      Some context + questions
      I have been stuck with PTTD I believe of 2-3years now. I have had an orthotics for the past one year and pain has reduced a lot. It does hurt when I do small hikes/long walks/play. Questions:
      1) How high should the arch be to support fallen arch? Should it cover 100% of the fallen arch? Or cover 50% and expect the arch to hold the rest 50% to regain strength?
      2) Should I get an MRI to check my Posterior Tibial status?
      3) Would I be dependent on an orthotics for the rest of my life?
      4) Should I try bare foot walking? Would that regain the strength of the arch muscle?

      I appreciate your video. As a pre-teen in Princeton, I began to experience Stage 1. I'm in my mid 40's now and I'm just learning about this after doind my own research. I'm annoyed that no doctor since then has ever addressed, nor expressed concern about my flat feet, nor referred me to any podiatrist. I'm discovering your video after learning what this sharp, temporary pain in on my inner ankle tendon is (Stage 2) and what I can try on my own and what to say to a doctor if/when I do visit one.     I have beaten posterior tibial tendonitis in 9 months!
    Read closely I am writing this because I would have died for someone to tell me this information when this first started when I though there was no way out. Now through all of these steps make sure to take Moringa, turmeric ,ginger and cocoa which are highly recommended for beating the inflammation (make sure to check with your doctor first to make sure these are safe for you to take 1st) and take collagen 1 and 3 preferably from green lakes or native path as well as tendoforte collagen from salt wrap or sparkling wellness which is a little cheaper. Now I was in agonzing pain with both ankles every time I would walk it literally felt like someone was shoving a staple through my tendon so it may depend on the amount of damage done with the time lapse needed on each step but I can only personally share my experience and if I would have done this the right way I probably could have healed a lot faster so here we go. Step 1. Relax your ankle for the first month to two always walk with a brace never unsupported as well as a shoe with a good orthotic always ( orthofeet reccomended) and ice to bring down infllammation I just shoved my foot in an ice bucket but any way is possible to help bring down the inflammation when walking supported gets noticeably easier its time for step 2.Step 2. once the inflammation stage starts to get better this is when stretching, rolling your legs and foot as well as taking longer walks comes into play but still always supported by an ankle brace or something just to keep you arch supported so the tendon can heal but take on a little pressure from the walking. Step 3. this should be at the 3 to 4th month time lapse once you are a professional at walking now comes strengthening a little bit more intensily with calf raises only off the ground I started with 5 the first day 10 the next until I finally made it to one hundred with out a problem from here you should be 65% improved if on any of these steps you do feel pain stop right away and go back to the earlier step you are not ready. Healing for me was a two feet ahead one step back process so be careful keep stretching and monitoring yourself. Step 4. Now when calf raises get easy start to use lotion and give your tendon a deep massage to tear up the tissue I would recommend watching a video on this first then as soon as soon as you are done walking or calf raises need to be done because the whole idea is that you are breaking up the scar tissue with the massage and realigning it with the exercise.Also start to motion your foot in a circle 5 times each way to improve mobility. By now you should be on month six and 85% better now its time for the final step. Step 5. Now at this point you should be more able to walk with out a cast unsupported not is when I started using the theraband I would point my calf down with the band on the ball of my foot and pull it in start with the weakest band start 5 then next day ten and until you can hit twenty then fifty by now you should be at almost 100%. Things to not do do not use n said cream I did this and luckily stopped and found out later it actually weakens the tendon long run. If your foot starts breaking down like all the tendons in my foot did stretch and stick in a bucket of ice every night eventually won't have to but from steps 2 to 4 this was a nightly ritual. Subliminal and mprphic fields may help speed up the process as well I definitely recommend sapien medicine for the fields. Like I said this can be done in a good amount of time if done right.     Very helpful film, with good explanations of the condition. I have it, and have orthotics and exercises from a physio that have helped a lot. But my therapist didn’t really explain the condition, so I am happy to feel better informed now.

    These are great videos! After complaining of a sore ankle and some swelling I was diagnosed with Posterior Tibial Tendinosis after getting an MRI and then was put into a boot. After the boot the PT had me do . After a couple weeks of that the ball of my right foot became painful and one toe began to swell. So, I stopped the exercises. My ankle is still sore and a bit swollen after a couple months of no exercises. Any suggestions?

    Thank you for this wonderful explanation and demonstration! I have pain now in both of my arches. I bought some expensive New Balance shoes to no avail. I even took the insoles out of them because they were so uncomfortable and hurt my arches more. I bought some over the counter Dr. Scholls gel insoles and they are not much better. I am miserable and home bound other than to work and back. I went to the grocery store the other day and only 10 minutes in and I had to leave. I have an appointment with a podiatrist at the end of the month. I hope he can help because this has caused me some tears. If you were any closer(Texas), I’d gladly drive to you. You give the best explanations!

    This is the best video I have found. I am 60 with arthritis my podiatrist has not staged my foot yet just said " your bones are already fused" degenerative tendon diagnosis from orthopedic surgeon. I have been nursing for 30 years.have 5 more to go. The pain is real...

    I'm 27 and already on stage 4 😣 although without arthritis the tendon on the ankle is inflammated but the tendon that's connected to the pinky toe is slightly torn and wearing a customed brace just to stand for 10 min without feeling pain. by the way, I was born with no arch at all that it looks I'm walking on my inner foot and ankle bone with the pinky toe lifted up.

    I have Ehlers-Danlos Syndrom (Hypermobility variant) and this is a recurring problem for me. Sometimes I can get it better for a couple of weeks but it always comes back. I have tried over the counter orthodics, but being female they don't fit into all of my work shoes (professional office) I see running shoe recommendation but not work shoe. Do you have any? Also with all my joints being wobbly (I can and do spontaneously dislocate about a good 1/3 of my joint), is there any hope to getting this stable or am I doomed to basically walk on what feels like a broken bone the rest of my life?

    Hi doc, I don't know if I'm stage 1 or 2, any advice would be brilliant... To look at my feet I have mild heel inversion and healthy arches. However if I relax my right calf, the arch visibly drops to about half its height and you can see a slight deformity with toes pointing outwards, heel eversion etc - this doesn't happen on the left. In normal standing and gait I don't have any weight on the base of my big toe.

    Hey Doc, first thank you so much for the super informative video.
    I've got a question: I'm probably at stage 2, just started physiotherapy and using orthotics. That being said, I'm unable to do calf raises with the affected foot. Will that ever come back? (I feel like I'm like this for years now). I'm surely gonna ask this to my physiotherapist on our next session, I just wanted to get a second opinion.

    Dr, I am unsure if I am stage 3 or 4. My right foot looks like it’s is fully collapsed and my ancle and Tibial bone is misaligned. However I am still able to bring it back to stage 2ish shape. I do heavy loads like 150kg squats and run 5k 2-3 times a week. Have been physically active for a long time. The only thing that is bothering me at the moment when a Achilles’ tendon pain on my right foot as I started to increase my mileage. I am 23 years old.

    8 years ago I got this injury by twisting my foot in a hidden dog hole. I was to young at the time to go to the doctors on my own. My parents never brought me and my father didn't even believe I was in any pain. Said it was in my head.
    My friends would note how swollen the area was and even purple when it was aggravated by walking all day at school and work.

    I don't have flat foot. I broke my ankle in 3 spots and dislocated with syndesmosis ligament rupture 14 months ago. Since then, I've had issues with multiple ligaments in the foot & ankle, with PTT being the current major issue. It's frustrating because while one issue heals up, another becomes present and so on and so forth. I never realized just how debilitating an ankle break/dislocation could be. 😢

    Saw my podiatrist 3 times in two weeks.
    I've been handed more handouts of what it could be and say to come back 6 wks. Like no!!!! But I'm still in pain. Scheduled another appointment. I kept going asking please help with the pain literally overnight debilitating pain. Got steroid injection in heels and a numbing cream. Told to get better stability shoes. Went to fleet feet got standing gait analyzed. Learned i have low arches & over pronate on 1 foot.
    Scheduled 3rd time still in pain gives me official office only sold orthotic and referred to PT.
    I'm literally never told what it is untill I get handed an explanation handout. Tarsal tunnel to plantar fasciitis.
    I'm convinced it's tibialis posterior tendonitis with the synonym described.
    Now i wait for my PT session.

    I believe I’m getting pain at the tibial tendon, not the entire length but only at the bottom, like at the side of the foot above the arch. And this happens only when I do long hikes like 15 mile a day, not on regular walking or running. The soreness lasts a few days after such hikes. Do you think it’s a condition I should check with a doctor?

    i just got an ankle arthroscope and tibial tendon repair and debriment of the tendon area and ankle due to having 4 total surgeries and my range of motion and flexibility not being present as i was doing physicals therapy, i also had 90% of my hardware removed so I'm hope i can get back to some form of strength and normal as i was before breaking my ankle it happened when i was 22

    i had trauma 7 months in knee but when started to press on ankle after 6 weeks i felt it was really weak..PT made extensive gym exercices and kept them for 3 month although i told him i really figured that am standing on the inner half only of my foot
    he knew i had flat foot but not 100% i still have an arch
    i stopped gym exercise and pain better but my foot is more flat and the same pain of a burn still found and increased when the pt made a very high voltage of electic waves on that area
    mri found contusion on talus. and fluid on the posterior tibilias tendon
    my doctor told me to wear air walker for amonth.. will it heal the tendon and thus dereasing the pronation?
    2- the doc said that air wakker prevents pronation nd that will release the contusion..or else going for surgery to release the contusion.. so the solution in air walker??

    I theach les mills bodyattack, in minimal shoe (vivobareefoot) and the next day´s my tendon is destroid... i tried to be progressive but it´s not working very well... and i dont like solid shoes like that because one time my achiles got very inflamated because of the exessive suport.

      That’s true but I’m on the other hand always had pretty good arch. 2 years ago I injured my back and suddenly a year later I can’t hold my arches to it’s normal position. More than 50% fallen of what’s it used to be (comparing with old photos). So yes people with normal aches can also develop it due to injuries in other parts of the body or simply overusing it like crazy runners or extreme sports without any prior conditioning.

    I’ve been scouring the web for info for what is causing my medial ankle pain. No known injury. X-ray is normal. The area that hurts is where a shoe rim comes to. Take a pen and trace a line from mid anterior to just between the 2 bones of medial ankle. I’m retired & am trying to avoid PT due to $.

    I have an Dr. recommended orthotic but still get tendon pain n shin splints. Hyper mobile knees. Arthritic toe. Ankle pain too. Just ordered stretch brace for ankles.

    I was told my foot is falling off my ankle, but I was told I need a hip replacement, & I did get a richie brace, but struggling to find shoes I'm in a men's 11 WW sketchers, they're soft sides. I guess I guess I should do the foot surgery first & see if that helps the hip , . My ligament might be torn, he told me to stand on one foot & then lift the heal & my heal won't come up off the ground & I forget if he said it's over stretched or if it was torn.

    Hmmm maybe I'm on my stage 1 to 2?
    Today I am diagnosed with tendonitis calcaneus talus and my doctor referred me to physiotherapist.
    Tomorrow is my first assessment session with my first physiotherapist.
    I saw the physiotherapist today just to arrange the first appointment and she asked me where exactly I'm having pain, and I showed her using my palm as referencing 3D-model to roughly where's the spot I'm in pain and she told me that medial side of ankle pain is really rare and she continued to ask me whether I sprained my ankle or not but I didn't. I know how a sprained ankle feels but usually it will take like 1 to at most 3 days total subsided and most of the time I over-angled my feet and I never feel sprained LOL (that's how clumsy I am LMAO)
    I decided to see doctor today because even I have changed my shoes to well-known orthotic brand shoes but the pain never subsided and most probably due to my overweight... since I have somehow ditch my 5km/10km public running events when this miserable pandemic started LOL
    Wish me luck
    Oh by the way thanks for suggesting cycling.
    My poor old school bicycle... is collecting tonnes of dust since the pandemic LMAO

    I partially tore my PT tendon. Was in a boot for a few weeks and stayed off it. I can walk with very little discomfort after 4 weeks. Lots of stretching was done throughout that time. Dry needling. After 5 weeks I could do some minor exercising. Frustrating injury. I can’t hop on one foot yet with the injured ankle, way too much pain on that tendon.

    I got into a accident June 2 and it gave me a fractured ankle. The ankle is healed, but I also got in another complication which is posterior tibial tendinitis. I was not aware that this could be posterior tibial tendinitis, so I was beginning to walk and go to the gym and also just go on kind of long walks. I started doing a research to see what could be the problem near my joint, and turns out I have been diagnosed with posterior tibial tendinitis after seeing a foot and ankle speacialist. since I was put into a cast, I lost my range of motion. I’m currently doing physical therapy, and there is this one exercise that I am supposed to do to increase my range of motion in my ankle, but I cannot perform the exercise because of my tendinitis I feel chronic pain when I try to increase my range of motion so until that chronic pain is gone, then that’s when I can get my range of motion back. My question is am I gonna get scar tissue built up since i did not know i had posterior tendonitis? im currently in a ankle brace.

      Not specific to you but when we see a patient with symptoms like that the next question is that is it so due to a chronic repetitive imbalance straining nerves and tendons? Or has something permanently been damages?

      In most people I would find the answer is the imbalance that led to the flat foot in the first place is still occurring, for example still a lot of compensation in the hamstring, hip, lower back and calf muscles leading constant overload of one foot vs another. unfortunately a very impossible diagnosis without a full workup. Hope you find a good doc that can help!

    What do u do about that inside of the ankle drop? I don’t have much pain but I think it’s causing issues in my toes.

    Maybe this is why my ankles spasm at night when I fall asleep and they contract so hard I have to stand up quick and put weight on it, then the joint pops back in place. I have told my primary doc at every visit but she only documents "ankle sprain". Saw a podiatrist three years ago and he just shrugged his shoulders and suggested I take more minerals (??) My ankles are bruised and you can see the broken blood vessels, lots of numbness, burning, jabs, jolts.. I only use tizanidine and tylenol or occasionally motrin and none of them really help. The muscle relaxer will let me sleep for about two hours before the spasms start. I usually cry myself to sleep. Quality of life is seriously being affected now. I'm not sure how to even keep describing this to my primary, she's going to refer me to psych soon!

    I've had PTT for 5 months now. Even tried the DonJoy PTTD brace (I think it made it worse due to fitting). Now I'm "booted" half the day (as physical therapy tech, we put on a lot of mileage). Great video! Now I can gauge my options.

    a stabbing pain at the back to the midway (calf ankle par near the muscle) even touching the floor makes it very painful and I can't move it it's been 4 days and it really hurts a lot, I can't even sleep at night because of the pain and I see redness on it too.     I'm in stage 1 , I got this while doing a leg workout at the gym. on medication for swelling and pain. should I stop hitting the gym or can I continue my workouts

      Hi Krishna, generally at the beginning I would strongly recommend my patients stop aggravating it. This might mean better shoes, insoles or a laceup ankle brace. If the aggravation can be protected to the tendon, it can be ok to continue working out, but if it is still getting worse, then cross training (Swimming, biking, weight lifting etc) might be a better option that does not irritate it until it gets better.

    Could it be tendonitis if you suffered an impact trauma and ever since said trauma you have swelling behind that medial ankle bone? You can’t plant and explode off of that foot without pain surging through the area. The pain doesn’t go down into the arch and the foot has always been flat...

    Hello! Thank you for this video. I went to the podiatrist , here is my dx:
    .
    1.Equinus deformity of both feet
    2. Bilateral pes planus
    3.Posterior tibial tendon dysfunction (PTTD) of both lower extremities
    I'm so miserable with pain. I'm going to have my orthotics made tomorrow. My doctor didn't tell me what stage I have.
    My ankle is extremely sore and painful. My entire feet hurt, and my achilles.
    I would love a second opinion from you.

    I’m a lifelong bike racer. Switched to running while awaiting a new bike. I’m not inexperienced running but definitely increased volume too quickly. Went on another run and have been in pain with collapsing arch ever since. This is my version of hell, not being able to exercise anymore. I hate it. Hate it.

    you added that surgery is rarely needed. After my MRI, my doc is recommending surgery, right now I am in a cast. ?

    Ive been jumping rope for over 1 year. I noticed mild pain in my left ankle. If i decide to continue, will i be risking injury?

    I'm late to seeing this video as I just have been diagnosed with PTT yesterday. My doc didn't tell me a stage, but I believe it to be stage 2 from the info here. My doc wants me to start Diclofenac for two weeks. I noticed you did not mention meds here. Do you think they help in the long term?

    I had sprained ankle almost 2 months ago and is on my 5th week of physical therapy. 2 weeks ago I had posterior tibial tendonitis. There were somedays without pain and at times pain coming back after certain activities. Is it safe to still do some exercises to strengthen my ankle even with pain?

      I am a layman not a Dr but if you have tried numerous shoe types, and orthotics and tried physical therapy and it has a major effect on your life then consider surgery. Be careful there many types of surgery that they can do. Every Dr has surgeries that they prefer to do !
