I actually did this recently! But you will have to machine down the other safety a little to fit the 1 5/8" width limit in the IDPA CO box. I might make a video showing this specifically in the future.
My Defensive (2x,xxx S/N) had this extraction issue while my 92X Performance Carry Optic (4x,xxx S/N) has been perfect. I replaced the extractor spring, extractor and cleaned the area really well in the Defensive after this match and have had no issues since then. Online people report the same thing on a few of them but seems they can get them working well with a new extractor spring.
