This is the best PT hip adductor testing video on the internet. I have had surgeons test me and this technique he used is the only one that activates my pain! Imagine the FADIR doesn’t provoke the pain. (I am a Doctor of Physical Therapy too). Excellent video.
I have a strange problem. I get almost no pain doing any strength exercise using adductors. But I get pain when trying to ABDUCT and resist a force or just raising my leg against gravity standing on one leg for example or lying on the ground. The pain I get is in the groin area. I also got it through playing football and the pain grows to excruciating levels when playing sports with high explosive breaks and turns etc.
@@JohnGibbons I would love to! But unfortunately many of the orthopedists here in germany take a maximum of 1-2 minutes to assess your problem. The first one told me its probably a muscle strain or something like that without really looking and told me to stop any activity besides going to the gym. He also recommended me a physiologist which I couldnt attend yet. The second orthopedist also looked only a bit at the problem and after I said I have no pain doing the tests she touched around the pubic bone which caused me a very high pain. She also said I should do an MRI to be safe and she even bet that we would find a big oedema at that place and prescribed me ibuprofen for a week to remove the inflammation. After that I should just do easy exercises like going for a jog or stretching the muscles

Can't drop my leg out to the side at all like in the video. Please could you tell me which muscle/tendon it might be? In general. As a guide. I can't find a test video for abduct just the adduct. I know it is hard to know without seeing a patient. The pain is right up at the top where the thigh joins. I already went to 10 physio sessions up until January of this year for other injuries which haven't resolved, this groin injury happened right after the last appointment when I was doing a bridge to activate the hip area. I had injured my hip 2 weeks prior while running with a limp due to plantar issue. I'm finding it hard to get info on abduction. What I found out for me was that the pain occurs whenever I try standing and then abduct/raise my legs sideways WHILE also hunching over a bit using my torso. The hunching over part is really important. That caused me lots of pain near my pubic bone area I guess but its really hard to feel using my hands. I get almost no pain in any other position.
Thank you for this really useful video. One question... When you talk about "pain" - do you mean any pain *at all*, or substantial? After doing heavy squats a week ago, I got very sore. Doing any of these tests would have resulted in pain of about 4 or 5 on a 1-10 scale. A week later, the pain from these tests is a 2 or 3. Does that suggest a tear, or just a strain that is healing normally?

I would get someone like me to assess your hip joint - especially if you are training for a marathon as stress fractures to the neck of femur or the femur itself can cause symptoms within the adductors, you might also need an X-ray or MRI to truly diagnose - regards JG

Thank you so much. I am 60 and have strained my adductor. Which one is it when pain radiates from groin area across inside of leg and to just below the knee on the outside. I basically need to do my own research. Local doctors seem to be going in the wrong direction regarding this injury.
Few years ago I fell off a motorbike nearly went over handle bars but hit the mirror mount with groin docs told me I had completely torn the abductor and particularly torn another muscle and the first docs I seen said I would need surgery but than went to see about surgery they told me it would fix itself don't know how a muscle is meant to reattach itself
Sorry to hear that - I went over the handlebars the other day as ride off road motorbikes - the adductors will repair but not like the way it was before the injury and you will probably be left with a lump and typically doesn't normally cause pain or to much weakness, regards JG
Well done video....very helpful. I have a torn adductor from hockey, confirmed by MRI, that I am rehabbing with a lot of these movements you've shown....trying to build strength back. The tear was fairly bad, pulling some bone from my pelvis as well. They don't believe I'll need surgery at this point, just long rehab. I guess my question is, do you have a good way to test yourself to know whether you'd be safe to start playing sports at a high level again after this kind of injury? I can currently run with mild pain, but can't sprint yet. I'm about 4 weeks post injury. Appreciate any info! Thanks!
I have been dealing with pain on the inside of my knee for over 2.5 years i had a surgery and it did little to help i keep telling them the pain is just on the inside of my knee. I am no Dr so i have gone along with all the treatments that i have gotten to this point but something is not diagnosed correct because i feel the same as i did so i started looking into other things that could be my problem and i think i have a adductor issue of some kind but i dont know who to see about this. .......... What kind of doctor do i need to see if i think i have had a long term issue with a adductor?
Hiya, I did the splits a year ago without stretching first (dumb mistake I know) my inner thigh had a sharp shootig pain, ever since I can no longer pancake stretch, when I could get flat ith no issue at all, it just feels as if it's tearing again, but it's been over a year. 've tried everything, stretching it more, taking long breaks from stretching. I don't know how to fix it
Thanks. I’ve torn mine approximately 5 days ago. Had the MRI to confirm, see the next doc in 2 days. But I just wanted to do the test. I physically can’t get into some of those positions. I couldn’t walk for 2 days when I did it, and it’s still very painful. I thought I did my groin but the scan said the adductor, close enough haha

I’m 75 years old
That can happen and doesnt normally cause to much of issue when healed apart from a lump when it contracts - if the biceps tears (long head) they call it 'Popeye' arm...regards JG

Hi. My pain is in the left where the leg goes into the hip pubic bone. So although it feels like adductor strain it’s more of the joint area. When aggravated by running or cycling the pain radiates out all over the groin and stomach. Think I have a weaker left side. Could do with an osteopath. Would glucosamine & Turmeric help?
Why would a muscle test in a shortened position, if it is torn, not necessarily be painful? Furthermore, do you think it’s safe to assume a tear to a tendon in general will always examine as more significant weakness? How would one ascertain the difference between a tendon tear vs. a simple sprain to the tendon?

I have a 11 year old very athletic boy. The only time he pulls his groin is during football season. So he has done this 3 years in a row regardless of how much he warms up. He is small lean and the fastest one on the team but will probably miss at least half the season again due to his injury. Our school still does 6 inch leg lifts off the ground. Do you think that could be a contributing factor?
A lot of what coaches do at school can potentially cause an issue as their knowledge of corrective exercises are limited - those exercises you mention we did in the military and I was the physical trainer that told the recruits to do them - no need for 11 year olds to do as what is the goal of the exercise apart from to cause a potential injury - regards JG

I seem to have an adductor strain which I acquired by running uphill after continuous strain on it over days. One symptom that I have that no one really mentions is that it hurts when I lift my foot up behind me, bending the knee. It also hurts to lift my leg up in front to walk upstairs. Are these movements still controlled by the adductors? Definitely hurts on the medial side. I currently cannot walk.

So the reason why my knees and groin area has pain when it stretches it could be my long adductor muscle? It cramps often as well when it does cramps it's painful bcuz I couldn't stretch it that much.
Was squatting in gym. Warming up. Felt two pops in my right inner leg. One up too and one just above the inner knee. Been 2 years. Pain is sharp and extreme but very acute. Hurts incredibly when making a soccer ground pass motion, or trying to adduct my foot with resistance.

Quite rare to have surgery for an adductor tear and the most common is the adductor longs - if you still have symptoms after 5 months then I suggest you have someone like me assess your hip joint to rule out any pathology within the joint like a labral tear or an impingement known as a FAI - regards JG
Did a half marathon this year. Been battle pain in adductor but not painful during exercise. However pancake stretch (legs a part moving forward) sharp pins and needles pain in what feels like the gracilis(up high and then close to the knee). Any ideas? This is normally a stretch I could touch my chest to the ground. Ow I can barely press forward.

But i dont feel any impingement and the pain is primarily on the lower adductor muscles...again only when stretching with legs straight and open (hamstrings stretch no pain at all). Thats why im scratching my head..thinking there may be a low level tear. Everything on adductor tendinopathy/itis ive read online seems to point to the fact that i wouldnt be able to adduct under load or do other exercises under load without pain...but i can.
Suffer with groin pain , for example I do everything I possible can for hips and groin to strengthen it but yet I still get pain (firey tightness) play gaa (Irish football) a lot of running turning etc but first few minutes I’m ok then the groin starts to hurt and get worse and worse as I go on , was alot worse before but multiple physio chiropractor etc said different things but I don’t know what to do know….
hey sir, i actually feel no pain on the above test nor in any strength training but if i dont warm up and make a strong pass with the inside in soccer or something that requires adduction and i dont warm up i feel pain and a very strong contraction on my right adductor.The pain is so up that is where the leg is connected with the hip.Also only by touching the muscle in comparison to the other leg it's very stiff.

i am unable to do the butterfly stretch, it is painful, otherwise there is not much pain(1/10). Is it a sign of adductor tear??

Hi John Having Right Groin pain, only when i lift my legs from last 2weeks.
Might be It have started from sudden cross movement in Badminton.
Problem is when I stand or start walking for around 2 mins lifting my right leg is restricted causing inner groin pain and my long adductor muscles tighten up imflamating the pain to thigh getting like muscle pulling type sharp pain, same when I try to fold my knees. It's really hard for me to sit after standing for some time.
Have done adductor stretching exersices but not help me much.
Even I got done my MRI of my hip and spine but doc say both are fine.
Please let me know anything I can do to fix it. Please help.

Hi mate, love your videos. I had a groin tendon adductor snap playing football. It’s been over 5 months now but still doesn’t feel right. I got a scan and have results on an email. Bit of a long shot but could you look at it for me and see what you think? If op is needed or not. Would appreciate your thoughts massively
Quite rare to have surgery for an adductor tear and the most common is the adductor longs - if you still have symptoms after 5 months then I suggest you have someone like me assess your hip joint to rule out any pathology within the joint like a labral tear or an impingement known as a FAI - regards JG

Did a half marathon this year. Been battle pain in adductor but not painful during exercise. However pancake stretch (legs a part moving forward) sharp pins and needles pain in what feels like the gracilis(up high and then close to the knee). Any ideas? This is normally a stretch I could touch my chest to the ground. Ow I can barely press forward.
Hi Jesse - I would get a therapist to assess the hip joint as sounds like a problem there and sounds like an impingement or a labral issue, - regards JG

But i dont feel any impingement and the pain is primarily on the lower adductor muscles...again only when stretching with legs straight and open (hamstrings stretch no pain at all). Thats why im scratching my head..thinking there may be a low level tear. Everything on adductor tendinopathy/itis ive read online seems to point to the fact that i wouldnt be able to adduct under load or do other exercises under load without pain...but i can.
My right leg hip are a mess
Suffer with groin pain , for example I do everything I possible can for hips and groin to strengthen it but yet I still get pain (firey tightness) play gaa (Irish football) a lot of running turning etc but first few minutes I’m ok then the groin starts to hurt and get worse and worse as I go on , was alot worse before but multiple physio chiropractor etc said different things but I don’t know what to do know….
See a local therapist like myself as sure they can help - regards JG
hey sir, i actually feel no pain on the above test nor in any strength training but if i dont warm up and make a strong pass with the inside in soccer or something that requires adduction and i dont warm up i feel pain and a very strong contraction on my right adductor.The pain is so up that is where the leg is connected with the hip.Also only by touching the muscle in comparison to the other leg it's very stiff.
