    Aging IN THE SKIN is the main thing here. Remember that the look of aging is a result of multi factorial age related changes. This is not going to address sagging as a result of fat and bone loss in the face, which is the foundation in which the epidermis sits on. My mother and grandmother have beautiful skin with no wrinkles, but they still look their age because of the bone and fat pad loss. HRT treatments may help slow down bone loss, as well as supplementation with vitamin K2

      It's always been more than just having wrinkles, so you're right about the facial fat volume and bone structure. It's probably the main reason why many people think I'm in my mid 20s at 32, because I have very prominent buccal fat that doesn't seem to shrink much, even with exercise.
      Although, some people are not that good at detecting age, so I also tried using AI, which yielded the same results as people did. So I think it depends on your genes mostly.
      You can also have forehead lines in your teens due to frowning a lot, which is not aged related. Same with nasolabial folds, which you can have from birth.

      I'd call it a natural hormone for normal functioning when at normal levels.
      More is not better, unless you enjoy prostate problems, high blood pressure, low HDLs and high LDLs, increased chances of stroke, DVT and cardiac events, oxidative stress on brain cells, increased DHT/hair loss, increased estradiol/gynecomastia, etc.

      quite an old study for sure! I certainly have a systemic effect (on purpose) when I apply testosterone cream to my thigh or arm. I also use a .3% estriol cream on my face which I know is weak and hopeful that it doesn't have a systemic effect. BTW, once I found the strength, I went to my compounding lab and got them to make it for 1/4 of the price of the big online companies that are marketing it to you.

      @@cindysmallwood2695 I remember hearing that they used testosterone propionate. Propionate doesn’t cause a systemic effect. Btw, his video states how estrogen cream has little effect on the skin. I have to say I saw absolutely no improvement with estrogen cream (although I was at 1%) I stopped using because I found out that it has been giving some women post menopausal vaginal bleeding. I switched to testosterone face cream. So far so good

    Estrogen topically is great also. I am using hormone replacement therapy gel , estradiol, and I put it on my lower body, BUT small amount I mix in my hands with toner and spread on my face. I know you not supposed to spread on your face but I DO. It is said that it increase collagen production by 30%. Is there anyone reading this that is doing the same? 🥰 NOW should I get this testo gel as well if it is more powerful?

      I do the same, but only about once a week, and I like the results. I also take compounded testosterone cream, but I would never put it on my face because I get coarse hair growth in the area where I apply it.

      I do the same thing from a video from Dr Rajani a cosmetic surgeon on YT anyway he had a study he talked about combining estradiol and glycolic acid and yes it was a 30% increase in skin thickness so I put it on a once or twice a week and rotate with retinol on other nights which also increases skin health etc. glad to see others doing this-beauty thru science!

      🙋‍♀️hi, I've got a question. PLEASE, I really need to know ( extreme Chronic Fatigue Syndrome)
      I got those creams, but overlooked the fact they should be used within the 3 month. At the moment, because of my job I'm in the place where HRT isn't being practiced. And I'm here for long.     Many of the aesthetic treatments are targeted to promoting collagen formation. Topical testosterone will absolutely do just that. This is definately worth considering as a part of aesthetic treatment. Many thanks for sharing this video and the study.

    at 17:00: the effects limited to one side show that the treatment worked locally, not that there was no systemic absorption of the hormones (which there surely was). Systemic absorption would not affect such dramatic changes locally because concentrations would be much too low for that.

      Yup, agree with you. Think my choice of wording was bad - I think it's possible for some to make it beyond the skin depending on the skin thickness and other similiar variables. I think even if some made it through, it seems there wasn't enough for any systemic effect though which is interesting.

      Read the paper of the study. He put the link. They said NO systemic. It didn't even travel across the line of where they rubbed the cream- on the same face- so I wouldn't worry about what it would do systemically... They monitored the patients for years. Only the estrogen went systemic- ethinyl estradiol estrogen.

    For those wondering why in this study this topical estrogen was ineffective while all other human and animal studies say the opposite with estradiol, saying that it is effective, it is because this estrogen in this study is ethinylestradiol, which is a synthetic estrogen, and it does not really have the therapeutic effects of the natural estrogen 17beta-estradiol (known as estradiol, the main and most powerful endogenous estrogen) on many tissues, including the skin in humans. Studies have shown that while systemic and topical estradiol is very effective as an anti-aging therapeutic (and prophylactic) treatment for the skin, ethinylestradiol is not. This estrogen (ethinylestradiol) may have prophylactic effects but not therapeutic ones, and the same is true for the skeleton. Other much less powerful natural estrogens such as estriol and 17 alpha-estradiol appear to be just as effective (for estriol) or slightly less effective (for 17alpha-estradiol) than 17beta-estradiol for human and animal skin. The powerful synthetic estrogen formerly used, stilbestrol, also appeared to be very effective. And these estrogens are more effective than testosterone and other androgens because in addition to combating intrinsic aging, they also prevent and reverse extrinsic aging, such as solar elastosis (photoaging), due to their repairing and antioxidant effects in particular. Testosterone mainly fights intrinsic aging. Conjugated equine estrogens such as Premarin, whether topical or systemic, like ethinylestradiol, are also without significant therapeutic efficacy, but may have a moderate prophylactic effect. Progesterone is only slightly to moderately effective as an anti-aging agent (similarly for the weak natural estrogen estrone). For those who want, I can give you some studies on the subject.

      @@lindseycassini1560 It sounds like estrogen did not help, it made skin worse. But it would still be interesting if someone would do a trial on Estradiol, progesterone and estriol. Trouble is, since no one can profit from natural hormones, and almost all studies today are funded by the pharmaceutical industry, it’s very unlikely that they want to spend money on it

      I use mine on my face too. Its supposed to be used vaginally.... it's awesome stuff. At menopause you become less stretchy and flexible. Totally reversed the atrophy so I started it on my face.
      Game changer.
      It's not really pushed because cosmetics can't make big bucks off something they can't patent .
      Estrodiol is already generic so they don't bother. I think emapelle is one. I just get mine from the compounding pharmacy.

    There are a lot of comments here about how horrible the photos are. Photographic and printing technology in 1967 was quite good, and I suspect if you could locate an original copy of the journal at a large university library, the photos would look much better. They look bad here because what is available on the internet today are scans of the original paper copy. When the scans were done, it appears that they used too few bits, so there are only a small number of shades of grey. It's possible that the original photographs were particularly bad, but I think most of the damage was done in the process of converting from paper to an electronic version.

      DHT, a form of testosterone, has an effect of miniturizing hair folicles resulting in hair loss on the scalp, however is also responsible for hair growth on the face and body, the secondary male characteristics. Head hair and body hair are different, so testosterone is actually not good for head hair, but will grow a beard and body hair.

    Can't people understand what the Video shows through biopsy that Testosterone Propionate on the skin only stays in the skin and it has incredible Results that is Proven through Biopsy not someones opinion, So there are no side effects. Play the Video a few times if you can't understand.

      Propionate does NOT only stay in the skin. The amount absorbed into the blood to circulate to untreated skin is orders of magnitude less concentrated, thus no rejuvenation on untreated skin

    I am 62 and have no wrinkles. My parents were wrinkle free until their 80’s and even at death at 88 not much wrinkling. More creasing at that age. Genetics make a huge difference. All of us look mid-teens in our twenties.

      @@riri11- I avoid the mid-day sun, wear a wide brimmed hat, long sleeves mostly, whole foods diet high in animal products (meat, milk, yogurt, eggs) but no retinoids or other creams. Ivory or dove soap. I get a lot of exercise - I farm.

    We produce testosterone it's a building hormone so it makes sense. What I think this study signifies is that aging is related to diminished circulation and function of the body. The body dries out , not because a lack of water and nutrient back a lack of distribution of nutrients . My findings see a problem with sympathetic dysfunction that stimulates stress reaction for loading energy in the muscular body for responsive thrust for survival against a predator. This response is being locked on, reducing organ function and blood flow. Hypertension is a typical symptom of this behaviour . Muscles hold blood pressure against the pull of gravity, which will draw it to the lower regions. We basically calcify and reduce the ability to hold water for hydrolysis needed for regeneration of cells . Whole body regeneration is primary over aesthetic skin presentation.

    21:00 It’s great that it removes wrinkles, but the women also grew facial hair from this treatment, which isn’t exactly a desired effect. They’d probably have to go through laser treatments afterwards to remove all of that unwanted hair.

      @@Dr_Abs So, it’s better to do laser before usage of this product, or after? Will laser prevent hair from growing to that area, even with this treatment? Please record what your patients do, and let us know what is the best solution for this problem according to your studies & observations. I think the fear of growing a beard is what will make a lot of women hesitant to try a product like this, so I hope this issue can be properly addressed.

    Interesting experiment but a shame about the poor quality of the images. I would really like to see a follow up on this with your own patients who are trying the protocol with decent quality, "before and after" pictures for males and females. I want to see if your own trials duplicate these results. Are you taking photos of the patients on this regime? It should be possible to do so in ways that don't reveal their identities. If you come back here in six months with photos that show the improvements you are expecting, without the women having acquired beards and acne, THEN I'll be excited!

    As a man who uses TRT (in the form of cypionate) I can say from experience I have also used propionate in the past, It can be used to but needs to be injected daily. As opposed to twice weekly with my current ester.
    I'm also aware of many men who use topical testosterone cream on a daily or twice daily basis for TRT systemic effect.
    I am very interested in your protocol here and trying it but I do find it hard to imagine how tropical application to the face will not result in any kind of increase in baseline blood testosterone levels when applying the same cream elsewhere in the body would in fact lead to increased serum testosterone.. 🤔

      True, but perhaps the effective dosage for skin rejuvenation could be low enough to have a negligible systemic increase in testosterone?
      You probably know, but some parts of the body absorb much better than others - skin on the shoulders, under the throat and outer thighs absorb better than, say, skin on the stomach or face.

      For testosterone propionate specifically in cream form, the half-life would likely be shorter than injectable forms but would depend heavily on the formulation and concentration. Given there is other ingredients combined with the Propionate, the combinations are likely a key. A general estimate for topical testosterone is that it may stay active for about 8-12 hours, with some residual effects extending up to 24 hours. Given that the formula for the face is just 0.1% mg propionate verses a common 2.5mg cream shows how the test would not only last longer, but have more of an impact.
      The best way to take Prop through Injections is every day to keep the baseline at 100% -- this is typically with 50mg to 100mg each injection (100mg being the most common).

    Could you do a follow up to the Amino Acids 101? I'm getting lots into nutrition and It would be amazing to know where to get the essentials from different foods
    Would love to know more about what the body requires, maybe some insight of some practices used, like why does Bryan Johnson suggest Cacao (chocolate flavonoids) or what other Vitamins/Enzymes help synthesize all forms of collagen, etc.
    No better way to know of it than from the building blocks of chemistry!

      Topical estrogen works. Estrogen is available as gel and it has warning label to not apply that on face, but guess what I have been doing. My female workmates could not believe I am 50. Well I also use antiaging supplements, but this wrinkle reduction effect of estrogel I found already earlier than taking those. Would never apply testogel to face or anywhere else, testosterone is extremely potent in masculinisation as can seen from FTM trangender, irreversible beard growth, balding, voice changes etc is quite rapid after the transmen start using testosterone. I don't think progesterone is even beneficial.

      @@karoliinasalminen Yes, I’ve been applying an estriol cream to my face for some time now but this particular video/study has me questioning that. Like most things, there are those for and those against. I know Dr. Ray Peat believed estrogen to be very problematic and progesterone to be the real anti-ager, so I found this video interesting as it aligns more with his philosophy.

    I need clarification on one point: testosterone cipionate is the long lasting, i.e. with systemic effects (not to be used on skin unless systemic effects are desired). Testosterone propionate is the short lasting that can be used on the skin and has no (or less) systemic effects. Is that correct?

    The beard area can be stimulated by Testosterone as it coverts to DHT. Higher DHT in the beard are promotes facial hair growth. A few concerns is that this must have a systemic effect as Transdermal Testosterone is absorbed. Being propionate means it is available for immediate delivery to the body. Hair growth on the face can be a problem for woman and systemic Testosterone and its affect needs to be considered. Men for example might get baldness on the head in the DHT prone areas. Needs to be carefully considered before going down this path to fully understand the systemic and DHT effects (hair growth).

      @dazbike3247 I committed elsewhere that there has to be some kind of infinitesimal chance that the testosterone is somehow affecting humans. This test was only done on older folks, and a small number of them at that. The study needs to be multiplied, across all ages and see what happens. I mean, we've seen what hormone therapy has done to some people and the damage it's caused.

      I prescribe hormone therapies for a living. The amount of testosterone used in these topical creams for skin is nowhere near the amount needed to induce balding in men. And propionate doesn’t act or absorb quicker, it’s merely broken down quicker.

    Hi Dr., doesn't this study show that only 20% of the subjects had clinically apparent changes in the areas with sun exposure (face)? It looks like there was another study in 1986, "Topical Application of Testosterone and Aging Skin" (1986) by S.L. Riehl and Colleagues that was done in elderly males and it says a significant number showed visible improvements.

Does this study indicate that applying topical testosterone trigger scalp hair growth in men? I'm 63 & still have my hair! lol but its very thin in sections, mainly the crown of my head.     Absolutely incredible stuff and being from the 60s, pretty mind blowing…but I’m quite curious about the statement that topical estrogen on the face showed no improvement and even perhaps worsened things. Yet estrogen is one of the hottest ingredients for topical anti ageing hormonal treatment these days - what’s your thought on that Dr.Abs?
    Does topical estrogen really not have the efficacy and effect that it’s being celebrated for in recent years, even backed by clinical studies?

      @@josh16 Yes- they used ethinyl estradiol in the study- and women are using estriol today for good results. I believe it was the OTHER things they used which made the aging worse- the corticosteroids- because down further in the study paper they mentioned that estrogen was promising... but that that particular estradiol anyway, would cause uterine bleeding in menopausal women - which suits me fine if it reverses menopause! (Unless they meant for another reason- an unhealthy reason for bleeding)

    I'm interested in becoming a patient and trying the T cream. I live in the United States. I would NOT legally be able to take a prescription from a U.K. doctor to a U.S. pharmacy, it would not be valid. Can this be bypassed by having a U.K. doctor as yourself, write the prescription, and I would take it to an online pharmacy in the U.K. and have them mail to a U.S. address? Not sure if that is even allowed. I've never tried it. Are there other options?

      It‘s applied to the face, the DHT issue is when injecting the testosterone. At least that’s what I understood.
      Plus injected testosterone can be an issue on your scalp hair. Other facial and body hair usually increases.

      Using linoeic acid can be used to make the oil produced by skin less sticky(which will prevent acne), maybe this can be added to the testosterone cream in order to negate extra acne

    There are face creams coming out at the moment for post menopausal women, based on estriol as the anti- ageing ingredient. They seem to be helpful. Estriol is considered to have a local effect on the skin it's applied to, and not systemic.
    I wouldn't put testosterone on my face (as I am a menopausal woman) but I would try progesterone in a face product if that's helpful too. What does the progesterone do that's good for the face?

      It does the same as testosterone but with far less obvious r e sults and takes far longer. THAT particular testosterone (testosterone propionate) was safe to be used BY menopausal women and it was on menopausal women that the anti-aging studies were done!

    Really interesting presentation. Are there any papers on Testosterone Propionate (TP) that say there is no conversion to DHT when it is absorbed through the skin? Usually testosterone (no ester) does convert to DHT through the transdermal route. TP may be different.

    A fascinating video!
    I am a bit confused though…. The picture at 21:34, why would a woman want to increase facial hair? Have I misunderstood this?
    Also the reference to the moustache, I can’t work that bit out.
    Was there any improvement in the skin, because, isn’t that what we’re looking for?

      It's a great point. I think there will be an increase in facial hair for both genders and this is something that I talk about during consultation. The study seems to suggest that it would be in the region of something like 10mm over 5 years judging by the pictures but obviously that won't represent everyone and is just one picture. The hair is still something that is gotten rid of by means of laser etc. So if one was to have had laser hair removal already - there's no growth in the follicle to be stimulated; in which case, I think it would be no issue. But this is definitley a talking point during consultations and it's why I showed it in the video so that people were aware of this effect also. The skin biopsies are in the video too as well as before/afters.

      I have just started to watch the video but read your question and as a nurse, wanted to chime in. It may or may not apply to you.
      When in menopause, your estrogen levels drop and likely your testosterone levels as well. In my case, testosterone dropped significantly. Your body/facial hair growth will come to a screeching halt. So, the risk for facial hair growth may not be clinically significant for women in this demographic. As a matter of fact, hair growth has been a non-issue for me. Even with testosterone hormone replacement therapy. Hope that helps answer your question.

I have hit menopause and still have to shave my face every other day but now a lack of estragon makes sex painful .Hormones suck! also I don't have many wrinkles which I guess its a good thing, but to look younger and more attractive but cant put out!!

    Hi @Dr. Abs,
    You mentioned you will try this yourself. If like to request that you monitor your endogenous testosterone levels before, during, and after treatment to confirm this isn't having an effect on systemic testosterone. If like to confirm this treatment is free from unwanted side effects, at least for males.

    In 1967 those photos are terrible. Except for the hair, I can't see a difference due to the lighting. Maybe just a progesterone cream would be beneficial and not having a bunch of hair growth.

    Amazing study! I have used bioidentical hormone therapy for twelve years. A female, I use all three hormones in a schedule which mimics a thirty year old woman. It definitely boosted my weight lifting when I started the hormones at about age 60. And has maintained it since. I do have to deal with excess facial hair but it is worth it. I wonder if using the bioidentical testosterone topically on the face would give the same results as in the study.

      They monitore them for 3 years. Skin has testosterone receptors (womens skin too) so it is good for skin. Just a hardly known fact though...Sodium Bi carbonate made into a paste with distilled water and applied daily to any skin cancers has h-e-a-led them. I saw a video of a woman who h- e -a-led them in this way and my neighbor did it too on one that was on her neck :) Just something to pass along to those who don't know. Spread good information.

    33 male - I am starting topical testosterone today. I use testosterone cypionate for TRT so I have it. I will start keeping a syringe and squirt a bit in my hand, mixing it with my face cream or serums every day. Let me know if you think that there is a better way or if you want updates. My beard is patchy so I will be interested if it will improve that too.

      This regrew previously existing hair, that was lost and not creating new hair that never existed in your life. You‘ll not have more hair than you had in your 20s. This therapy regains lost cell function.

    So, I was thinking. Testosterone propionate 125mg/mL is available where I live. For simplicity, what about mixing 8ml of 125mg/mL Testosterone propionate with 92 grams of squalene? Getting about 1g 1% solution for full face with a dropper should be fairly easy. I have a magnet mixer with heating, but I think it shouldn't be necessary. Thought's anyone? :)

      My pharmacy will only offer the testosterone propionate 1% cream in a Pentravan cream, which I understand is for transdermal administration. As I understand the study when reading it, this is not good as we want only an epidermis effect.
      - My question is, is it safe to use testosterone propionate in a pentravan base?
