This was our last chance to give our 7yo son a little brother or sister. Furthermore it’s been a long long road with 4 rounds of ivf, multiple failures. and then more recently a missed miscarriage. On the plus side - we're using donor egg now. My hcg has been strong and rising well. I had a recent test 5 days ago due to some spotting so I know the indicators are there. But I truly can’t shake the feeling that it’s all going to be ripped away from me on official beta. I’ve had three spots which caused me to panic! Tell me I’m not the only one who feels crazy like this. This path is difficult and emotional. God's blessing to everyone in this boat.

You are the most detailed and open minded Dr that I have seen. I wish I could do my treatment with you if geographially allowed :( My first ivf with 3 embryos transfers failed, even with acupuncture treatment. I will start my next ivf in another new centre after the covid lockdown is over, so literally back to square one. My menses kind of messed up after latest transfer in Feb. My previous ivf Dr asked me to take 3 rounds of 30-day contraceptive pills to adjust it back. I am almost the end of 2nd cycle of contraceptive pills. Based on my observation on previous cycle, my period flow was lesser than usual. I also read that some ppl encountered lesser egg count after treatment with contraceptive pills. I wonder if I should start 3rd cycle contraceptive pills? I couldn't visit any fertility Dr during this period due to lock down. Appreciate your advice
Thank you for such a sweet comment. My heart breaks for everyone trying to conceive always but especially at this time. The pill will lessen period flow. Depending on your egg count, it could also suppress eggs and make your egg # lower after you start stim. You may also require more days of stim because of the pill. If you were my patient and I couldn't start cycles, I would suggest either taking 1 month off of the pill OR staying on it one more month. I'd make the decision based on when I can start cycles again.

For a retroverted uterus, I recommend a bladder that isn't full. I would just have someone not empty prior to IUI if you have a retroverted uterus. I don't have a bromelain brand I recommend. thank you for your great questions!
It can be lower this is why I recommend talking to your doc about treating endo + adenomyosis prior to transfer if needed. Ask your doc if they think you need to.

Question: Is there a difference on the effectiveness of shots or suppositories for progestrone? My friend did shots and she swears by the shots, rather than suppositories b/c she says they're more effective. Any thoughts? great videos by the way! and lots of funny moments too.
I tell my patients: if you skip one pregnancy is done. Meaning the suppositories have to be very well timed and if you forget one, then the shots are definitely better. I monitor and track progesterone levels very closely for my patients. When taking suppositories, I'll check and check again :) then again. And titrate the dose so the progesterone level is in my preferred range. So sometimes, I have patients taking endometrin for example 4x per day: 8 , 12 4 and 8pm with a prometrium oral capsule. There are some people that just can't tolerate the injection so of course I wouldn't have them do the injection. For the most part, I recommend the shot only because people forget a suppository here and there. And that could cause a pregnancy to stop growing. There is zero build up of the suppository in our system. Unlike the progesterone injection, after a certain number of hours, your progesterone level will drop if you forget to insert another one. I know, long answer for a short question, and I hope my answer helps.
@@kdmonte Hey! Hope the transfer went well! Most doctors WILL prescribe shots for fully medicated transfers. It's also the fully medicated transfers, where they suppress ovulation and use meds to replace its effects, that the progesterone is super crucial. With natural and modified natural transfers where you actually ovulate, it's completely possible to go without any progesterone at all because your corpum luteum that you ovulated from should be producing that for you. If you're not producing enough or if your provider wants to be extra safe, you might be given suppositories to help. However, because your body IS making its own progesterone, it shouldn't be a huge deal if you didn't take them on time. Even with fresh transfers, your body should be producing some of its own progesterone. If you're being prescribed suppositories only, I feel like chances are you either did a natural/modified natural transfer or a fresh transfer. Doctors generally know better than to use suppositories only for a medicated transfer. If that's not the case and yours was a medicated transfer using only suppositories for your progesterone, I'd honestly question that doctor and clinic for their competence.

I am so so sorry. My thoughts: 1. If you're still pregnant, request genetic testing of the pregnancy 2. Do an AMH if not done already in case this is related to low egg count/quality 3. Check the shape + anatomy of the uterus 4. make sure you're taking a good prenatal with fish oil+ vitamin D 5. Chromosome analysis for you and your husband is a test I do for my patients 6. Check for thyroid issues/diabetes and treat if they're problem 7. Eat healthy and maintain a healthy BMI and I wish you all the best and the likelihood that your next pregnancy is healthy is very high! But it is always helpful to ask your doctor: why do they think you had a miscarriage and if there's anything they think they can do to help.
Ask your doc based on your history which is better for you. Fresh is usually just as good as frozen however there are situations where frozen IS better. Your doc will know if frozen is better for you.
My most recent FET failed. My era showed I was receptive with 192 hours of progesterone. My doctor for some reason changed my transfer protocol and only did 96 hours of progesterone and he dropped my PIO from 2ml to 1.5ml. My embryo was thawed 24 hours prior to transfer too and it was a fully hatched day 5. I’m so confused and would love some insight.
I am sorry this happened to you :( Here's what i would do if it were me: 1. get your official reports 2. make sure all the info was entered correctly on the ERA worksheet submitted to Igenomix. 3. I'm sorry you had that experience. Typically, I transfer about 3 weeks after period start. You can use hormones to transfer even later. I have patients take 2 weeks of estrogen and then one week of estrogen + progesterone (3 week cycle of meds). Not sure how old you are or how many kids you have already but if you were my patient I would consider doing another ivf cycle now to bank and save more embryos. If doing implantation testing is an option: like the era test, receptivadx tests, I would do them. Make sure you’ve done uterine cavity check recently enough and ask your doc if they have ruled out adenomyosis and endometriosis.
Mam I am 32 years old. I have done laparoscopy and hysteroscopy and have another 2 embryos saved for 2nd transfer if the frst one doesn’t come out successful. I haven’t done ERA testing yet.

Hoping you were successful. Eating healthy, exercising and taking prenatal is the best thing you can do. And ask your doc this very thing. Make sure you’ve taken a careful close look at the cavity first.

Hi Mam, I am Devi 36 years old, conceived and delivered my first child naturally , while we struggled for the second one. It’s 9 years we tried . And we have started IVF and heading for our first transfer on December 11 2020. My embryo grades are 4cb and 4 cc. Can u tell me based on ur experience have embryos with such grades been successful for patients . Hoping that they stick. But in case they don’t, then I wish to go for a second egg retrieval . And if I go for my second egg retrieval will I be getting the same grade of eggs ?
debika jana I’m really sorry. I know how hard you worked to get to transfer. I’m happy to hear your have 5 still frozen! Best thing to do if you haven’t is make sure you have done a saline sonogram to rule out polyps, consider a mock cycle and do the ERA test if your doc offers it. Talk to your doc about what you can do to improve chances. And keep in touch! Sending positive thoughts your way!

Hi Dr Amy. My RE wants to do a natural mock cycle for investigative purposes and a medicated transfer FET. My prior mock was medicated and transfer was successful. I’m concerned about the change. Is this normal?
You’re right to ask more questions about this. I would ask why. You’re right it doesn’t make sense unless they are doing a mock to do the receptivadx test and not the ERA. My gut is that’s what they’re doing? Doing the Receptivadx and not repeating the ERA then doing the transfer the way it was successful with the ERA results you have already had success with .
Egg Whisperer Show from what I understand, he doing the same endo biopsy I did the last time. It wasn’t an era, it was the one you mentioned. He said since I have never done a natural cycle, he wants to see how my body responds. I found it strange that he doesn’t want to repeat exactly what was successful the last time.

I'm not sure if you have access to the Receptivadx.com test. If you don't, I would recommend you: get an HSG done. If you have a hydrosalpinx, consider removing it. If not, consider this protocol: www.ncbi.nlm.nih.gov/pubmed/30328740

I tell my athletic patients to continue to be athletic until your transfer. Then after the transfer, I tell my patients that low impact and low intensity exercise is fine until I see the heartbeat then after that appointment, we talk about increasing intensity if desired. Saying relax at the beach is an analogy :) I want you to lounge around. That's all. Not to literally go to the beach. Coffee isn't harmful for your fertility or pregnancy however it is something that can cause insomnia in pregnancy so that could be one great reason to switch to decaf. Decaf coffee is fine too. Hope your cycle is successful for you.
-You have the best IVF channel in all of RUclipsland, Dr Aimee -I suffered a first trimester loss after my first round of IVF. My doctor checked the natural killer levels and said mine her high. My doctor said IIV intralipid helps with that (I’m not sure how, to be honest), but I didn’t go forward with it because I was scared. Any thoughts On that Dr Aimee. april lawson I don’t focus on the bmi as much as other things like: blood pressure, glucose, cholesterol and your happiness :) if you’re bmi is 35 do what you can to slowwwwwwly lose weight to 30 but if you’re really healthy then I’m less worried about the BMI. So keep track of the things I have mentioned and talk to your doctor about what your goals should be for the very highest chances for pregnancy. Long lupron protocols are great. Issue though with young people is that you can't use lupron as a trigger shot if you experience symptoms of OHSS. HCG is the only option as a trigger shot. I hope your cycle is successful! Sending love on your IVF journey!

Imagine a little chick hatching out it's shell. The top half of the shell is already off and the chick is visible from it's tummy up! That's the equivalent of a "5" of the 5AA :) and the first A = great inner cell mass (which is the part that becomes the baby" and the second A = the cells that become the placenta.

gretchen vidal I am wishing you the best for this next transfer!!! I’m sorry this last one wasn’t successful. As far as travel if you feel like it could then don’t travel. Perhaps stay longer at the clinic and do acupuncture before you leave if they have that as an option. The reality though is that it doesn’t matter. You can feel really confident that you can go home and even if it’s a long drive know that it won’t hurt a thing

I'm going to try my IVF the second time around the first time I had an ectopic pregnancy my question is is it true that it unsafe to bending over for any reasons like pick up something or doing house chores what can do or not and when is it safe to clean or cook any detail will help!!
I just ask patients to avoid heavy lifting, high impact and high intensity exercises after the transfer. Daily activities like usual chores and benign are safe. I'm sorry to hear about your ectopic. I'm sure your docs have done this but consider a HSG before you transfer and if you have a hydrosalpinx, make sure the tube is removed before your next transfer.
Hi dr Aimee, I’m 41 yrs old. Trying to conceive for 1yr. I been taking prenatal pill since October last year. My last period was feb 24. I’m wondering why period stopped suddenly. Eversince I have no problem with my period very normal. My estradiol is 11 and fsh is 50.4. What is that mean? Do u have any advice what should I do? Do u think I still get pregnant because of FSH? cupcakes Thompson I do think Pilates is a great form of exercise but when your ovaries are swollen from Stim meds I recommend listening to your body. If you feel too swollen to do Pilates then skip it.
Hi 😊 I had my egg retrieval and have 3 genetically normal embryos. I did the ERA test on 4/9, results came back pre-receptive, so starting my progesterone on 5/1 and transfer 5/7. My question is should I be concerned that my last period ended two days early, which hasn't happened before to me. Then today 4/29, I spotted. These are both different for me. Could this be because of the meds? I'm currently just doing estradiol shots. Should I be concerned? I really appreciate your insight. Thank you 🥰
I am so happy to hear that you have 3 embryos. This is so great. As far as spotting while on the estrogen injection it could be: 1. early in your cycle and still part of your period (doesn't sound like it) 2. Maybe an ovarian cyst (unlikely because you did the ERA this past cycle and didn't ovulate and probably had a lining check) 3. Uterine polyp or cervical polyp. 4. From sex . If you haven't had a saline sonogram, it may be a good time to do it. If you were my patient I would have you come back in for another lining check and speculum exam to see what's going on then we would decide whether to continue with the transfer or not. As soon as she took it out I was fine .. is it okay to have pain while embryo transfer .? My FET is due next month
Yes it’s okay for it to be uncomfortable. I offer all my patients a valium 30 minutes prior. This should really help. Ask your doctor if they can do the same thing for you too.

As advised by my gynac I started Candid V3 , vaginal tablets for 3 days . Looks better to me today .Will this candidiasis affect my FET ? FET might be on 7th - 8th Jan ( D5 blastocysts grade A) .

hi Leo. Repeat your prolactin fasting after two weeks from the first, make sure it's first thing in the morning and don't touch your nipples/breasts until your next blood draw. If the prolactin is still high, then consider evaluation and treatment. I call PCOS = HOPE syndrome. H = hair O = ovulation P = period E= eating+exercise. Women who have PCOS have to very carefully managed during their IVF and you can have a great IVF cycle with the right approach.
Great tips before the embryo transfer procedure! In addition, here are some more tips on what to do before your transfer day: - Get your folic acid - Stay hydrated - Try warming your lower abdomen before your transfer - Avoid strenuous activities - Try acupuncture Here is what to do and what to avoid after your transfer: - Avoid strenuous physical activity (such as aerobics, running, jogging, cycling on hills). Instead, do light-to-moderate activities, such as walking and cooking. - Eat as if you’re already pregnant. - It is recommended to avoid sexual intercourse post-transfer as it can cause uterine contractions. - Follow the instructions of your doctor regarding the type and dosage of the medication to use in the first two weeks. - Avoid tub baths, swimming pools or beaches to protect yourself from infections. - Drink plenty of fluids to help regulate cell hydration (including endometrial cells). - Explore relaxation techniques, such as yoga, meditation or Tai Chi. - Take progesterone to prepare your uterine lining for pregnancy. - Avoid taking a pregnancy test at home as you may get a false-negative or false-positive in the first two weeks. Before the embryo transfer, consider having the following procedures during your IVF treatment: - Assisted hatching to facilitate the implantation of the embryo(s). - Embryo screening like Preimplantation genetic screening (PGS) to screen embryos for chromosomal abnormalities and reduce the risk of miscarriage.
Hi Dr... I just did my 1st IVF, I’m 28 retrieved 5 eggs only 4 mature all of them were fertilized, two of those had SER disc ( the clinic fertilized them anyway), I had a day 5 blast 3BB we did PGS test and it’s a normal embryo, and the clinic also froze a day 6 cavitating morula... I’m planing my transfer on my next cycle, looking for FET on April 6, what’s the chance for a 3BB to end up in pregnancy ? Should I transfer both embryos ? Even though one is just a morula?
hi Camila. If you want two kids, I would consider doing another cycle now to make more embryos for your future. Otherwise, a day 5, 3BB has an over 50% chance of a good pregnancy. I would have you ask your doc what they think your chances are. In my clinic, the chances would be over 65% that a euploid 3BB ----> healthy pregnancy. If you thaw the day 6 morula and it looks like it's expanding, then transferring is something to consider. I don't freeze day 6 morulas but since I don't know all the details of your case, I would have you ask your doctor the same question. Sending you positive energy.

Jus a follow up on my report on 31 Jan second day of cycle my dr put me on sysfol-active 1 od twice per week and ecosprin-75 1 @ night and lycoRed 1 on morning and progynova 2mg 3 times daily. Now I was requested to come in today 6th feb checked my lining told it’s ok and give progynova 2mg more with increase dose of 4 times per day. Have to visit back on 10 feb for another check. Prerna Singh There are different protocols. One is for a session right before and after the transfer. And another protocol is day before and then a day or two after. I don’t think one is right or wrong but find an acupuncturist that feels like the right fit and go from there. Ask your doctor for referrals because they typically know of the best fertility acupuncturists
Dear doctor, i have one question.I had 2 embryos with ivf and they told me ( before transfer)not to urinate and gave me water. My bladder was so full that it was really hard to controle and just on the operation table i started to have contractions! But the doctor started to transfer embryos despite uterine contractions.Then by putting me on the transfer bed came last contraction and i felt wet! I think the embryos came out by this fluid. parisa D I tell my patients not to worry if they pee on me because I have extra scrubs. I also tell patients they can’t pee their embryos out. You can’t pee, laugh, sneeze, cough or fart them out. And I use liquid to prep for transfer and tell patients they will feel liquid coming out and not to worry because the embryos are like peanuts in a crunchy peanut butter jar.....not going anywhere

Hi Dr.., when I had my 1st embryo transfer, since I am very sensitive to pain or discomfort, my fertility doctor recommended me that they will do this procedure using General Anaesthesia just in case if I am not able to bear through. Finally, She did the embryo transfer without general anaesthesia though I had lot of pain which I beared. The result went finally to biochemical pregnancy. Now I have a 2nd embryo transfer planned soon again with general anaesthesia, but wanted to check if with anaesthesia is it still okay to proceed? Do you have cases where you have to do Embryo Transfer with the help of general anaesthesia? Is there any problem doing so.?
Reshma Lewis I do about 10 transfers a year under general anesthesia (its more like a deep sedation ). for the exact reason you describe: severe pelvic discomfort.

This was our last chance to give our 7yo son a little brother or sister. Furthermore it’s been a long long road with 4 rounds of ivf, multiple failures. and then more recently a missed miscarriage. On the plus side - we're using donor egg now. My hcg has been strong and rising well. I had a recent test 5 days ago due to some spotting so I know the indicators are there. But I truly can’t shake the feeling that it’s all going to be ripped away from me on official beta. I’ve had three spots which caused me to panic! Tell me I’m not the only one who feels crazy like this. This path is difficult and emotional. God's blessing to everyone in this boat.

My first ivf with 3 embryos transfers failed, even with acupuncture treatment. I will start my next ivf in another new centre after the covid lockdown is over, so literally back to square one. My menses kind of messed up after latest transfer in Feb. My previous ivf Dr asked me to take 3 rounds of 30-day contraceptive pills to adjust it back. I am almost the end of 2nd cycle of contraceptive pills. Based on my observation on previous cycle, my period flow was lesser than usual. I also read that some ppl encountered lesser egg count after treatment with contraceptive pills. I wonder if I should start 3rd cycle contraceptive pills? I couldn't visit any fertility Dr during this period due to lock down.

Thank you for such a sweet comment. My heart breaks for everyone trying to conceive always but especially at this time. The pill will lessen period flow. Depending on your egg count, it could also suppress eggs and make your egg # lower after you start stim. You may also require more days of stim because of the pill. If you were my patient and I couldn't start cycles, I would suggest either taking 1 month off of the pill OR staying on it one more month. I'd make the decision based on when I can start cycles again.

For a retroverted uterus, I recommend a bladder that isn't full. I would just have someone not empty prior to IUI if you have a retroverted uterus. I don't have a bromelain brand I recommend. thank you for your great questions!
How is the implantation rate for women with endometriosis (and having healthy, tested embryos)?
It can be lower this is why I recommend talking to your doc about treating endo + adenomyosis prior to transfer if needed. Ask your doc if they think you need to.

Question: Is there a difference on the effectiveness of shots or suppositories for progestrone? My friend did shots and she swears by the shots, rather than suppositories b/c she says they're more effective. Any thoughts? great videos by the way! and lots of funny moments too.
I tell my patients: if you skip one pregnancy is done. Meaning the suppositories have to be very well timed and if you forget one, then the shots are definitely better. I monitor and track progesterone levels very closely for my patients. When taking suppositories, I'll check and check again :) then again. And titrate the dose so the progesterone level is in my preferred range. So sometimes, I have patients taking endometrin for example 4x per day: 8 , 12 4 and 8pm with a prometrium oral capsule. There are some people that just can't tolerate the injection so of course I wouldn't have them do the injection. For the most part, I recommend the shot only because people forget a suppository here and there. And that could cause a pregnancy to stop growing. There is zero build up of the suppository in our system. Unlike the progesterone injection, after a certain number of hours, your progesterone level will drop if you forget to insert another one. I know, long answer for a short question, and I hope my answer helps.
@@kdmonte Hey! Hope the transfer went well! Most doctors WILL prescribe shots for fully medicated transfers. It's also the fully medicated transfers, where they suppress ovulation and use meds to replace its effects, that the progesterone is super crucial. With natural and modified natural transfers where you actually ovulate, it's completely possible to go without any progesterone at all because your corpum luteum that you ovulated from should be producing that for you. If you're not producing enough or if your provider wants to be extra safe, you might be given suppositories to help. However, because your body IS making its own progesterone, it shouldn't be a huge deal if you didn't take them on time. Even with fresh transfers, your body should be producing some of its own progesterone. If you're being prescribed suppositories only, I feel like chances are you either did a natural/modified natural transfer or a fresh transfer. Doctors generally know better than to use suppositories only for a medicated transfer. If that's not the case and yours was a medicated transfer using only suppositories for your progesterone, I'd honestly question that doctor and clinic for their competence.
Is it okay to take just 5 mg of diazepam vs. 10 mg when the patient is under 100 lbs on the day of transfer?
Yes! I dose based on body size too. If heavier then 10 mg, if lighter then 5 mg.

I am so so sorry. My thoughts: 1. If you're still pregnant, request genetic testing of the pregnancy 2. Do an AMH if not done already in case this is related to low egg count/quality 3. Check the shape + anatomy of the uterus 4. make sure you're taking a good prenatal with fish oil+ vitamin D 5. Chromosome analysis for you and your husband is a test I do for my patients 6. Check for thyroid issues/diabetes and treat if they're problem 7. Eat healthy and maintain a healthy BMI and I wish you all the best and the likelihood that your next pregnancy is healthy is very high! But it is always helpful to ask your doctor: why do they think you had a miscarriage and if there's anything they think they can do to help.

Ask your doc based on your history which is better for you. Fresh is usually just as good as frozen however there are situations where frozen IS better. Your doc will know if frozen is better for you.

Before transfer does doctor check progesterone level? My doctor didn’t check is it normal ?
My most recent FET failed. My era showed I was receptive with 192 hours of progesterone. My doctor for some reason changed my transfer protocol and only did 96 hours of progesterone and he dropped my PIO from 2ml to 1.5ml. My embryo was thawed 24 hours prior to transfer too and it was a fully hatched day 5. I’m so confused and would love some insight.
I am sorry this happened to you :( Here's what i would do if it were me: 1. get your official reports
2. make sure all the info was entered correctly on the ERA worksheet submitted to Igenomix. 3. Review the information and then plug everything in here: clinics.myigenomix.com/pet/calculator
3. get it all together and make a post-FET consult to talk to your doc so you can have clarity, closure and a plan forward.

I'm sorry you had that experience. Typically, I transfer about 3 weeks after period start. You can use hormones to transfer even later. I have patients take 2 weeks of estrogen and then one week of estrogen + progesterone (3 week cycle of meds). Not sure how old you are or how many kids you have already but if you were my patient I would consider doing another ivf cycle now to bank and save more embryos. If doing implantation testing is an option: like the era test, receptivadx tests, I would do them. Make sure you’ve done uterine cavity check recently enough and ask your doc if they have ruled out adenomyosis and endometriosis.
Mam I am 32 years old. I have done laparoscopy and hysteroscopy and have another 2 embryos saved for 2nd transfer if the frst one doesn’t come out successful. I haven’t done ERA testing yet.

Hoping you were successful. Eating healthy, exercising and taking prenatal is the best thing you can do. And ask your doc this very thing. Make sure you’ve taken a careful close look at the cavity first.

It's always a good idea to check again and see if your gut is telling you to do so. A saline sonogram is an easy enough test to do.

Best thing to do if you haven’t is make sure you have done a saline sonogram to rule out polyps, consider a mock cycle and do the ERA test if your doc offers it. Hi Dr Amy. My RE wants to do a natural mock cycle for investigative purposes and a medicated transfer FET. My prior mock was medicated and transfer was successful. I’m concerned about the change. Is this normal?
You’re right to ask more questions about this. I would ask why. You’re right it doesn’t make sense unless they are doing a mock to do the receptivadx test and not the ERA. My gut is that’s what they’re doing? Doing the Receptivadx and not repeating the ERA then doing the transfer the way it was successful with the ERA results you have already had success with .

He said since I have never done a natural cycle, he wants to see how my body responds. I found it strange that he doesn’t want to repeat exactly what was successful the last time.

1. I am a slow pace runner 🏃🏻♀️ and running release my stress tremendously. Do you recommend go to back to run after the egg retrieval in those weeks or days before the Embryo transfer? And after the transfer when can I run back again ?

I tell my athletic patients to continue to be athletic until your transfer. Then after the transfer, I tell my patients that low impact and low intensity exercise is fine until I see the heartbeat then after that appointment, we talk about increasing intensity if desired. Saying relax at the beach is an analogy :) I want you to lounge around. That's all. Not to literally go to the beach. Coffee isn't harmful for your fertility or pregnancy however it is something that can cause insomnia in pregnancy so that could be one great reason to switch to decaf. Decaf coffee is fine too. Hope your cycle is successful for you.

april lawson I don’t focus on the bmi as much as other things like: blood pressure, glucose, cholesterol and your happiness :) if you’re bmi is 35 do what you can to slowwwwwwly lose weight to 30 but if you’re really healthy then I’m less worried about the BMI. So keep track of the things I have mentioned and talk to your doctor about what your goals should be for the very highest chances for pregnancy. Long lupron protocols are great. Issue though with young people is that you can't use lupron as a trigger shot if you experience symptoms of OHSS. HCG is the only option as a trigger shot. I hope your cycle is successful! Sending love on your IVF journey!

Imagine a little chick hatching out it's shell. The top half of the shell is already off and the chick is visible from it's tummy up! That's the equivalent of a "5" of the 5AA :) and the first A = great inner cell mass (which is the part that becomes the baby" and the second A = the cells that become the placenta.

gretchen vidal I am wishing you the best for this next transfer!!! I’m sorry this last one wasn’t successful. As far as travel if you feel like it could then don’t travel. Perhaps stay longer at the clinic and do acupuncture before you leave if they have that as an option. The reality though is that it doesn’t matter. You can feel really confident that you can go home and even if it’s a long drive know that it won’t hurt a thing

I just ask patients to avoid heavy lifting, high impact and high intensity exercises after the transfer. Daily activities like usual chores and benign are safe. I'm sorry to hear about your ectopic. I'm sure your docs have done this but consider a HSG before you transfer and if you have a hydrosalpinx, make sure the tube is removed before your next transfer.

cupcakes Thompson I do think Pilates is a great form of exercise but when your ovaries are swollen from Stim meds I recommend listening to your body. If you feel too swollen to do Pilates then skip it.

Hi 😊 I had my egg retrieval and have 3 genetically normal embryos. I did the ERA test on 4/9, results came back pre-receptive, so starting my progesterone on 5/1 and transfer 5/7. My question is should I be concerned that my last period ended two days early, which hasn't happened before to me. Then today 4/29, I spotted. These are both different for me. Could this be because of the meds? I'm currently just doing estradiol shots. Should I be concerned? I really appreciate your insight. Thank you 🥰
I am so happy to hear that you have 3 embryos. This is so great. As far as spotting while on the estrogen injection it could be: 1. early in your cycle and still part of your period (doesn't sound like it) 2. Maybe an ovarian cyst (unlikely because you did the ERA this past cycle and didn't ovulate and probably had a lining check) 3. Uterine polyp or cervical polyp. 4. From sex . If you haven't had a saline sonogram, it may be a good time to do it. If you were my patient I would have you come back in for another lining check and speculum exam to see what's going on then we would decide whether to continue with the transfer or not. Hello Doctor , beautiful video .. my mock embryo transfer was quite painful for me , pain was while passing the catheter through my cervix .. though my gynaecologist did not find it difficult . As soon as she took it out I was fine .. is it okay to have pain while embryo transfer .? My FET is due next month
Yes it’s okay for it to be uncomfortable. I offer all my patients a valium 30 minutes prior. This should really help. Ask your doctor if they can do the same thing for you too.

As advised by my gynac I started Candid V3 , vaginal tablets for 3 days . Looks better to me today .Will this candidiasis affect my FET ? FET might be on 7th - 8th Jan ( D5 blastocysts grade A) .

Question: please if one with pcos has a prolactin level of 30.96, a high AMH ABOVE 50 does that affect the chance for a successful ivf?
hi Leo. Repeat your prolactin fasting after two weeks from the first, make sure it's first thing in the morning and don't touch your nipples/breasts until your next blood draw. If the prolactin is still high, then consider evaluation and treatment. I call PCOS = HOPE syndrome. H = hair O = ovulation P = period E= eating+exercise. Women who have PCOS have to very carefully managed during their IVF and you can have a great IVF cycle with the right approach.

Great tips before the embryo transfer procedure! In addition, here are some more tips on what to do before your transfer day:
- Get your folic acid
- Stay hydrated
- Try warming your lower abdomen before your transfer
- Avoid strenuous activities
- Try acupuncture
Here is what to do and what to avoid after your transfer:
- Avoid strenuous physical activity (such as aerobics, running, jogging, cycling on hills). Instead, do light-to-moderate activities, such as walking and cooking.
- Eat as if you’re already pregnant.
- It is recommended to avoid sexual intercourse post-transfer as it can cause uterine contractions.
- Follow the instructions of your doctor regarding the type and dosage of the medication to use in the first two weeks.
- Avoid tub baths, swimming pools or beaches to protect yourself from infections.
- Drink plenty of fluids to help regulate cell hydration (including endometrial cells).
- Explore relaxation techniques, such as yoga, meditation or Tai Chi.
- Take progesterone to prepare your uterine lining for pregnancy.
- Avoid taking a pregnancy test at home as you may get a false-negative or false-positive in the first two weeks.
Before the embryo transfer, consider having the following procedures during your IVF treatment:
- Assisted hatching to facilitate the implantation of the embryo(s).
- Embryo screening like Preimplantation genetic screening (PGS) to screen embryos for chromosomal abnormalities and reduce the risk of miscarriage.

I just did my 1st IVF, I’m 28 retrieved 5 eggs only 4 mature all of them were fertilized, two of those had SER disc ( the clinic fertilized them anyway), I had a day 5 blast 3BB we did PGS test and it’s a normal embryo, and the clinic also froze a day 6 cavitating morula... I’m planing my transfer on my next cycle, looking for FET on April 6, what’s the chance for a 3BB to end up in pregnancy ? Should I transfer both embryos ? Even though one is just a morula?
hi Camila. If you want two kids, I would consider doing another cycle now to make more embryos for your future. Otherwise, a day 5, 3BB has an over 50% chance of a good pregnancy. I would have you ask your doc what they think your chances are. In my clinic, the chances would be over 65% that a euploid 3BB ----> healthy pregnancy. If you thaw the day 6 morula and it looks like it's expanding, then transferring is something to consider. I don't freeze day 6 morulas but since I don't know all the details of your case, I would have you ask your doctor the same question. Sending you positive energy.

Jus a follow up on my report on 31 Jan second day of cycle my dr put me on sysfol-active 1 od twice per week and ecosprin-75 1 @ night and lycoRed 1 on morning and progynova 2mg 3 times daily. Now I was requested to come in today 6th feb checked my lining told it’s ok and give progynova 2mg more with increase dose of 4 times per day. Have to visit back on 10 feb for another check. What are your thoughts 🙏🏻

When do you recommend accupuncture before and after transfer please ?
Prerna Singh There are different protocols. One is for a session right before and after the transfer. And another protocol is day before and then a day or two after. I don’t think one is right or wrong but find an acupuncturist that feels like the right fit and go from there. Ask your doctor for referrals because they typically know of the best fertility acupuncturists
Dear doctor, i have one question.I had 2 embryos with ivf and they told me ( before transfer)not to urinate and gave me water. My bladder was so full that it was really hard to controle and just on the operation table i started to have contractions! But the doctor started to transfer embryos despite uterine contractions.Then by putting me on the transfer bed came last contraction and i felt wet! I think the embryos came out by this fluid. Do you think that i am wrong? Is correct to transfer embryos with such a full blader and contracting uterus?

parisa D I tell my patients not to worry if they pee on me because I have extra scrubs. I also tell patients they can’t pee their embryos out. You can’t pee, laugh, sneeze, cough or fart them out. And I use liquid to prep for transfer and tell patients they will feel liquid coming out and not to worry because the embryos are like peanuts in a crunchy peanut butter jar.....not going anywhere

Hi Dr.., when I had my 1st embryo transfer, since I am very sensitive to pain or discomfort, my fertility doctor recommended me that they will do this procedure using General Anaesthesia just in case if I am not able to bear through. Finally, She did the embryo transfer without general anaesthesia though I had lot of pain which I beared. The result went finally to biochemical pregnancy. Now I have a 2nd embryo transfer planned soon again with general anaesthesia, but wanted to check if with anaesthesia is it still okay to proceed? Do you have cases where you have to do Embryo Transfer with the help of general anaesthesia? Is there any problem doing so.?
Reshma Lewis I do about 10 transfers a year under general anesthesia (its more like a deep sedation ). for the exact reason you describe: severe pelvic discomfort.
