I dont think Livia would have agreed to go. In her book, Tony was flipping by seeing a therapist. In fact, this is what made her order the hit on Tony. Livia would have treated Melfi the way she treated that nurse who walked away yelling "thats it!"
The costuming: Tony and Melfi in darker colors that go with the room, Melfi in a mature looking (she dresses like a Mad Men character) suit, while Carmella is in a cream colored suit feeling like she has to defend herself against Tony and Melfi

It’s not necessarily plot driven, yet there’s always something happening. It’s a case study in psychology. Primarily dysfunctional families. Dysfunctional, unhappy people. The acting from all three in this scene is particularly powerful.
Carmela is so right but at the same time has trapped herself in a life and marriage with this man, happy materialistically but otherwise is a very unhappy woman
Carmela only thinks Tony's cheating is reprehensible because it affects her. She could care less about the violence and intimidation and murder because she benefits from that.
