She jumped into the foam pit on their "gladiator arena", and the foam pit was less than a foot deep. So instead of the foam cushioning her fall, she hit the concrete and broke her back in two places. Wasn't even the only injury in the pit. Showing that not only was the pit extremely poorly designed, being way too shallow for the purpose, but also the negligence of the staff for not pulling the plug after the first injury, thus allowing a very serious injury to happen. Hence, Ivan Drago Twitch, an emotionless beast causing destruction.
