I love the insight from a derm who has rosacea. I’ve grown so tired of seeing deems try to explain it and instruct on it without having the condition themselves. It’s always preaching the same stuff: use cetaphil, never go near retinols, stay out of the sun, stop drinking coffee and alcohol. It’s all very repetitive like reading a script of what they’ve read in literature. In reality, you can have sensitive skin and rosacea and still need something more cleansing than cetaphil, you may be able to tolerate and even benefit from retinol usage, and can moderate triggers rather than cut out entirely. Thank you for putting this out there because speaking on a derm condition you HAVE is very reassuring and comforting to those who have it and look to you for real life advice/tips.

So true! My rosacea improved and was banished by proper use of tretinoin. It was so hard to find a derm that would finally agree to prescribe it. I had a feeling that it would work because otc retinoids helped and didn’t seem to irritate my skin (ie most info says to stay away from retinoids). In my case tretinoin is my #1 treatment.

wearing sunscreen indoors was a huge game changer for me. i work from home and my home has huge windows getting a lot of natural light throughout the day. and i noticed that if i weren’t wearing sunscreen, my skin would get red and hot even if i stayed at home all day. then dr dray informed me about uv rays from the sun penetrating the windows and basically being in all visible light so… yes, you need to wear sunscreen indoors unless you have light blocking curtains in every room of your home

Thank you for sharing this information and also mentioning skin of color. I’m African American. I was 37 years old before a great dermatologist diagnosed me with rosacea. It presents differently on my Carmel brown skin tone than fairer skin. Game changer. I also grew up being told to wash my face twice a day. I was embarrassed to tell people I didn’t wash my face in the morning. It hurt! Now, I use Bioderma Micellar water in the mornings, and Bioderma Oil Cleanser at night. I live in Arizona. The desert has been frying my face. I diligently use and reapply sunscreen to no avail. Thank you for suggesting making life changes. I’ve decided that from 12pm-sundown, I’m not going outside in this desert heat. My skin can’t handle it.

    • @Jennifer-gr7hn
      @Jennifer-gr7hn 9 месяцев назад +1

I always wondered if the desert was helpful. I'm in NJ, and it's very wet, a lot of mold and when I started working in a moldy environment and then found it at the gym and in my home. What are your thoughts of castor oil? My mom who is Sicilian (I'm half Sicilian, half Italian) and used olive oil and even at 77 her face and overall skin is smooth like baby's butt :) I take after dad's fairer skin with freckles, but he doesn't have a bump or issue at all). For me, it was mold - with out a doubt. I highly disagree with the 'sunscreen' thing. Didn't grow up with it, and for good reason....it's toxic, AND it prevents what we need to activate vitamin D, the parts from the sun we can't get from a pill.

I'd like to chime in and say an often overlooked trigger for rosacea is hair products. These products get on our skin and all over our pillows. If you're handling your other triggers but still flushing for unknown reasons, try using fragrance free hair care for a while and avoid aerosol hairsprays to see if there's an improvement.

  • @DebiJo

Gosh! I have your exact problems with rosacea. I’m 67 and have always used SOMETHING to keep it quieted. There was a period of time where I developed some, well, CYSTS! Not the kind you would have drained or have to remove, but these bumps about the size of a nickel and very firm, always painful! One was to the left of my nose and just under my eyebrow. It actually HUNG THERE. My derm doc did the sulfur route and also helped me get a “special needs” scrub cap because the fibers from the caps falling on my face was causing this! Ugh. So that cleared up and now, I have every symptom you have. Avoiding the triggers is #1. I use Prosacea after CeraVe hydrating wash, which is nice but it doesn’t make me feel fresh and clean. Dermatologist recommended it and I’ve been at it for 2 years. I would like to firstly, find a cleanser that refreshes my skin and plumps a little but protects my barrier. The very same goes for my night cream. CeraVe again. I feel like it’s weighty and gummy and sticky, very mildly so but, as you said, you want to feel comfortable skin and I would like freshness with those two. What REALLY makes me feel skin happy is the CeraVe Vitamin C. Loved it from day one and actually look forward to it. I stopped their Retenol. I tend to get milia and it got worse with retinol. I’m looking into the azeliac acid. Trying it next.
Thank you so much for going in depth with YOUR experiences. It was uplifting and I learned so much from hearing your approach to your own care. I had blepharoplasty for invasive 😊 hooded eyelids. There was an additional laser treatment done just above my cheeks and after it healed, I really didn’t notice much difference. Okay. I just really loved this video and your channel. I am like you in so many ways, you could be my daughter! When I was out and about it was ALWAYS 1n nature. I still have horses, ride my old lady 26 speed bicycle (with the basket option), go mud riding with a Polaris SXS, swim, play golf, and sit by the fire and sing 60s & 70s rock with other old hippies. My habits have drastically changed, as yours have, to keep my face QUIET. But let’s got give up nature & MINERAL sunscreen! BLESSINGS, Dr. Sam, AND BE SAFE ❤

Thanks for the video, Dr. Ellis! Unfortunately Rosacea is not studied as in depth as other skin conditions (Derm rep here). Nonetheless it can and does significantly impact the quality of life for those that suffer from it. I developed rosacea a month before I turned 38. No family history. I was flushing all day and all night for well over a year. I was unable to sleep and basically participate in life. I was even evaluated for carcinoid tumors (also due to some test results). About a year and a half into this a Derm where I live put me on Hydroxychloriquine to drive down some inflammation. It took a few months, but has has take the edge off for the most part. I do avoid triggers, such as alcohol and direct sunlight. I do have natural light sensitivity as well. Things are way better than they were. I am about to have my second V-beam treatment. It has taken 2.5 years, but I am getting better at dealing with and managing the condition. I do hope that there is more information in the coming years about the gut/skin connection and what lifestyle changes may be helpful in that regard.

Roseaca sufferer here. The only two cleansers I have found that work for me are the 1) Aveeno Calm and Restore (in the clear cylinder bottle) and 2) Geek and Gorgeous Jelly cleanser. The Aveeno leaves my skin just good not quite moisturized but good. If I only used that cleanser, I'd think I had normal skin. It's my first choice. The Geek and Gorgeous leaves my skin just skin (not stripped or dry or moisturized; just skin) but I can tell within about 30 minutes that my skin really is dry and needs hydration and moisture.
failures for me: La Roche Posay Toleriane Hydrating Gentle Face Cleanser (stings), Glow Recipe Avocado Ceramide Moisture Barrier Cleanser (left my skin feeling dry and tight), Hydrating Facial Cleanser (skin felt stripped and tight while using, let alone after rinsing), Fresh Soy Hydrating Gentle Face Cleanser (stripped and tight), Krave Beauty Matcha Hemp Hydrating Cleanser (same result as Glow recipe; fine while using and seemed good but left my skin feeling dry and tight in a bad way). *I have not tried the reformation from Cetaphil so I don't know how it works for me (but it wasn't good in the past).

I am Native American, dark skin tone, my parents never got rosacea, so guess I am the lucky one of 3... was diagnosed in 2017 in im mid 50's. I attribute it to that time in my life and all I can think of is stress related... I spent many years in the sun when I was a young girl playing softball, at the lake or beach, then after had 2 sons who were 3 sport athletes, so in the sun during the week and tournaments on the weekend. My mother was always in the sun as well, she had a beautiful complexion until she passed in 2019. My rosacea is not so much of the uncomfortable flushing, some redness, but the bumps are bothersome that are mostly on my forehead and right cheek near my nose. Before rosacea I used the "Calm line of Dermalogica" cleanser, very nice, cream based and this oil based booster serum that they no longer make was clutch. I found it on Amazon, so I will be ordering online. As far as sunscreen, never thought to apply it, it wasn't a thing when I was growing up, but I keep seeing the tinted Elta MD in other vids, so I will try that out first. Thank you for your story, Dr Sam ❤ Much love from the Bay Area of Cali

I was diagnosed with rosacea acne over 10 years ago and some of my acne is also hormonal which is SO FUN :) I'll say finally finding skincare products that worked for me and avoiding certain foods/situations has helped so much.
    - Neutrogena ultra gentle creamy cleanser - (hydrating cleanser creamy formula) I like this more than other creamy non-foaming cleansers because it has very few ingredients, no fragrance, no bells or whistles - I also use it on my body in the shower for my sensitive skin everywhere!
    - The ordinary 10% azelaic acid - love this during the day - it's pretty silicone heavy but my skin likes that as it almost acts as a protective barrier, absorbs oil, reduces redness, helps my bumps not get huge and angry, and it's so cheap!
    - curology
    - cerave moisturizing lotion - holy grail moisturizer head to toe for the past 12 years.
    - skin aqua super moisture gel - this sunscreen is spf 50 and is very liquid-y, but my skin likes that - the super thick, chauky sunscreens require lots of rubbing and tugging to rub it in, which exacerbates my rosacea. This sunscreen glides on and soaks in with hardly any rubbing.
    - food/situation triggers are: alcohol, stress, heat, super hot (temperature) food, tomato-based spicy foods (pasta sauces, salsa, etc.), dairy, gluten, whey, foods with lots of color dyes and super high sugar (candy and high processed high sugar foods), friction/lots of touching on my face

I got my rosacea diagnosis in April of this year. I use metrogel (topical for face) in the morning and 15% azelaic acid in the evenings.
My triggers seems to be stress, and the cold Canadian winters.
I had very bad pustule/pimples from my rosacea. The treatment worked, but unfortunately I was left with awful scarring. I did 2 rounds of IPL photo facial which worked with helping it not spread and have my redness lessen. My dermatologist then said my skin was in a state were I could undergo mirco-needling to help with the scarring. I will be going to my appointment this week so hoping I will have results. The only way I have been able to afford these treatments is by getting it done my students in training. They are always supervised by professional (as well as have prior training before doing it on someone's skin) so that definitely made me feel better. I have had no bad experience yet. I have rosacea and after seeing the dermatologist I gave up on big pharma options and started seeking my own treatments. I went to arabic store and bought rose water and I use that on my skin daily when I want to cleanse my face. I'll use it to dilute any essential oils I want to use on my skin and probably won't spend money on anything else because I can't think of a healthier option.

    Your skin is radiant!
I don’t have a major issue with redness, just a little on cheeks, nose and chin. But I DO have pimples in those areas, which IS a major issue. The typical rosecea treatments have not worked for me. Sulphur cleanser dries my skin and I’ve seen no improvement. Azelaic acid made my face sting and increased redness and pimples. Metronidazole cream is virtually worthless. I eat a clean diet, no alcohol, caffeine, or spicy foods, very little diary or sugar. I’m not out in the sun other than normal coming and going for errands. I haven’t been able to determine specific triggers, except the treatments! I’m very frustrated at this point. Just purchased a red light panel and hope to see some benefits from that, but only 4 days in and no difference so far.

Regarding Rosacea and the theory it is caused or worsened by demodex mites I‘d like to know your opinion on a certain skincare product called Demoderm. Main active substances are sulfor and zinc. It also contains menthol. This skincare product is supposed to detox the skin and doing so it is said it can lead to an intense worsening of the skin in the beginning as part of the detox process. The skin is supposed to heal itself eventuall so that - according to the Demoderm-thoery - the skin doesn‘t need any skincare at all and should regulate itself. During that detox period it is said, that Demoderm is drying out the sebum in the skin so demodex mites would basically starve and die and the skin can recover. The pores on Rosacea-skin are referred to as Demodex-pores.
I‘d really like to know your opinion on these theories because I can‘t find any statement by a dermatologist, not on Google nor on RUclips and I would like to know if Demoderm is worth a try. Many Thanks!

Actually, I do believe in the Demodex mite theory. When I was diagnosed with rosacea almost 20 years ago now I used a cream I bought from China. I honestly don’t remember what it was called but it was very cooling but very drying and I used it for several months off and on and boy did my skin improve. It did get worse for about a month before it got better because they say the mites die off and cause your skin to get worse but then it eventually got better and better. I also took doxycycline a prescription antibiotic a couple months on a couple months off a couple months on for a year and that really helped. Since then I got rid of everything with fragrance I stripped down my routine to Vanicream face wash, and Vanicream lite lotion and sunscreen. My skin likes chemical and now I use Korean sunscreen. All these years later, there is no trace of rosacea. It has never returned. I stick with minimal skin care. I do use Tretinoin at the lowest percent every night I use Azelaic acid occasionally . I avoid my triggers which are spicy foods and red wine.

Hello 😊! I was diagnosed with the Type 2 Rosacea. I’ve been trying to get it under control for approximately 2 years. It has been quite challenging living with this condition. I won’t get into the details…. I’ll just tell you what has helped me get it under control. Initially, I had severe flushing that seemed like it just wouldn’t let up no matter what I did or didn’t do. So right now I’ve just given up on the most common triggers that are talked about. I ve never really been a big wine drinker but I did enjoy a cocktail or two when we would go out to a restaurant or most social occasion. At this time, I just choose to stay away from alcohol. So, long story short my dermatologist has tried numerous different products for my skin and also oral medications as well as IPL and the red light laser. It’s sometimes difficult to know exactly which products are working because it seems as though none of the products or lasers have that immediate serendipity moment. But over time we’ve found that what works best for me is just washing my face 1-2 times per day with warm tap water. The only product I currently use is Winlevi cream, and I really can only use a scant amount. Otherwise it just will make my skin feel very dry especially over a long period of time. I also find that showers are better for me at just a nice warm temperature rather than sitting in a warm bath!!😮 I’m still learning every day what to use to decrease the breakouts. I also really love love Epicerum lotion and I do use Elta MD as far as any lotions. I still have to be careful with the lotions because the skin is extremely sensitive. I’m still experimenting with my diet because I’ve learned that many advisors feel that Rosacea could be related to any gut issues. I’ve read some articles that say if you have Irritable bowel syndrome that it could have an impact on the Rosacea. Also, as much as possible I’ve cut back on some of my prescription medication and I’m just taking the absolutely necessary meds and a few vitamins and minerals. I live in Florida so naturally I do carry my wide brim hats with me, sunscreen, umbrella and always a large cold bottle of water. This may be going over board but I have this unconscious or conscious?? fear of running out of gas in this hot and humid weather 😂. Not really funny! So I’m very conscious of wear my gas gauge is and I never let it get below half full before I fill it up. And of course I have road side assistance. 🎉😢
It’s unfortunately, a bit of a challenge but I’m at least just trying to find what works best and keep trying to avoid dryness, stinging, flare ups and itching. I’m sure there’s something I’ve left out. I hope this has at least helped someone else out there who is maybe new to this disorder. Good luck! 😊 Thanks for your information!💯

I had really really bad rosacea- my derm said I had one of the worst cases he’d ever seen! It took a really long time for me to seek treatment as I just thought it was skin sensitivity and acne….yeah I was wrong and I was making my skin worse. I was on various different antibiotics for around 3 years or so, which worked really well in the beginning and then my skin kind of leveled off to where my skin was still kinda of red, but the bumps and skin sensitivity were gone. Finally just a little over a year ago my derm put me on a different topical treatment- one of them has azaelic acid as well- and it’s worked so well for me. I use cereve for my cleanser and double cleanse before bedtime, as well as use sunscreen. For my rosacea chemical sunscreens can irritate myskin so I tend to stick to chemical ones.

I, personally, like to "Double Cleanse" by using a cleansing balm first (Clinique Take the Day Off Cleansing Balm - the regular one, not the charcoal one). It will remove all makeup and sunscreen. Apply it to dry skin and then when you add water, while rinsing, it emulsifies so that it doesn't leave a greasy film on your skin. Once it's rinsed, I use my "regular" cleanser to make sure all the sunscreen and makeup is ;thoroughly removed. If I don't double cleanse, I don't feel like the sunscreen is all off. @@Emerald-u4m

I have Type I Erythematotelangiectatic Rosacea and have had it for years. Just signed up for my first IPL Laser treatment this August. I've changed my skincare regimen and have begun incorporating an azelaic acid derivative to condition my skin before I start using my prescription cream. I've also become very regimented about caring for my skin by using better, more hydrating products and largely sticking with fragrance free lines (I have a few guilty pleasures though I can't lie LOL). I've never used retinoids of any kind as I'm afraid of the retinization process (hate the feeling of peeling, red skin)
As for triggers, I hate sunlight anyway and keep myself covered at all times when outdoors during the daylight hours (which is almost constant this time of year in Alaska) and keep my home cooler than most people will. I like wine but have significantly cut down my intake. And though I hate the way it feels, I've forced myself to incorporate sunscreen (mineral) if I am going to be outdoors during peak daylight hours (always wear a giant visor and giant Rx shades regardless along with keeping my body covered in UV protective materials).
A major trigger for me is stress and light sensitivity (light eyes, mydriatic pupils). As soon as I'm upset or stressed about something I feel the heat rising to my face and the flushing sensation. In winter because stores keep the heat so high, I've actually been asked if I had a sunburn by cashiers because I turn so red. It's horrible and I'm sick of it. I have fair skin and would rather just stay pale than this red crap. I can thank my NW Euro ancestors for this LOL
Thankfully I don't have the pustules or swelling but I do have the redness and visible capillaries. Rosacea sucks lol
My skin is starting to look better. Plus because I've always stayed out of the sun, I have no wrinkles and very few lines at age 56. Moreover, I am now lacto-ovo vegetarian (seafood too at times) which means I'm eating much better.

@@liviusss that’s why I mentioned getting a second opinion with another doctor. I realize that there are various things that cause rosacea, but you would expect a dermatologist to know what they are and try different treatments.

For me it was diet that got rid of my rosacea. Many years of being sold different products by dermatologists that did absolutely nothing ( these drs just making $). I even asked if diet could be the problem? And was told to just not eat spicy foods and no coffee. Eventually a relative introduced me to keto, which lead me to ketovore/ carnivore which basically removes most inflammatory foods. I now do not have rosacea flare ups and my skin no longer sunburns as it used to. All that inflammatory food had my skin very sensitive.
I too only cleanse at night and rinse with water in morning 😊👍🏻

Such an informative video! I was diagnosed with rosacea about a year ago. I first noticed a pronounced redness to my cheeks in 2020. As a healthcare worker, I attributed this to the necessity a that time of wearing a mask 8-10 hours per day at my job. Fast forward to 2023, the redness and now pimples were still there. Glad to know some of the options available for rosacea maintenance. I've found the azelaic acid 15% topical to work wonder, also.

Just at the beginning of the video. Although I don't envy that you have struggled with rosacea, my mom's rosacea was much worse than yours! It was visibly worse, deeper red and all over, not just on the cheeks but also she said her skin would get hot and hard. She's allergic to so many things. When she would eat something she was allergic to (tomatoes so no Italian food and sesame so no Asian food with sesame oil in it) her rosacea would flare up. Thankfully, in a weird way, she was on the Atkins diet for a while and ate bacon and other fatty foods so she ended up with a lot of gall stones and had to very painfully pass a gall stone. She had to have her gall bladder removed which ended up clearing most of her rosacea! She can now enjoy some of the things she's allergic to as long as she doesn't over do it.

I typically have a mild rosacea and use the Azelaic Acid 15% in the morning and then metronidazole cream .75% in the evening along with my Retin A and I am happy with the results. That being said last night coincidently I had an evening meal of cod fish with an Asian sauce and about an hour later I had a very bad flare up on my cheeks. I had to apply cold compresses and Cortisone cream to calm everything down. I woke up this morning and all is well so I guess something in the sauce caused this flare up. I would love to know what does your skin look like after the laser treatments. That would be very interesting. I once had and it looked like someone had put a cigarette out on my skin for about 7 days. Please let us know and such great content, thank you.

Same, I also use azelaic acid, metronidazole cream and tretinoin as well. For makeup removal I use Farmacy Clearly Clean or micellar water and Ren Evercalm cleansing milk. Products I can’t use that others have loved: Anything from CeraVe(burns like a thousand bees), lactic acid/peptide products. The way peptides work by triggering your skins “trauma/repair” response is too much for my skin and sends it into overdrive. I also do only iced coffee/tea and keep a big cold pack in my fridge and apply to my face after I get out of the shower. That has helped a lot as far as heat triggers.

Indoor heating is the worst. I run hot and i turn into a tomato when I'm in other people's houses. I don't need anything above 18 or 19 degrees C inside. Sadly can't cut out alcohol because i don't drink already (doesn't taste good to me). I have never washed my face as a daily routine because I don't wear makeup (or sunscreen, which i probably should). I started applying moisturizer (only after I shower, so every other day) because of dry skin a few years ago. I sued effaclar H by La Roche Posay because of dry skin, but they changed the recipe I think, and now my face turns red when I use it

I've tried many products over the years and nothing seemed to make my skin happy other than selected products of Clarins, Clinique and Avene. I use Clinique Take the Day Off cleansing balsam as the first step of dubbel cleansing which cleans all the oil solubles including the sunscreen. For the second step, I use Clarins Soothing Gentle Foaming Cleanser (pink one with shea butter). Clarins also have calm-essential restructuring oil which helped me a lot. Because of extreme dryness, my skin doesn't absorb the products well, to hydrate better I use Gulsha rose water, as micellar water before applying my moisturizer. During the day, I use the Moisture Surge facial spray by Clinique multiple times to keep my skin moisturized. I use SPF day creme and night creme from one of the brands I've mentioned above based on the climate.
In general shea butter and rose water were bets for my skin and I need to avoid Aloe Vera at all costs.

I am beginning to suspect that I have rosacea. I cannot use many moisturizers I tend to have many facial cleansers that I cycle around to keep from getting allergic to them. I get swelling and then my face peels. Many times I don't know why but I tend to become allergic to things I have used for a long time.

The thing I hate about the avoid triggers advice is that my triggers are literally unavoidable. My rosacea flares up terribly any time I shower (even just luke-warm body showers), and when I eat: meat, dairy, anything acidic, alcohol, caffeine, spicy food, or anything hot. You geard that right, food that is WARM. I cannot live off of cold plain vegetables and never shower.
