    My second day on the floor alone (after only 4 days of training in nursing home) one trach patient became decompensated. Transferred to the hospital and later died. Family tried to make an issue with me and the facility . The only thing that saved me from that situation to escalate was that I documented EVERYTHING that was going on after she was admitted on my shift. So the family just left the situation alone. Chart people! Even if it takes you 2 hours after your shift is over!!

    I am returning to LTC/NURSING after a 5 year break of nursing to be with my babies while they were little. My goal is to be a DON now (at the facility I just started at. It's right by my house and paying me well. I'd like to run the place some day 😆)

    Yes the repetitive part is what kills me !!! I get so mad but I appreciate the part when you said charting is patient care. I never really looked at it that way before always more of a nuisance

    I like to chart with a difference from other nurses. I love to stand out. I use words that even a non medical person can understand I also use medical terminologies as needed. I have learnt a lot from you. Btw I am a new grad.

    I learnred the hard way
    When i first started as aCNA
    I forgiot to document.
    The nurse suoervisor
    Yelled at me.I was very embrassed
    Another issue was there was only two wall pcs.
    Which was always being
    Used by other CNAs
    Quite difficult to gain
    Access.To do.any doumentation
    I used paper n pen alot.
    As a new CNA the older workers would hold the pc wall system for fellow co- workers,; their frends.

    Thank you. I've been in my first year of clinicals. During my first charting and I am being ripped apart without instruction. This is how this school rolls. This is the reverse. I'm receiving points off from not knowing until I get my grade. So thank you. The vidio provided a lot of great information. Keep you informed. Thanks again!

    6 years later in this video is much needed today 8/4/2024 brand new PCT and this truly has helped me to understand patient care is part of documentation. Now if I can just get how to document every 2 hours and vital signs every 4 hours working in a hospital setting. Most patients I have had since been in training is 7 patients. On a acute care unit and it ranges from ACHS, stand by assist × 1, 2, or 3, total, fall risk, 1013, 2013, PUREWICK FOLEY'S CATHETER.... The list can go on and on. And what is so amazing the older patience 75 to 90 Plus want to get up and go use the bedside commode or either go to the toilet. Some younger 65 and under they want somebody to clean their behinds.🤯 they don't want to get up and take showers. 🤨

    Thankyou, can you make a video on what vocabulary( words) are more better to charge in progress notes. I’m a MED TECH and I hold b/p meds and give prns and I do let the nurse know but I know they also get busy.

    if your chart is subpoena in a court of law your charting needs to be accurate

    You really hit some really great points in this video. I recently graduated have not taken boards yet but I work as Nurse Tech II/ GPN and they facility I work for has me working under someone else's login and when I said I did not feel comfortable doing they looked at me very crazy but I feel like if I make a mistake im getting this person in trouble or doing things under their license

    favornurseb Sorry i did finish my comment my computer went out. I didnt realize i should have done a 108 clinical hours. I done one 88 hours. My Instructor said that people at facility said i could not come back to do other 20 hours. I left on good turns with the facility. She said that she will get back in touch with me. I am going to keep On pursing this until i get my paper in mail. Ive been a cna before i know about caring for people. I know I'm not on charting right now just need a friend to talk it out. Like charting advise. Response, Pam

    yourfavnurseb, I finish phlebotomy, I had 88 hours in clinical when I should have had 108. I got all my sticks. The leader that's over

    Hello this is a very nice video! I have a question, what would you do if during the endorsement of patients, the outgoing nurse tells you that, " Doctor said that if the Troponin I is less than .1 do not bother calling him/her about it". Would you call and verify this to the doctor or would you take it from the outgoing nurse? Plus, you are working night shift. Thank you very much.
