I diagnosed myself years ago and yes, had confirmation from a podiatrist. PTT is only getting worse as I age (62). I try so many different shoes and tend to have the best results with wrapping my foot/ankle. Stabilization of the ankle and arch is very important.

What do you wrap your foot with? And do you have pictures of your wrapped foot?

I was born with club foot. I had a complete triple fusion of all mid bones and Achilles lengthened. I walk on the outer edge of my foot and my foot is in supination. I have very high arches as well . This seems strange as all Docs say it’s people with flat feet that get this PTT.
Anyway I have managed for 50 yrs but I get that pain right in the exact area you are showing. When I go to walk forward and the pain is so sharp that I have to halt. In fact the only way to walk is to point my foot outwards and then I can sort of get around.
Went to the podiatrist last week and at first he thought I may have osteoarthritis in the fused bones but he thinks it is the PTT tendon and taped up my leg. To my surprise this is the most pain free I have been for weeks, I was literally crying with pain before 😢
My podiatrist said it might be worth making a rocker insole for my shoe to stop me putting to much pressure on this tendon.
One more thing is that ice baths work but heat doesn’t .
Long term I don’t hold out much hope for this condition because it has been flaring up on and off for quite a few years and is getting more frequent as I get older. .,
Thanks for your for your in-depth info.

I am so sorry that you have these issues. Proper evaluation is required to figure out the root cause of the issue. Typically with osteoarthritis pain could caused by inflammation, scar tissue and inflammation.

What functional exercises do you recommend?

After breaking scar tissue, releasing fascia restrictions and decreasing inflammation you can do proper gait training and correct any foot anomaly (proper evaluation required to correct foot abnormality)

@Dr.Jacobs I was diagnosed with posterior tibial tendonitis a few years ago. I'm flat footed, and I can't stand or walk for long periods of time.

I had a triple sprain and minor fracture in my right ankle. I did surgery 7 months ago, currently have a screw in my ankle also. I did physical therapy wasn’t much help because I still can’t run and lack mobility with pressure. It’s been so long and I’ve seen little progress. Also did an MRI couple days ago and the Doc says I have inflammation posterior tibial tendonitis. I followed the doctor and therapy instructions but I still lack movements, swelling is still there and it pain a lot. What else can I do?

I am sorry that you have posterior tibial tendonitis. Typically posterior tibial tendonitis caused by scar tissue, fascia restrictions, trigger points and inflammation. Exercises and stretches will not break scar tissue or release fascia restrictions and it could cause more inflammation. In order to treat the root cause of your pain you will need to work on superficial/ deep scar tissue, superficial/ deep fascia restrictions, trigger points and inflammation simultaneously. ASTR's patient tool kit has 4 tools to address these issues.
Here is my protocol for posterior tibial tendonitis:
      - Mild Pain Package: pain level 0-3/10
      - Moderate Pain Package: pain level 4-6/10
      - Severe Pain Package: pain level 7-10/10

I have had PTTD for so long. I know I have been on a vicious cycle of inflammation and pain. Have been to PT and I believe they make it worse. After viewing your video, I feel you are spot on, unfortunately not too many will do soft tissue work and insist the only way is to strengthen. This has not served me at all. May I ask what is needed to proceed after soft tissue work and trigger point therapy. My foot is so inflammed, that I cannot even wear shoes. Please any help would be beneficial. At my breaking point with pain.
