  • The pain from TMJ disorders can be debilitating! This video is to help patients who are experiencing jaw, head, neck and/or shoulder pain. There are some helpful exercises - some you can do in the mouth and some outside the mouth - as well as other recommendations to help relieve pain.

    These videos are really helpful; I am a chiropractic student and I am learning a way to adjust the cranium and the mandible is a very important bone to balance: the TMJ and sacroiliac joint have the most propioception of all joints. When a patient's hips are off it will effect the entire temporal mandibular joint complex. I hope chiropractors and dentists can work more in the future together to solve TMJ problems.

    I really appreciate U Dr. Priya, I just found U today and really happy about Ur teaching being so organized and comprehensive.🌹 And also really appreciate U bringing the caption which really helps me to get the anatomy terms.👍 I would like to ask a few following up questions related to this video : 1) U mentioned about NOT to open the mouth to the MAX, and suggested only 6x6 times in exercise for lubricating the joint. My question is what if my opening is relative narrow, and I saw several Other RUclips from either oral surgeon or TMJ specialist suggested to do the Ah sound to open the back of the throat which also opened the mouths at the same time as part of my tongue exercise for helping Sleep Apnea, etc. ? So is it common that experts in this field have different to almost opposite views about this? 2) about the side of the Jaw, U mentioned about not to sleep on the sides. But I have sleep apnea, I could not fall asleep in Supine position, and so I got doctors suggested me to sleep on my Sides. I do fall asleep faster on the sides. But as U suggested that affects my Jaw. So What do I do? 3) about self intra-oral massage, for the Lateral Pterygoid, I understand from U and other videos that I need to go to the end of the last molar to the soft tissue area to give some push/ or say expansion or massage, that is my area of confusion after I have tried. I do not know exactly where to massage or push in terms of direction. As I can go to the soft tissue push against the direction of the upper pallete root, or in parallel to the end of the upper pallet. This is the best way I can describe as a layman to dental terms. It would be really helpful if U can help me with the above questions. And thank U in advance❤🙏

      yes, it's common for tmj doctors to have vastly differing opinions. I actually do recommend sleeping on your side, as long as you don't push/put your hand against your jaw. As for the self massage, not everyone can reach the lateral pterygoid on themselves.

    I was scheduled for athrocentesis for my left joint. I started doing massage. For three years I had severe pain 24/7. I could not eat any!! food on that side.. nothing - nor yawn without stabbing pain. I do feel there was some bone degeneration - as I’m post menopausal (but on HRT now).
    Suddenly I can yawn and eat hard!!! food without pain. I can only believe it was the massage and relaxation of the soft tissue.
    I have postponed the surgery. This was a three year ordeal of acute pain and 30 plus years of loud “clicking”.
    I was told there was bone breakdown (mri, CT)
    What I think happened is the massage relieved the soft tissue and the bone reshaped and maybe found a new position for the disk?
    I had to wait 9 months to see the surgeon so I’m kind of own my own here. So simple to find so much relief. I had my 1800 saved up for my copay.

      I am on pause for the joint clean out -arthrocentesis- but with this relief, I’m afraid of messing up a good thing. So moral of my story is massage was life changing. I also felt I could massage jaw gently and it my teeth would align.
      I loved your video. Christine Byer who is an aesthetician on RUclips also had a good video.
      I wanted to post because I do believe the massage and relaxation of the soft tissue has been the foundation for my jaw to find a solution.
      I do it very gently with the massage and don’t do aggressively. I am amazed after 33 years- my pain has out of no where decreased, no clicking, and I can eat normally.

      We treat with orthotics and osteopathic cranial manipulation. I'm sorry but I don't have an at-home solution for this.

      @@theTMJdoc thanks for your reply. As you know, most doctors and dentists are reluctant to treat or even diagnose TMJ. My GP has recommended trying a few alternative health options, inlcuding laser accupuncture, osteopathy or Bowen technique. I'm in South Australia.

    Trying the 6x6 I feel a strong resistance right away (which I don't really do when opening my mouth in the normal position), and my tinnitus goes haywire. I guess that means I've run straight into some very tense jaw muscles. My main problem isn't pain or clicking (only occasional clicking, though crackling is frequent) but precisely tinnitus, which I'm sure is in part jaw/neck related. I clench my jaw during sleep (done many years) and have a mouthguard, and at one point a long time ago my jaw was so tight i couldn't open my mouth enough to eat solid food. Just looking around for exercises/advice, good video.

      You can have 1-2 cups of coffee a day but I wouldn't recommend more than that.

    Thank you for the recommendations and exercises ect…. I just spoke with my doctor about trying physical therapy so hopefully my insurance will cover it so I can try it out because I’ve been suffering nonstop for years and years and it’s just go worse not better and interferes with my daily life along with my severe anxiety, fibromyalgia and other issues. 🙏🤞🤕

    Hey Doc Priya,
    Hope you are doing great,
    I wanted your opinion on this condition called Pterygoid Hamulus Syndrome/Bursitis . I'm medical doctor (just completed my medical school) and I diagnosed myself recently with this condition after unsatisfactory results I have had with couple of doctors. They would go back and forth between differentials(TMJD, Eagle's syndrome, glossopharyngeal neuralgia, myofacial pain etc.) without ruling in the possibility of pterygoid Hamulus Syndrome.
    My left pterygoid process is sharp, prominent and tender. Sometimes I get this sharp stabbing intractable pain which radiate to ear, face and temples and is not relieved by NSAIDS/opiates.
    My left upper third molar tooth has pain and hypersensitivity to liquids which I think is because of this Hamulus syndrome.
    I need your help badly, I've been looking for some competent doctor who can understand my problem and after watching you on RUclips I think you are that person who can help me with this.

      Hi Dr. Addy - I hadn't heard of Pterygoid Hamulus Syndrome/Bursitis specifically. Having said that, many of my patients present with you symptoms and we treat them with custom orthotics and myofascial release with success.

    Hi Doctor,
    Your videos are really helpful. I am facing neck pain issues from last 8months which is now getting resolve but i am facing tingling or you can say very mild pins & needles sensation at left side of my jaw....it goes away by its own but also come back after months..Tmj adjustment is done having no jaw pain. Now getting confuse that what to do for this sensations ?

      @@theTMJdoc Ma’am yesterday morning all of a sudden I woke up with a muscular spasm in the upper trapezius muscle and now I’m in ER they’re doing multiple meds muscle relaxers, Toradol, lidocaine patches , heat packs etc and nothing is helping me. I’m telling them it’s cuz of TMJ but the docs here say TMJ doesn’t bother you here and now they have done a cervical spine MRI

      But I’m screaming out of pain may be cuz I slept with a lot of stress later night, when I woke up I felt terrible.

    Hey doc, I'd like to know if my TMJ disorder is serious or if it's just mild. I've got the clicking noise when I open my mouth but it's when I open my mouth wide and the clicking noise It's very low (other people around me can't hear it, only I can) and for me to be able to hear the noise, I have to put my mouth on a specific way just to occasionally hear the clicking. However, when I fully open my mouth, I feel like a bone right before my ear on the right side of my jaw is moving a bit to the right. I'd like to know if that's normal, or if it's serious. And finally, I don't have any problem fully opening my mouth (except for the slight feeling of the bone), and I don't have any pain while I'm eating/chewing but my right side of the jaw is a bit swollen. I'm applying some ice to reduce the swollen area so hopefully, it'll go away within a few days.
    If someone knows if my TMJ is serious or mild, please let me know. Thanks :)

    I am 19 and I have had this since I was young but just recently got it diagnosed. Extra strength acetominophen and ibuprofen no longer work when my jaw gets so inflamed. I found you through Facebook groups hoping people had some solutions so I am definitely going to be following your account and trying your advice. Thank you so much for these videos! These could help me cope through my last couple weeks of classes because once the pain acts up its over for hours to days. I hope I can sleep better with these tips too. Thank you for all you do 🙏🏻

Its very cold where I live right now and as the winter has set in my jaw has been so inflamed. Usually its both sides but when it's just one side you can literally feel the temperature difference of each sid from the inflammation in my jaw. Sleeping and concentrating when my jaw is aching so bad is nearly impossible and it feels like I want to rip my jaw off. I have oddly shaped ear canals because my jaw is not straight which leads to seriously mean ear infections and aches. I suspect it was all caused because I grew up biting my nails and its a bad habit I havent been able to kick because of anxiety. The jaw clenching is real and I try to keep a relaxed jaw but my muscles in my jaw are so strong now they never relax fully. I sleep lying on my back if I can because the pressure hurts from the sides and the pain sometimes will radiate down into my neck and shoulders. Ive gotten so frustrated because nothing helps and massaging hurts, ive thought about seeing a chiropractor to get advice and maybe other relaxing drugs that are natural like medical marijuana cause I will admit I have tried it before and it helps to relax my jaw and almost makes it feel like is cooling down. TMJ qualifies for a medical marijuana card because it can be chronic but I'm only 19 so I would look more into as a last resort. Ive had other people massage my jaw and neck area with peppermint essential oil and I think it helped take sinus pressure off but I didnt have relief for long. I found Facebook groups about TMJ to see if I could get advice cause thats how desperate I am. It feela like someone is slowly stuffing my head with cotton and the pressure just keeps building.

      Where do you live? The way we would most likely treat you is with custom made orthotics that put your mandible in its optimal position relative to your maxilla, myofascial release, and osteopathic manual jaw manipulation. We usually see pain go down within a month and jaw joint noises and problems go down within 4-6 months.

      @@theTMJdoc Oh my gosh that sounds like a dream. I live in the pacific northwest of the United States. Oregon actually. I was diagnosed by an ENT in my area but never got any help other than a prescription for tylenol which now doesnt work.

    I love what you are doing, so I subscribed. I am a weightlifter that has worn down my teeth from clenching during weights. and am diagnosed with secondary trigeminal neuralgia. I am trying the things you show. I wish your office were closer. Priya, is chewing sugar free gum bad for the jaw?

    This is so helpful. Every time I ask a dentist they’ve just asked if I have a night guard. I have chewed through multiple night guards. I get jaw pain and headaches often. I end up putting a lot of pressure on my joints while sleeping. I do realize that caffeine and sugar starts making my jaw muscles start to clench up and then I start opening my mouth and they start popping. How do we get rid of the popping?

      Hi Erika! I’m so glad you found this helpful. The way we get rid of the popping is by making custom made mouthpieces that align the jaw properly to allow for optimal healing. This is called an orthotic and is very different from a dental nightguard. Along with that, we do myofascial release and jaw manipulation. We see the popping go away within 3-5 months typically.       Hi! Yes - great question. It's usually due to chronic clenching, chronic bruxing, sometimes the way the teeth fit together, history of injury to the head, neck, jaw, spine, or pelvis. There are a lot of factors that can throw the muscles into a dysfunctional state!

    Ive been having ear fullness/clicking for the past few months and I guess was chewing gum one weekend more than usual. Woke up and couldnt open my mouth wide since Sept and never had any issues before. I still do not have full range have jaw pain and now just started to get popping when chewing. A doctor recomended a daytime orthotic but im scared. Will this alter my jaw and repoisiton it permanently? Will this fix my issue help!!

    Hey I need some advise for tmj. I have experience some pain on my jaw and notice I have minor underbite on my teeth. When I opened my mouth i heard some clicking sound. Do I need to go to doctor or dentist or it will heal on it’s own? Thanks for replying.

      The only thing I can think of offhand is don’t rest your face/jaw against your palm. And if you have concerns about sleep apnea, definitely get tested! :). Do you want anything more specific answered?

      @@theTMJdoc i mostly sleep on the left side. My right side is tighter. Is that fine to sleep on the side? I read better not buts its quite hard. 😌 if i lie down on my back i always tend to move my head on the side.

      @@nimrak03 it’s not really too much about sleeping position to be honest. With a good orthotic and the right therapy and a sleep test if needed, the sleeping position shouldn’t matter.

      It’s hard to say without examining you. But yes if your palate is too narrow, it’s very difficult if not impossible to get your tongue up against the roof of your mouth.

      No I do not. The people that come to see me are in severe pain and/or have incredibly unstable jaw joints and we do not ask them to chew gum through this as it can make things worse.

    I am thankful for your efforts. It is helping me. I suffer from Oromandibular Dystonia. My lower drifts horizontally towards left. I take BOTOX injections regularly.     One day i woke up and started yanning ..i was no able to yann , my right jaw was very pain. I was not able to open my mouth. 7 days completed since the jaw pain started.now i am able to open my mouth a little bit not full..
    Now, i dont feel pain when my jaw is at rest.when i am trying to open it hurts.
    I am hoping the pain will go itself without any treatment
    Tell me what to do...

      There is not much you can do on your own unfortunately. Is there a TMJ physical therapist you can see? They are generally less expensive than TMJ dentists..

    Some of the movements from your channel are very helpful, but you lost me at the position of the tongue. It is supposed to be resting at the floor of the mouth, keeping it at the roof of the mouth creates unnecessary tension and is a risk factor for TMD and mewing.
    At least thats what we teach in Scandinavia.

    Have you ever tried the soldier sleeping 🛌 position with no pillow? Excellent for posture

      I don’t agree that teeth should be touching at rest. There are too many variables that can affect the jaw joints and the muscles in an unfavorable way - crossbites, deep bites, etc.
