Surprised that ppl are hating on this channel in the comments. I enjoy these episodes. They are intricately re-enacted and complemented by expert opinions. As far as rehashing old episodes, well, crash causes need to be stressed from different perspectives depending on the theme of that particular program, and thankfully, commercial airlines don't crash daily so "new material" isn't readily available to analyze. To me, this is an educational channel that both layman and aviators can learn from to make future accidents less and less likely. To that end, I salute the producers!
They do a good job looking at the errors, the improvements and the hard lessons learned. It reminds us that most of the regulations and procedures that keep us safe are written in blood.

I was thinking the same thing, there are only so many aircraft accidents, I don't think there's been a major accident in buffalo in 08', that's my home airport, it's rarely a smooth ride in fair weather.
The crash happened 7 years before I was born and I feel like I remember it sometimes just because of all the memorials and people talking about it. My childhood friend’s mother knew someone who died on the crash.
My elementary school classmates father was on this plane. His name was Jay Strausbaugh. It was so devistating. You have to drive by where this happened every time you go to the Detroit airport. It puts chills down my spine each time.
I lived less than 5 miles from the airport and heard the crash. A mixup in paperwork kept me from being a rookie firefighter on the scene. I was supposed to get my pager the day before, but I ended up getting it 3 days after. I couldn't imagine being a rookie firefighter on this scene. Our job is to save lives, but other than Cecila, there were no lives to save.
FYI Cecelia did eventually talk about the crash in a 2013 documentary titled 'Sole Survivor', a documentary about others who were the sole survivors of fatal plane crashes.

I remember taking off on a Northwest flight out of Det in those same conditions in 1984. Bumpy with lightning but really amazing scenery as we flew between towering thunderstorm peaks. That was one lucky kid.
I live in the Phoenix area and they have a Memorial downtown next to City Hall since many passengers lived here! I actually checked it out last night. R.I.P to everyone that passed away in this tragedy and thanks to Nasa for improving the takeoff checklists!!
Except that weather had nothing to do with this accident. I can't imagine being on the flight deck, in charge of the safety of all those people and knowing, if even for a few seconds that what you did (or didn't do) is about to impact everyone. A horrible moment.

Calling this pilot error doesn't capture the full scope. Any time you have a warning that cries wolf, you have a structural problem with the system. I don't care what it is, if the alarm is important and it is giving type 1 errors, it is a disaster waiting to happen.
Agreed!! This is also a maintenance issue, and we've seen where plenty of airlines don't do proper maintenance. There's so many factors that contribute to the horrific loss of life. Pilot error might have been just a small part of the bigger problem.

I would add that when a warning is being disabled so often that the associated decals for the indicator are smudged or damaged, that means an annoying false alarm is being generated very frequently. Though I personally would not disable such a warning due to the potentially life-and-death nature of what it might be indicating, I understand why some would; the alarm itself might have been very distracting and one would only know what that's like doing the job full-time.

Solid materials being used in transportation equipment means more trauma on potential impact. People complain about how cars are “plastic” nowadays, but that’s a safety feature. It absorbs the shock much, much better.

This is why Biden's comment during the SOTU (State of the Union) address about forcing airlines to give refunds if your flight is cancelled or delayed bothered me. I'd rather be delayed than for a pilot to feel pressure to make up time. :\
The Christmas breakdown of transportation with one specific airline is what he was referring to. People being stuck for days in an airport with no food or drinks, and no support or options given. If no prior alert is given and no refund, many are unable to buy a new ticket on the spot.

The truth is though, commercial flight is so much safer than it used to be and pilots are better trained. I'm going to ask a question because I don't know any better. Why didn't they simply have a paper checklist that gets things crossed off? And you simply can't leave the ground until everything's crossed off? And know it's over simplified it but it seems like things wouldn't be forgotten as often...
there is a paper checklist for all the checklists they need to complete before take off. the problem is that if pilots forget to check one of or some of the checklists (which is what happened in this accident), no matter how many checklists there are it still wouldn't help the situation.
My attitude about plane crashes, is the same about car crashes, house fires, etc. Yes crap happens. But that's no excuse to not put any effort to avoid it at the same time. As well as learn from it, if it dose happen.
I have a suggestion. Why not make pilots push a verify button for each checklist as they are completed before the plane will accelerate? This would make them perhaps be more diligent in performing the crucial checklists!
As a systems Engineer, I can tell you that such a system adds complexity to an already complex system. It's another system that can go wrong and create a bad situation. The solution goes to cockpit resource management. As the flight crew go over these checklists the pilots need to speak the item out and the other pilot speaks to say checked. This is why there are 2 pilots in that cockpit. Just today I was reading that United Airlines was fined a million bucks by the FAA because of a preflight checklist issue.
In spite of the commercial crashes, they are rare. Statistically your safer flying than you are driving to the airport. As a pilot , I feel this is very true

All these crashes make me so sad. The 4 year old now without her Mom not to mention the other families that lost loved ones. I think of what their lives could have been but I think about that with all accidental deaths and some that happened on purpose. I saw so much death working security in the ER at a regional hospital.
Just as was pointed out in previous uploads of this same video, the only thing "Northwest" had to do with this incident was their haste to put their name on the fuselage. This occurred a very short time after the NWA/Republic merger. The aircraft and crew were in all aspects a Republic operation. The two entities had not yet merged in any way other than the finalization of the agreement.
Interesting that you felt it necessary to point this out. You are correct that it was a former RC aircraft and crew, but the merged airline was operating as NW for nearly a year at the time of the tragic incident. Unfortunately, this type of pointing blame lingered on for several years and made a successful merger much more difficult for many of their employees.
There is never a reason to try to Russ to leave for weather or any other reason. A good pilot even if the company wants you too stay on schedule thing like this happen can’t be helped. your on that plane you are responsible for lives and you never cut corners or be so careless to leave. Distractions and delays are all part of the airline business. If you can't handle it, you shouldn't be a pilot. There's a common theme in a lot of these crashes: An older "experienced" pilot with a bad case of the hurry-ups.
How is it possible that aircraft nowadays don't have a cockpit camera, and a couple external fuselage cameras? For the cost of a fcking $50 dashcam? WTF.

So … there are dual/redundant annunciators for the *_stall warning_* but not the *_takeoff configuration_* alarm(s)? Or were *BOTH* connected to the *SAME* circuit breaker? If so, that sort of defeats _fault tolerance_ and _redundancy_ by introducing the dreaded *_single point of failure_* which most engineers strenuously try to avoid.
I have been troubleshooting electrical for 20 some years. You use flow charts for this kind of thing. Not checklists. Like nasa said theres a certain flow in witch you should be doing things.

Yes it was established that it was pilot error, and that there were several contributing factors. But the one contributor - pulling out the P40 circuit breaker. Yes, it is a vital piece because it alerts the pilots if the plane's going to stall or it's not configured right. But the thing is going off when the pilots are on the ground, doing a single engine taxi, and have to throttle up further to do the taxi. Forget the fact that it annoyed the pilot. The thing shouldn't be doing that, right? So I think there's a design flaw or something in that circuit breaker that got overlooked and contributed to the disaster. If they had addressed that, the pilots wouldn't be tripping it all the time, and they would have been warned in time to abort the takeoff.
Sometimes the simplest solutions are the best solutions. All the pilots need is a piece of paper with the checklist items numbered with boxes to check check and areas for signatures of both pilots at bottom when done. Can even have this done electronically on an iPad type device and that checklist needs to received by the tower before takeoff clearance issued. Simple solution's people!!!!
I flown once in and out of Detroit metro, [Wayne county international airport]. And that was back in 1996, when they still only had the old north terminal and 5 runways. Now there's 6 runways with a new south terminal. the north terminal has been demolished and rebuilt. During the time this plane crash happen, the airport only had 4 runways.
Incredible investigation, to follow the trail till you determine 'what happened'. This is truly awful; seems to me the last minute runway switch by the ATC had a big hand in this...just my view.
Get-there-itis is a dangerous and contagious disease affecting pilots and it’s so sad their need to be done with their day. They’d rather risk their lives and everyone else’s lives than be guaranteed to arrive alive, albeit late.
Why can't they take a pen and mark each item as they complete it? It is called checklist. Make a check mark next to each item after you complete it. If they get interrupted, they know where they are up to so they can continue with the next item.

When they switched to computerized checklists, did they still keep the paper checklists readily available in the cockpits? It seems like the technology offers something new that could go wrong. Imagine if the pilots can't get the computerized checklist to work, and don't have a paper backup.
Well, I think the systems are thoroughly designed enough that they’re not worried about that problem. Although I am sure that physical backups exist. It’s just that almost everything in the cockpit is now done through a computer, so just having a checklist is quite a minor task compared to the dozens of major computerized systems on every aircraft
But when they do, the least they could do is include the hundreds of comments from the other uploads that demonstrate how much BS this video really is. A lot of drama and exaggeration. This could easily be presented in a five-minute video.

Back in the 80s and 90s and even early 2000s plane crashes were more common. There would be at least one crash a year during that span. Nowadays there isn’t any in US aviation.

BTW, I believe it's called a "black box" mostly because it contains all the tragic data. It's orange to make it easier to find.

The dude at 11:10 isn't telling the entire truth about the smell. He's refraining from describing the smell of gutted and burned humans. I've seen other videos that described that particular smell.

All these crashes make me so sad. The 4 year old now without her Mom not to mention the other families that lost loved ones. I think of what their lives could have been but I think about that with all accidental deaths and some that happened on purpose. I saw so much death working security in the ER at a regional hospital.

She lost her entire immediate family. After the crash, She was under the care of her uncle and aunt.
Just as was pointed out in previous uploads of this same video, the only thing "Northwest" had to do with this incident was their haste to put their name on the fuselage. This occurred a very short time after the NWA/Republic merger. The aircraft and crew were in all aspects a Republic operation. The two entities had not yet merged in any way other than the finalization of the agreement.
I think the other upload of this video was mislabeled. They had a bunch of those lately.
Interesting that you felt it necessary to point this out. You are correct that it was a former RC aircraft and crew, but the merged airline was operating as NW for nearly a year at the time of the tragic incident. Unfortunately, this type of pointing blame lingered on for several years and made a successful merger much more difficult for many of their employees.
There is never a reason to try to Russ to leave for weather or any other reason. A good pilot even if the company wants you too stay on schedule thing like this happen can’t be helped. your on that plane you are responsible for lives and you never cut corners or be so careless to leave. I would rather be late than not get there at all.

Distractions and delays are all part of the airline business. If you can't handle it, you shouldn't be a pilot. There's a common theme in a lot of these crashes: An older "experienced" pilot with a bad case of the hurry-ups.
How is it possible that aircraft nowadays don't have a cockpit camera, and a couple external fuselage cameras? For the cost of a fcking $50 dashcam? WTF.
A cockpit camera is a horrible idea that they did try to instate. The idea was scraped after vehement protests from the pilots.

So … there are dual/redundant annunciators for the *_stall warning_* but not the *_takeoff configuration_* alarm(s)? Or were *BOTH* connected to the *SAME* circuit breaker? If so, that sort of defeats _fault tolerance_ and _redundancy_ by introducing the dreaded *_single point of failure_* which most engineers strenuously try to avoid.
The engines were at full throttle! The aircraft was traveling closer to 200 mph upon impact!

I have been troubleshooting electrical for 20 some years. You use flow charts for this kind of thing. Not checklists. Like nasa said theres a certain flow in witch you should be doing things.

Yes it was established that it was pilot error, and that there were several contributing factors. But the one contributor - pulling out the P40 circuit breaker. Yes, it is a vital piece because it alerts the pilots if the plane's going to stall or it's not configured right. But the thing is going off when the pilots are on the ground, doing a single engine taxi, and have to throttle up further to do the taxi. Forget the fact that it annoyed the pilot. The thing shouldn't be doing that, right? So I think there's a design flaw or something in that circuit breaker that got overlooked and contributed to the disaster. If they had addressed that, the pilots wouldn't be tripping it all the time, and they would have been warned in time to abort the takeoff.

Sometimes the simplest solutions are the best solutions. All the pilots need is a piece of paper with the checklist items numbered with boxes to check check and areas for signatures of both pilots at bottom when done. Can even have this done electronically on an iPad type device and that checklist needs to received by the tower before takeoff clearance issued. Simple solution's people!!!!

Detroit metro airport is in ROMULUS, MI.
I flown once in and out of Detroit metro, [Wayne county international airport]. And that was back in 1996, when they still only had the old north terminal and 5 runways. Now there's 6 runways with a new south terminal. the north terminal has been demolished and rebuilt. During the time this plane crash happen, the airport only had 4 runways.
Sactly what happened about 20 years later to that 737 taking off from Buenos Aires....not configured for TO and faulty alarm....
Incredible investigation, to follow the trail till you determine 'what happened'.
This is truly awful; seems to me the last minute runway switch by the ATC had a big hand in this...just my view.

Why can't they take a pen and mark each item as they complete it? It is called checklist. Make a check mark next to each item after you complete it. If they get interrupted, they know where they are up to so they can continue with the next item.

When they switched to computerized checklists, did they still keep the paper checklists readily available in the cockpits? It seems like the technology offers something new that could go wrong. Imagine if the pilots can't get the computerized checklist to work, and don't have a paper backup.
Well, I think the systems are thoroughly designed enough that they’re not worried about that problem. Although I am sure that physical backups exist. It’s just that almost everything in the cockpit is now done through a computer, so just having a checklist is quite a minor task compared to the dozens of major computerized systems on every aircraft

My sister introduced me to these episodes in 2013 and I’ve been watching them on and off…I do not like the style of TheFlightChannel as that bores me to death.
Back in the 80s and 90s and even early 2000s plane crashes were more common. There would be at least one crash a year during that span. Nowadays there isn’t any in US aviation.
