  • In this video, I describe in detail the difference between bleaching/whitening trays, retainers, nightguards, and TMJ orthotics. ALL OF THESE ARE VERY DIFFERENT IN DESIGN AND SERVE DIFFERENT PURPOSES/FUNCTIONS EVEN THOUGH THEY MAY LOOK SIMILAR. Please watch this video to find out what each appliance is for and the reason why only a TMJ orthotic/splint is recommended for treatment of TMJD.

    I’ve recently been diagnosed with tongue tie and am scheduled to talk to a myofunctional therapist before probably having it released. Previously, I’ve had a lot of nightguards made and can’t keep them in my mouth due to gagging. I’ve broken teeth, then the dental work. How can I learn about the connection between grinding and how treating the tongue tie will solve the problem about destroying teeth and dental work? I’m reading and listening to videos that talk about how tongue tie causes the grinding. I’d really like to hear stories of people who are trying to address breaking teeth and dental work before going through all the treatment.

    Thank you for your excellent content! I have recently developed TMD with a sinus infection. I have noticed that my bite is off. Is this common and will it resolve with treatment?

    Hey, thank you so much for bringing us such useful information! I have been suffering from TMJ related issues for the past 6 months...I have been wearing a splint and I go to adjust it each month. Now I am consulting with other doctors for a second opinion but they all dismiss my symptoms. My question is: are these symptoms a sign that my trigeminal nerve is compressed? I mostly have tingling on my eyelids, cheeks and mandible, a numbness sensation ( I can move my face), muscle spasms on the right side, clicking, popping ( which are more common) and tinnitus...I sometimes feel my entire head gets numb. My doctor does not believe these symptoms are related, I ran a bunch of tests to exclude other causes, and it frustrates me I don't get a proper diagnosis and explanation. Can these symptoms be related to TMD? The way I feel them, I believe they are :)

    thanks for all the information. with the TMJ orthotics, does the same appliance needs to be worn at day as well as night, or the night one is different. also does one need to eat with the appliances, thanks

      I made a daytime orthotic and a nighttime orthotic and I do not ask my patients to eat with them.

    Thank you so much for your valuable information. I have continuous dental pain in the last 2 upper teeth on the left, for the past 5 months. It all started with dental cleaning at the dentist. Since then I changed the fillings, and afterwards did route canal on these teeth. I have seen a periodontist, an orthodontist and several dentists, wear an orthotic and do physical therapy on my mouth and acupuncture. But the pain persists. Dental scans show no inflammation but only wear and tear of my joints because of age. I don’t have apnea and don’t clench my teeth. It’s such a despairing situation - I don’t know what to do anymore 😢 . I wish I could come and see you but I live in Greece. Thank you for all the great job you do for us all.

      @@conniephillipson2424 Yeah, same here. When it comes to teeth, dentists can help. But if it escalates to headaches and lymph nodes and TMJ etc, then you're just screwed. What kind of scans did you do?

    do you recommend a gelb appliance (lower, open in the front, brings jaw forward) for closed lock? That's what my TMJ specialist wants to make me

      It's not about the type of appliance; it's more about the prescription built into it that is unique to each individual.

      I don't recommend any extractions if you can avoid them while you're having TMD pain. Acupuncture i Do recommend.

      It depends on the skill and experience of the TMJ dentist and it can also be dependent on the cost of living in your geographic area.

    Is splint another way to say orthotic? Do they mean and do the same thing? I don't think I heard you call it a splint.... thanks.

      I never use the word splint because it can mean orthotic OR nightguard.

    When I have a flare-up, I feel like I have a headband on. The pain starts at my jaw henge and goes vertically upwards towards my head (like where a headband would sit, vertically with my jaw, but forward from the ears). Is that normal? Also, could having TMD lead to possibly developing Sjogren's later on in life?

      It is common to have the pain that you have. I haven't seen a connection between TMD and Sjogren's but it's possible.

    Hi Priya,
    I hope all is well. I recently got diagnosed with Disc Displacement with Reduction. About 2 months the left side of my jaw starting popping extremely loud every time I open my mouth. I went to my Dentist that said there is nothing he can do and I would have to live with the constant pop for the rest of my life or possibly get surgery. I then went to a TMJ specialist like yourself who said we can possibly recapture the disc with 3 different phases. Phase 1 would be the orthotic, Phase 2 would be braces and phase 3 would be crowns all to change my bite. He then said after all that he still cant guarantee that the disc will be recaptured permanently. I have been wearing the orthotic for 3 weeks and its been helping but my jaw still pops just not as much. My question is can by disc be recaptured and this popping ever stop if I went along with all 3 phases or am I wasting money and time? Thanks!

    The TMJ dentist I'm seeing says my bite causes my jaw to move forward 3mm from central position. He said that is causing my masseter muscles to be over worked and is why they are very large and bulging. So he made an orthotic that positions my bite to the central position. Does that sound like the right approach and something you would do in my case? I'm wondering because you said you try to place the muscles in the most relaxed position but what if the current most relaxed position is actually a dysfunctional position? He's saying it will feel uncomfortable at first because the muscles are used to being in a different position but they will adjust in time. I would love to hear your opinion on it since true experts like you are few and far between, thanks!

    Can TMJ be help with braces? I have uneven placement, I already suffered from split tooth and lost 2 molars. I went to my dentist and said they don't do TMJ treatments.

    How long should I wear the repositioning orthotic/splint? Will I be able to discontinue using this orthotic/device once my symptoms improve?

      I had mild/moderate tmj symptoms and my doctor made a custom repositioning orthotic for my lower teeth. But now I have a gap in between my front teeth. I didn’t have a gap before because I was wearing a night guard. My TMJ symptoms have improved now…Can I discontinue using my lower teeth orthotic and start wearing my night guard for my upper teeth so that the gap in my front teeth doesn’t get worse? Please advice.

    How do you do that ?

      I look at the position of the condyles when the teeth are together and take that into consideration when designing the orthotic.

    What if i buy one of these and then get treated by a dentist who changes the shape of my teeth by making new fillings...do i have to make a new mould each time?

      If you get multiple crowns, then we have to make a new orthotic. If it's fillings, we usually don't have to make a new orthotic.

    My orthodontist is recommending invisalign braces to correct my bite, and therefore give me relief from my TMJ pain. I am hesitant to do this! I had invisalign 15 years ago and have been wearing my retainer ever since. It still fits....so I dont get it. How can my invisalign tray still fit, but my bite is now 'off'?

      A tongue thrust habit can make it so the trays still fit but the bite is off. I don't recommend braces or invisalign as a first line treatment for treating TMD.

    How do I find someone like you in my area (I'm in the US)? My dentist says he specializes in TMJ but the only thing he tells me is that I need a night guard. I have searched online but it seems like every dentist around here also "specializes" in TMJ issues. None of their websites says what they can offer as far as treatment. Who has time to have a million consults to find someone who really knows what they are doing?? It's very frustrating.

    My dentist recommended my nightguard for TMJ! What should I do?! I payed a fortune for that. He is telling me that it’s what I need and there is nothing else to do! But I have severe TMJ 😢
