  • Informed consent for therapy is more than just a form, it’s a whole process. In this video, Dr. Maelisa McCaffrey of QA Prep describes the components of informed consent forms in counseling/psychotherapy private practice.
Maelisa reviews what informed consent for therapy includes, which policies and procedures to consider when starting a counseling private practice, and what other forms you might need in your psychotherapy private practice.
This video is part of the paid course, Documentation Made Easy, which is currently being updated and improved. This lesson is a foundational lesson for the course and you can join the live re-recording by signing up for the new course by the end of July in 2024.

Thank you so much, this was very helpful! I've been in the midst of creating my forms for some weeks and this video confirms I'm on the right track, which makes me feel really good because I've stressed out over a lot of this (not just the forms, but getting the whole practice together). I need to check and possibly tweak a few things, but I believe I'm finally close to being done with all my paperwork. Thanks again Maelisa, several of your videos have been helpful to me in this process. 🙂

This is great! I believe you said you would share a checklist and a link for your Privacy Policy but I do not see either, I may have misunderstood. Thank you! :)

Sorry it's confusing but that was within the course. This is the older version of a module within a course I sell. However, here is the link to a free privacy policy: docs.google.com/document/d/1yMQHSg4jPEupOpRCnlkpIjujl6SCxECam4iXJAVBKS0/edit?usp=sharing
