It's been a lifelong issue for me, all the way back to my earliest sexual encounters as a teen. I attribute it to being desensitized by circumcision. It's usually faster solo, which can take anywhere from 20min, up to as long as 2 hours, or sometimes not finishing. With a woman, it typically takes at least an hour, rarely less than that, with it sometimes taking all night, (I've gone 5-6 hours several times) or just not happening at all, due to physical exhaustion and/or soreness. I've dealt with this my whole life and I became really frustrated over the last few months as my gf and I like to have sex often and this issue does get in the way of the potential pleasure to be had. One thing I found through some research that has been really helping is...now hear me out...squeezing your butt. Assuming everything else is ok with you, changing your technique might help. We use our hips to thrust, but there might not be active glute engagement which is limiting the overall sensation. Squeezing your butt as you thrust puts pressure on your perineum which is connected to your prostate which is connected to the tip of your 🍆. Squeezing your glutes as you thrust increases sensation in this way. There's additional build up of internal pressure which increases blood flow, which increases sensation, that increases in pleasure, and voila, potentially an easier finish. If you lay down on your back, and squeeze only your butt, you'll see your hips naturally rise. Treat it like a workout, 3 sets of 15-20 at a medium speed and controlled pace and see how you feel. Focus on your glutes and that general area and you should start sensing a physiological response. The biggest cause of the inability to ejaculate is medication. Anti depressants are a huge culprit here along with urology drugs such as Flomax and drugs for the prostate and hair loss such as Finasteride and dutasteride.

Doc, you failed to discuss the importance of assessing chemical reasons. Years ago, a doctor prescribed Zoloft for me. That ended any ejaculations for the next 2 years. Fluoxetine (Prozac/Lovan) is the same. It's even used off label as a treatment for premature ejaculation. Super common in SSRIs.

The biggest cause of the inability to ejaculate is medication. Anti depressants are a huge culprit here along with urology drugs such as Flomax and drugs for the prostate and hair loss such as Finasteride and dutasteride.
Interestingly, you didn't specifically mention a large class of drugs that physicians frequently prescribe that can have a significant effect on ejaculation. A common side effect of SSRI's and SNRI's is delayed ejaculation (or anorgasmia.) While the SSRI may treat psychological/depression issues, long term use can backfire and affect (hopefully temporally) a persons sexual function.

This just started with me. I never had this problem in the past. I noticed over the past few years that my sensitivity is down. Recently, my testosterone levels really dropped. I've been on TRT since 2008 and it has worked great. As I have aged over the years my testosterone levels keep dropping. Erections are normally strong as long as I remain on TRT. I started taking finasteride for a slightly elevated PSA and now have this issue and retrograde ejaculation. Quit the finasteride about a month ago, not back to normal yet.

@@rompinronny Tamsulosin is less selective than Silodosin (Rapaflo). I had zero functions (not just EjD) on latter, but was okay on Tamsulosin. Finasteride is an anti-DHT (not an alpha blocker) and causes some permanent and very serious problems in many men.

Doc, you failed to discuss the importance of assessing chemical reasons. Years ago, a doctor prescribed Zoloft for me. That ended any ejaculations for the next 2 years. Are you on any antidepressants? SSRI, SNRI, or pretty much any medication or supplement boosting serotonin can make it difficult to reach climax.

Had this all my life. I have high-normal T-levels and take no meds. It's less of a problem when I'm in a relationship for a while and more comfortable. Lack of sleep makes it much worse. Adding nipple stimulation will cause me to finish almost every time within a few minutes - so once I figured that out, problem basically solved.

Some might think that delayed ejaculation is a real benefit. But, as someone who has had this in the past, for me, it was not always. Sure, I could last as long as I wanted for my wife but, after a while she was no longer enjoying it and the desensitization caused me to lose my erection, not finish and causing her distress.
Usually after a short rest period, I would regain my erection and finish which was fun but distressing at the same time.
The biggest problem was my previous self-pleasure practice and, though I did not know it at the time, my ED med is designed to delay ejaculation. I have adjusted my solo practice and now that we understand about the med it makes our time together more fulfilling. If it looks like it make take too long, she knows exactly what to do for me to release and, I think she enjoys having that power.

It is frustrating when this happens, but I’m guessing in my case it’s aging. Happens most often though if I’ve already done it that day (myself or with partner). Went through a period where I would ejaculate with no orgasm at all and that was incredibly frustrating. Timing is still sometimes off (ejaculation will occur before orgasm), but by and large this kind of thing can be corrected.
