a misaligned bite that broke a tooth. He referred me to PT where I began to understand it was muscular issue. I remember when I first started stretching and getting relief I could feel the fibers pop as they relaxed. My SCM was like a steel cable. I worked and worked on stretching and trigger point pressure (mostly at home exercises) and listened to my body and did things that made sense to stretch. It gradually got better and I regained all feeling. I never knew the bundle of nerves were so compressed. I went through pain so bad I felt like someone beat me in the face. The nerve pain was constant burning even up the side of my face.
    Dont give up and listen to your body. I Still have to stretch and massage and keep it at bay. Thus the reason Im watching this video.
    Sometimes the simple things are the answer!

    1.-scalene relive test
    2.-scalene cramp test
    3.-finger flexing test
    Do this three test and can confirm that there in some trigger point or spasm or shorten scalene muscles which press the Brachial plexus and subclavian artery

    Thank you! Understanding this is super helpful. Recently at physio to pre-condition for oestoporoisis overall baseline testing showed I had a 10 % neck side mobility on one side (and not much more on other. Started with 2 older whiplashes that probably never healed since the scalneous muscles sore to push on since old whiplash despite lots of neck stretches. With better understanding will try your three suggestions for a month and see where I get to. Thank you - I agree its worth correcting even if I can "make do" with it as is. Big thanks.

    Where are you located? You’re so knowledgeable. I’ve been bounced around and referred to specialist upon specialist for 15 years. My issues are only getting worse. I sit here sobbing like a little girl feeling helpless and like nobody cares. I seriously doubt anyone will ever be able to help me. I’m in so much pain. Do you do Skype sessions? I’d love your input on how I should proceed and where to seek treatment. I’m at my wits end.

    You need to strengthen the scalene muscles for long term solution. Stretching will make it worse over time, it’s detrimental advice! Watch MSK Neurology RUclips channel

    Unbelievable!! For seven months I'm in pain, spent THOUSANDS on chiropractors and massage therapists with basically no relief... Did the massage myself on the muscles you pointed out and the stretch and I could move my arm forward without the chronical pain in neck! I will continue to do the exercise and massage technique, because that's the first time something actually helps. The pain between shoulder and neck was so bad at one point that I could barely wash my hair...
    Now I'm massaging my neck for a few minutes and I'm pain free... Unreal 🤔

      My pain is basically gone, I only have a flare-up here and there which is in no way comparable to how it was. All in all, the exercises and watchful waiting led to success.

    What exercises would you suggest for this chronic scalene activation? To keep it brief I have multiple chronic subluxations that will not hold after chiropractic adjustment and the pain can become EXCRUCIATING in the occipital, nasal and frontal sinus. I have my first PT appt in 2 weeks and can only go to chiropractor once a week. I’ve seen improvement and know a lot starts in the hips but targeted exercises will immensely help, it can be hard to sort it all out. Thank you for your videos they are often a go to for me when I’m in pain, love and blessings 💖🌈🌟🙌🏻 The ability to self heal is the greatest gift 🎁💆🏻‍♀️

    Just recently I have been watching your videos, they are brilliant, the way you explain the problem and suggest remedies is excellent.
    About a week or two ago I have developed excruciating pain in my right arm and it appears that it is Scalenus Syndrome. I play Badminton twice a week. I have been playing regularly I am just wondering if I should stop playing.

    hi dr can you please advise me ,couple of years ago I was going thru a stressful time .all my life when iam stressed my left large muscle at front of neck would get tight ,this point in my life I had a high level of stress so I ended up having a habit of pulling that big muscle constantly and really stretching it for a period of months ,now it seems as if I have a blocked ear and the muscle is very loose compared to the right hand muscle .ive went to numerous specialists and they have no idea ,what have I done and how do I repair this thanks

    Fantastic videos and presentation Dr. Mandell. Quick question, can scalene nerve entrapment cause symptomatic pain on the non thumb side of the hand? My elbow, lower arm and three fingers on my little finger side are on fire when I touch the scalene trigger points and do the seated holding stretch. 4 weeks of murderous pain so far and weeks before an MRI and osteo. appointment!

    Thanks so much for your simple and effective self diagnostic tools. I put my head through a plate glass window when my skid steer loader was stopped abruptly by a steel ribbon drain while clearing snow last winter. After being diagnosed with a neck strain, 3 months later I was having even more problems, lack of grip in my hands, nervy sparkler shooting pains, swollen left hand, loss of power, etc. Thanks to your informative videos I have successfully diminished a lot of my symptoms. My question to you at this point of recuperation is, "I've found light weights to also be helpful in assisting getting a great stretch and bands to help rhomboid development ... is there a next step that may include strengthening?" Have you done or would you do a demonstration of appropriate strengthening exercises to support optimum back and neck health into the future.

    Those stretches are great for someone who does not have the syndrome like the doctor. For those who have the syndrome, it will irritate and worsen the condition.

    Well after using the wrong keywords to search in google I have located the medical condition that I have . I have been having these symptoms for a about a year now. I thought it was just apart of getting old because Iam active and iam use to being sore from hockey,gym, and toughmudder. But the pain has been getting allot worse and more tight.i have tighteness all over my body and I have what feel like little and big bundles of knots all over. My body most notably on my wrist are the bigger balls but when you massage a point down to a smaller level once it's stretched it feels like little balls of tightness and once I use my fingernail or push on them hard enough tension is released but extremely sharp pain flowed by inlfsmtion and swelling. Are these other symptoms of this disease or is it compounded by this diesease

    I found lying in a supine(face up) position while doing scalene stretches helped almost immediately, more than sitting or standing upright

    Nothing helps mine. My neck has no curve, these scalenes are always aching at least at a 3/10 sometimes radiating to my hand chest and upper back on the right and to a lesser degree the left. 25 yrs of violin playing, with bad neck structure genetically resulting in two non contiguous bad discs (c2/3 and c6/7) and I am always in pain. So discouraging. PT doesn’t help much.

    motivationaldoc, why do the MRI's say the brachial plexus is normal when I obviously feel and know it is not since my accident that torqued my neck? Could they have missed looking at something on a CT scan? What is the best image diagnostic test for this syndrome? Do people have this condition for the rest of their life? What do you think of Botox injection for this condition? Any other ideas or thoughts?

    Looks like I have that problem, along with others. I failed the first two tests. I also have lots of inflammation in lower neck, upper back. I have suffered from chronic upper back pain on and off for the last thirty years--ever since I slipped in grease and fell on my back when I was 16. Can you provide some exercises that will help with upper back/inflammation. I have an exercise ball, but all the exercises I found for it are particular for the lower back.

    For many years I have had KNOTS in the back of my neck above C7 on my left side, with my right hand I can grab my neck from behind, thumb on the right side and fingers on the left and squeeze the knots of tissue and they make noises, kind of muffled crunchy noises and they are not bones they are actual, I dont know what it is, I dont have it on my right side, Im sure its cause of the way Ive slept on my belly for all my life and now cause I have to sleep on an incline cause of GERD and on my left side, I ended up twisting and turning and eventually sleeping on my belly which twists my neck to one side or the other and Im back to square one, I even thought of strapping myself down somehow so I dont move out of position, but not sure how to do that.
    On and off I get tingling or numbness in my fingers, left side usually and I know its cause of this, I really do have to fix my sitting posture when working on the computer, watching TV, and sleeping.
    Can you please tell me what are those neck knots? My father told me that he used to have them as well.

    Thank you for the great videos. Have you any videos on myofacia chronic pain syndrome?
    I have had this syndrome in my neck for 6 years now since a motorbike accident. All these stretches help some what with trigger point massage therapy and also needling...even eaten a small amount of weed switches of the muscle spasms. I also had steriod injections to no avail then developed reactive arthritis. I understand and feel having the perfect posture helps prevents the myofacia setting off.

    I have a question: Can this be caused by trauma? I fell of a jetski and was diagnosed with bulging disc and lost alot of strenght at c5 movements. After 4 weeks of keeping my head out of the forward posture and doing exercises to get the bulge back in place i still feel the same pain in my underarm, bicep, shoulder and the back of my shoulder blades. Can it be that i was misdiagnosed?

    Hi Doctor, I have a serious neck situation. I am an arborist who has been looking straight up while trimming trees for many years. I use a pole clip or pole lopper sometimes for many hours at a time. The stress of looking straight up with my neck "folded" back holding the pole in position with my left arm and pulling down with my right to clip branches has caused sometimes severe pain in my neck and shoulder on my right side. I have also experience pain on the left but mostly on the right.
    I have also noticed a small muscle in the middle of the top of my forearm has completely atrified. This has caused severe weakness in my left forearm when holding something out in front of me. My wrist just drops.
    Any suggestions....please?

    I am a hairstylist, lifter, train horses....so my upper body takes a beating! Thanks for reminding about the scalenes!!! I also get tingling whenever I grip- say even my toothbrush. Would this be due to my scalenes. I also have sternoclavicular dislocation,, my chiro assumed it could have been caused doing burpees...???

    I have had tightness in this area for years. I'm always massaging that trap area. Moreoever, this side is overworked when i exercise (weakness in shoulder and elbow - my arms are even bigger on this side).
    However, i recently suffered an ulnar nerve entrapment (extreme pain from behind clavicle, rhomboid and down arm to pinky numbness).
    Doctor thinks it's a herniation (without even examining me), but i'm convinced it's overuse in this shoulder.
    Any feedback would be appreciated.

    I have a lot of spasm and pain in the front of my neck at the moment. It feels like a vertical pain going down by the sides of my larynx and out to my SCM, both sides. Do you have any advice? I am working on my forward head posture and rounded shoulders (weak rhomboids/ tight pecs and scalenes) I have myofascial pain disorder following horse and car accidents.     In addition to the pain, my son’s pulse become weak in his arm and he can’t keep up with his 3 year old walking. While cooking dinner he became too exhausted to finish.
    He has Factor V Leiden disorder.
    I’m terribly frightened he will have a PE while driving.
    Will these stretches help until he gets in to the right doctor?
    The TOS shuffle is no joke.

    Good video. I have read a lot of stuff online saying that the clamping down of that bundle can cause upper back pain but I don't understand why it would; which nerve(s) would cause pain and problems in the brachial plexus? My doc is alluding to a disc issue but my symptoms are bilateral, i'm a chest breather and I got REALLY bad symptoms when i raise my arms.. As far as I know, raising of arms with disc issues usually results in an improvement of symptoms..
    Which nerve(s) in the bundle is responsible for the upper back pain and dysfunction?

      Hi, most of the nerves in the plexus can send pain or referred pain into the scapular region

      motivationaldoc Thanks doc, you're the man. With TOS I know most people don't need surgery (I've read as high as 90%), how quickly can people relieve their symptoms almost entirely with these stretches and once symptoms are gone, how often should one continue to do these stretches?
      Also, your graphic shows subscapularis muscle. Does the subscap play at all into the TOS equation?

      Forward head posture with the opposing muscles in the upper neck/trap region are common.

      I know what you are saying! For me this is anxiety producing as my medical "professionals" say they want to do surgery and it won't still imo fix a thing!

      Rose Browning Oh yes we are the same - anxiety can cause muscle spasms especially in these places. Even temples and jaw. But dont discount surgery if you have a mechanical problem such as severe nerve impingement. Surgery is not all bad. Sometimes it really is necessary.

      NoStrenghtIn Numbers Oh neck pain is horrible, just horrible. My whole spine is a mess. Pain and radiating pain, numbness and tingling has been part of my life for years now. I have DDD but stress really does affect my neck too, causing spasms.

    i have great weakness in my arms, legs, back. Dr says blood test says no muscle problem. Soft tissue problem. I must see my lupis Dr. could be my hydroxicloraquine

    On another video I believe you said not to do this stretch if you have cervical stenosis. I have it and I believe scalenus syndrome. So can I still do this stretch? It does feel good to massage. My scalenes are exceptionally tender so is my brachial plexus. Now causing me one sided eye strain/discomfort. All discomfort pain is one sided, neck/shoulder.

    Greetings, Rev. T Fey again. I'm finding your videos very interesting and informative-thank you very much! I find that when I stretch out the scalenus muscles on my left side-I get pain in my left scapular region. It's only unilateral on my left side as I was mentioning and in fact, it's primarily in the medial/inferior aspect of that scapular region. Do you have any possible explanation for this scenario? Could it have something to do with the brachial plexus of nerves? I thank you very much Dr. Mandell and respect you! My knowledge model is based on allopathic or osteopathic medicine; I lack quite a bit in the study of chiropractics. I was on my way to becoming a doctor when I became too disabled to finish the schooling (I had leukemia twice, with the second time having full CNS involvement. I had full spinal/cranial radiation and believe that may have had something to do with my spinal issues; not to mention all the bone marrow biopsies and hundreds of lumbar punctures.) But, I found a calling as a minister and continue to study medicine on my own. Just thought I'd share a little about me, if you don't mind.       Can be a lower cervical nerve root irritation or rhomboid/subscapularis adhesions. Might want to consider finding a good massage therapist to lift the scapula up and outwards. Forward head and rounded shoulder posture can cause these muscles from overcompensating.
