I think the key issue here is the 'vague requirements' from client, and then the 'challenging communication' . The answer B) collaboration with stakeholder to establish MVP would be one of the best solution for that key issue, and also from PMP mindset as Andrew mentioned in the video, don't make decision all by yourself, which make answer C) not a good choice comparing with B) in this case.
You just have to remember that MVP is your powerful weapon, that’s why B is better, especially when the requirements are not clear, MVP can help the customer to understand what the end product they want, which is the root cause, also MVP is a tool to gain feedback so you clear 2 birds in 1 stone.

I was so confused because I was so sure it was a quality issue.I had to really understand the question and I think the reason for his answer is if the project manager would have received the status report when they were supposed to, then they would have caught the important aspect of the structure that wasn’t tested earlier. So that’s how the whole issue could have been prevented. He went to the root of the problem.

On question 17th, is it not a bad idea to collaborate with PO and not the team as the team will be doing the work? Can you also please explain why on the 20th question the option C is correct? Should it not be evaluated first before taking or directing any execution?

Hello Sir, for Q.No:14, it was also asked "one of the important aspect WAS NOT TESTED". So why not the Quality Management Plan can be the right answer? He might have used this to check if all aspects were tested or not and take the action accordingly either by asking the QA team to intervene.
The question here shows that stakeholder missed an important information, when he received he noticed that some thing has not been tested. Now they ask- that how the PM could have avoided this situation? If the stakeholder got this info on time, he would have told well on time that this XYZ thing has to be tested. So the actual issue (root cause) here is - uninformed stakeholder- which led to another issue. So to make sure that stakeholder got this info on time, PM should have checked the stakeholder register, which has all the details about which stakeholder should be updated with what project related information. I hope I could explain it well. However, Stakeholder Register is not a tool, it is a document. ;)
