That is a Roto Ray light that spins while the truck is driving and stop when the truck is parked and is designed to be at the height of a rear view mirror to grab you attention while your driving
I know your comments old but based on how flat the front of the truck is it's most likely a Seagrave. They are common makers of ladders and towers along with American Lafrance.
