Depend karta hai pain kiska hai.
Wysolone se bone , muscle sare kamzor ho jate hai. Uska pain ho sakta hai.
Agar RA ke karan deformities hai to uska pain ho sakta hai.

One of the top rheumatologist in my city started tofacitinib to me in my psoriasis and psoriatic arthritis even after knowing that I have hepatitis b from childhood but in inactive mode.. after one year due to tofacitinib my hepatitis b viral load increased tremendously and now i am on antivirals..Still consuming tofa once a day..

Tofacitinib could induce HBV reactivation in both HBsAg+ and HBsAg−/HBcAb+ patients. HBsAg+ patients receiving tofacitinib have a high incidence rate of HBV reactivation, which could be prevented by antiviral prophylaxis.
Although the risk of reactivation is low in HBsAg−/HBcAb+ patients,
closely monitoring HBV DNA and alanine aminotransferase should be suggested.
Stay in touch with a Hepatologist & Rheumatologist for further treatment and opinion.

My all videos are for awareness purposes.
Few years back, this medicine cost was approx 25000 for a month (30 tablets) due to patent by company.
Many higher arthritis medicines cost 20000-25000 monthly ( specially biological drugs )… Now u can compare it’s cost with others.
Yes, but still very very expensive for me, because of severe arthritis i can't do any work so I've to depend on my family for this medicine. I hope govt should subsidise such costly medicine for patients 🥺

Yes. Koi bhi medicine bina side effects nahi bani.
1. Lekin wo side effects hum 5-10 percent me dekhte hai ( not in 100 percent)
2. Doctor jaante hai ki kya side effects ho sakte hai, so usko manage kia ja sakta hai.
3. Immune system ki bimario ko hamesha ke liye khatam nahi kar sakte, because for aapko aur bohot bimaria ho jayengi.
4.Agar ilaz nahi le to aapko 50 problem aur hoti hai.. jaisa ki maine bataya hai videos me ki gathia fail ke kaha kaha jata hai.
5. Medicine agar regularly doctor ke salah aur regular jaanch kar ke le to 1 percent side effects hote hau.
So choice is urs … regular ilaz doctor ki salah se le with 1 percent risk of side effects ya chod ke apna sharir aur future barbaad karein.

Sir, your video is very informative. I am an AS & IBD patient and presently, on Folitrax 20 mg Injection and feeling better. But now I am having sensation of Nausea & discomfort with Folitrax 20 mg injection. I have moderate neck pain and back pain. Dr. advised Tofacitinib. Whether to continue Folitrax with reducee dose with few more months or can start tofacitinib.
2. What is indication that I am having nausea with folitrax 20 mg injection. Significance. Pls explain
If your disease is not stable or under control ( assessed by your Rheumatologist) then u need tofacitinib for full control.
Folitrax acts partially in AS and IBD. Nausea is the common side effect of methotrexate.
So decision should be taken after evaluation. Thanks

Biological is best if money is not an issue.
If money issue the tofacitinib .
Plus other factors are also taken in to consideration. Like age , any other medical illness or kidney issues , cardiac problems etc

Darkening skin - Hydroxychloroquine.
Gout may be because of many reasons.

For RA (Rheumatoid arthritis) : Neither Saaz nor Tofacitinib is the 1st line therapy or treatment.
You should meet a Rheumatologist. Check degree of your doctor.

Yes can .. MD Medicine. In India , there is no restrictions or laws regarding this. Any doctor can prescribe any medicine. This is harsh truth. But this drug ideally should be prescribed by Rheumatologist or MD Medicine ( physician) with at least some training in Rheumatology. Thanks
Sir....I have AS and other disc issues, knee, hip joints and heel/ankle pain.......after SAAZ and other medicines stopped being effective within 2 months, I had tried two doses of Adalimumab 15 days apart but doctor asked me to discontinue as there was no improvemeny at all and he started Tofacitinib.... initially Tofacitinib helped partially but gradually it's effect diminished totally.....what options do I have now? Should I try Biologics again? And how many doses? And when we take Biologics, do we have to simultaneously take AS medicines also ? Because Biologics + medicines + liver tablets and other supplements are costing a bomb. Please advise sir

If methotrexate toxicity then urgently visit emergency. It’s a life threatening condition.
If intolerance then need to change treatment plan. Meet a Rheumatologist

Ей 6 месяцев назад прописали TOFAJAK 5 mg (по 10 mg в день).
Стало намного лучше,боли ушли чувствует себя хорошо. Параллельно принимает "Метотрексат".
Все инструкции по данному препарату только на индийском.
Хотел бы получить более подробную информацию на русском.
И напишите пожалуйста как долго можно принимать данный препарат. Заранее благодарю.
